Since hands are at the front line of human contact, a high percentage of thermal injury involves the upper extremity and, in particular, the hand. Hand burns can vary in severity from shallow burns requiring local wound care and aggressive range of motion therapy to complex wounds requiring repair of joints, tendons, and other soft tissue. Historically, given the poor survival associated with severe burn injury, many patients with hand burns simply did not survive the acute phase of treatment. If patients survived the systemic insult of burn injury, often the hands were neglected relative to more extensive areas on the trunk. However, with the widespread use of early excision and grafting, as well as great advancements in critical care, survival following thermal injury has become the rule rather than the exception. Therefore, emphasis in burn care has shifted towards optimizing the functional and psychosocial outcomes of those that survive their injury. Accordingly, optimal management of hand burns has received increasing attention given the critical importance of hand recovery to long-term outcome. In this volume of Hand Clinics, experts in burn care present an overview of pediatric and adult hand burn management - including shallow burns, as well as complex injuries from deep thermal burns or electrical injury. In addition, chapters on pathphysiology of scar, burn hand rehabilitation and assessing outcomes of hand injury emphasize critical concepts in achieving optimal hand function after injury.

This is due to their anatomical, functional and aesthetic characteristics. Surgical treatment is a priority in these types of burns, especially on the dorsum of the hand and fingers.MethodsEarly tangential excision is the method of choice for treating deep dermal and subdermal hand burns. In these cases surgery yields considerably better functional and aesthetic outcomes, as opposed to conservative therapy. Consequently, hands are given priority over other parts of the body in surgical interventions.Physiotherapy and rehabilitation play a very important role in the treatment of hand burns. Functionality of the hand is of paramount importance for the quality of life of burn patients. Some general rules apply for the treatment of hand burns, i.e. an individualised approach, careful patient selection and operative therapy based on the principles of the reconstructive ladder.ResultsPatientu2019s psychophysical state and participation are undoubtedly important; they have to be monitored in addition to providing the patient with objective information regarding their treatment. The treatment should be opportune and carefully planned in regard to the expected timeline as well as to the prediction of the end result. Postoperative follow-up is lengthy, several operations may be required and the rehabilitation programme completes the whole treatment.ConclusionGuidelines and experience occasionally differ. The former primarily protect logistic and forensic facts, while the latter expresses operative, surgical and patientu2019s outcomes. Nevertheless, hand reconstruction, be it primary or late, should abide by the protocol without sudden changes or improvisation, while being in accordance with the expected timeline and burn scar maturation.

In high-voltage burns, the injury associated with deep extension and underlying tissue damage, resulting ischemic tissue and eventually, higher possibility extensive necrotic tissue The importance of nonvital tissue debridement and adequate defect closure in electrical burn injuries determine the outcome of limb salvage.Method:A case series of electric burn injury extremity with two stage of reconstructive surgery using groin flap. Author questioned clinically whether two-stages surgical method affect morbidity of electric burned patients and relation ship between type of harvesting flap (delayed and non-delayed groin flap) interferes flap vitality.Result from PubMed, Burns Journal and Googleu2122 search generated 53 articles. Screening articles based on inclusion criteria, exclusion criteria, and full text reading. Three remaining articles appraised in regard of validity, importance, and applicability.Result: Time of reconstruction did not appear to affect flap survival and outcome. Fasciocutaneous flaps also relatively thin and, therefore ideal for coverage in regions with exposed tendon and bony structure. No evidence found from world literature whether type of flap harvesting will affect flap vitality.Conclusion: The use of groin flap as distant pedicle flaps, provide alternative where in case of free flap reconstruction was not be able to do. The possibility of compromised does not related with timing of reconstruction surgery, since pathophysiology of vascular occlusion and progressive necrosis tissue in electric burn injury. Pliable characteristic for covering small cross sectional diameter anatomic region such as wrist is advantageous, however, due to bulkyness appearance, secondary reconstruction will be needed for thinning flap.
