Cindy Forster reveals the critical role played by the poor in organizing and sustaining this period of reform. Through court records, labor and agrarian ministry archives, and oral histories, Forster demonstrates how labor conflict on the plantations prepared the ground for national reforms that are usually credited to urban politicians. She focuses on two plantation zones that generated exceptional momentum: the coffee belt in the highlands around San Marcos and the United Fruit Company's banana groves near Tiquisate. Although these regions were unlike in size and complexity, language and race, popular culture and work patterns, both erupted with demands for workers' rights and economic justice shortly after the fall of Castañeda in 1944. A welcome balance to the standard "top-down" histories of the revolution, Forster's sophisticated analysis demonstrates how campesinos changed the course of the urban revolution. By establishing the context of grassroots mobilization, she substantially alters the conventional view of the entire revolution, and particularly the reforms enacted under President Albenz.

Babies who are breastfed are less likely to become overweight and obese. Many mothers in the United States want to breastfeed, and most try. And yet within only three months after giving birth, more than two-thirds of breastfeeding mothers have already begun using formula. By six months postpartum, more than half of mothers have given up on breastfeeding, and mothers who breastfeed one-yearolds or toddlers are a rarity in our society. October 2010 marked the 10th anniversary of the release of the HHS Blueprint for Action on Breastfeeding, in which former Surgeon General David Satcher, M.D., Ph. D., reiterated the commitment of previous Surgeons General to support breastfeeding as a public health goal. This was the first comprehensive framework for national action on breastfeeding. It was created through collaboration among representatives from medical, business, women's health, and advocacy groups as well as academic communities. The Blueprint provided specific action steps for the health care system, researchers, employers, and communities to better protect, promote, and support breastfeeding. I have issued this Call to Action because the time has come to set forth the important roles and responsibilities of clinicians, employers, communities, researchers, and government leaders and to urge us all to take on a commitment to enable mothers to meet their personal goals for breastfeeding. Mothers are acutely aware of and devoted to their responsibilities when it comes to feeding their children, but the responsibilities of others must be identified so that all mothers can obtain the information, help, and support they deserve when they breastfeed their infants. Identifying the support systems that are needed to help mothers meet their personal breastfeeding goals will allow them to stop feeling guilty and alone when problems with breastfeeding arise. All too often, mothers who wish to breastfeed encounter daunting challenges in moving through the health care system. Furthermore, there is often an incompatibility between employment and breastfeeding, but with help this is not impossible to overcome. Even so, because the barriers can seem insurmountable at times, many mothers stop breastfeeding. In addition, families are often unable to find the support they need in their communities to make breastfeeding work for them. From a societal perspective, many research questions related to breastfeeding remain unanswered, and for too long, breastfeeding has received insufficient national attention as a public health issue. This Call to Action describes in detail how different people and organizations can contribute to the health of mothers and their children. Rarely are we given the chance to make such a profound and lasting difference in the lives of so many. I am confident that this Call to Action will spark countless imaginative, effective, and mutually supportive endeavors that improve support for breastfeeding mothers and children in our nation."--Page v.

Previous studies have found that positive breastfeeding outcomes are associated with fathers providing any type of supportive behavior. Above other types of support, responsive behaviours sensitive to the mother's needs are more likely to increase exclusive breastfeeding significantly longer. This study tested the feasibility of an intervention that provided a one-time 60-minute session to an all-fathers group about the breastfeeding team and the effect of the fathers' satisfaction with the education in terms of fathers' breastfeeding support behaviours. This study was carried out in a maternity hospital in Canada where an average of 1,500 infants are born each year. Twenty fathers from a variety of ethnic backgrounds received this learning. The group was led by a male facilitator as some research suggests that men prefer to learn from their male peers. The facilitator was trained using the Father Breastfeeding Involvement Group: Facilitator Manual. The average session lasted approximately 60 minutes with an average group of two participants. The fathers discussed ways they could practice responsive behaviours unique to their partner and infant as an active member in the breastfeeding team. The fathers reported high satisfaction rates of attending the session and that the session created a comfortable atmosphere to share their thoughts openly. Second time fathers highly enjoyed the session finding it a valuable, informative refresher. The fathers liked the convenience of attending a session in hospital before their infant was discharged home. At one month, fathers completed the Partner Breastfeeding Influence Scale to determine the amount of specific involvement they practiced. There was a positive relationship between fathers' overall satisfaction with the session and reports of greater father breastfeeding support behaviours. One limitation of this study is the small sample size. Data collection was interrupted by the COVID-19 pandemic because nonessential research was abruptly stopped within a few weeks of the program's commencement. However, the fathers were interested and enjoyed the session, providing evidence that a larger number of participants was feasible. This knowledge can be used in planning future father involvement in breastfeeding interventions.

Book excerpt: The participation of fathers at the earliest stages of child carework can be somewhat undistin-guished when mothers handle all of the feeding carework through breastfeeding, but a possible solution to this gender inequity could be to encourage male breastfeeding for fathers. In this study, ten fathers who have a child living with them no older than four years of age were inter-viewed. A quasi-experimental design was used to examine their thoughts on male breastfeeding before and after they watched an edited version of the film Milk Men and read a handout about facts related to the possibility of male breastfeeding. The findings were examined using a post-modern feminist approach. Fathers had entangled ideas about masculinity and the body or nature that negatively affected their thoughts about the practice, though only one of the fathers overall rejected the idea that men could or should breastfeed. Many of the fathers showed their re-sistance to the examination of male breastfeeding by tangentially exploring body ownership ideas and using other deflecting strategies. Subverting social norms, three of the fathers were proponents of the idea overall and six of the fathers were on a continuum of willingness to en-gage the idea. The positive aspects of male breastfeeding that spoke to some of the fathers were the bonding aspect this type of child carework could involve and the chance for male breastfeed-ing to lead to more equity among mothers and fathers.
