Book excerpt: Since Locher first suggested Boron Neutron Capture Therapy (BNCT) in 1936, this theoretically ideal system has intrigued investigators. Unfortunately, the first clinical trials between 1951-1961 were not successful. However, they served to implant firmly the seed of BNCT, the growth of which has been carefully nurtured at a number of locations world-wide. This fact is attested to by the ongoing clinical trials in Japan as well as by the presence of researchers from active groups in the ten countries represented at this Workshop. In 1983 and 1985, the first and second international biannual symposia on BNCT were held, in response to a resurgence of interest in this field. In 1986, the DOE sponsored a workshop on NCT, in large part directed toward evaluating the national effort and the various neutron sources available within the United States. It now seems likely, because of various factors including improved neutron beams and boron delivery systems which have made the modality more attractive, that clinical trials will be initiated in the United States within the next few years. This 1988 special workshop, interspersed between the biannual international symposia, represents an effort to seek ideas and advice on the clinical a~pects of BNCT, from all those diverse groups with a national commitment to this project. Our purpose is to facilitate our endeavor to incorporate the best procedures and techniques in the upcoming clinical trials.

We have been investigating BNCT applications in various types of malignant brain tumors, especially malignant gliomas, as medical applications. Recently, we have conducted clinical trials using the developed accelerator neutron source. Research on pharmaceutical applications of compounds that transport boron to cancer cells is expected to be in even greater need. Currently, the only boron agent used in cancer therapy is BPA (Borofaran 10B), which takes advantage of the demand for essential amino acids, but the research and development of boron agents are an absolutely key technology to further improve the precision of this treatment modality. This chapter summarizes and discusses the results of BNCT in the treatment of brain tumors.

Book excerpt: Despite the many advances made in the diagnosis and therapy of cancer, the mortality rate is still about half that of the incidence rate. However, the odds are not evenly distributed. Prognosis for some cancers is good, but for others, few patients will survive 12 months. This latter group of cancers is characterised by a proclivity to disseminate malignant cells in the host organ. The degree of surgery possible may be limited by the critical nature of the organ, and chemotherapy and radiotherapy are of palliative value only. In some cases systemic metastases occur, but in other cases, failure to achieve local control results in death. First among these cancers are the high grade brain tumours, astrocytoma 3,4 and glioblastoma multiforme. Local control of these tumours should lead to cure. Other cancers melanoma metastatic to the brain, for which a useful palliative therapy is not yet available, and pancreatic cancer for which localised control at an early stage could bring about improved prognosis. Patients with these cancers have little grounds for hope. Our primary objective is to reverse this situation with Neutron Capture Therapy (NCT). The purpose of this fourth symposium is to hasten the day whereby patients with these cancers can reasonably hope for substantial remissions. The first symposium on NCT was held in Boston in 1983, followed by Tokyo in 1985 and Bremen, Germany in 1988.

In BNCT, a compound labeled with the stable isotope boron-10 is systemically administered, and tumor cells selectively uptake the boron-10 containing compound at higher concentrations than normal cells. A general problem with the tumor seeking compounds is that drug delivery is dependent upon sufficient vascularization within the tumor. To investigate the possibility of delivering boron to hypoxic regions of tumor, a new boronated nitroimidazole delivery agent has been synthesized as a carrier of boron-10 for BNCT. It is expected that this will be used in combination with the existing boron carrier boronophenylalanine-fructose to treat solid tumors. An immunohistochemical protocol to visualize hypoxia was tested and refined to confirm the suitability of two tumor models established in the lab for hypoxia related uptake studies. The immunohistochemical protocol is used to detect pimonidazole, which localizes at hypoxic regions in tissue and is the parent compound for the new hypoxia-selective boron carrier. The protocol was used to test and confirm the suitability of a hypoxic in vivo tumor model. Two tumor lines were tested: SCCVII squamous cell carcinoma and EMT-6 murine mammary carcinoma. Both exhibited hypoxia. Finally, quantitative studies using Inductive Coupled Plasma Atomic Emission Spectrum demonstrated that the synthesized boronated nitroimidazole reaches suitable concentrations in SCCVII and F98 tumor. Future therapeutic studies are required to empirically confirm the effectiveness of this compound.

In this type of therapy, a nuclear reaction is employed to kill cancer cells. When boron-10 is irradiated with thermal neutrons, the result are highly reactive alpha particles and lithium-7 nuclei, both with a very short path length. When boron-10 is selectively internalized into malignant cells, the reactive nuclei after neutron irradiation will selectively kill the malignant cells. However, research on boron compounds is still necessary (on selectivity for example), which involves experiments in vitro. The aim of this research is the development of a method for the determination of boron in cells exposed to boron-containing molecules in vitro. For the quantitative determination of boron, inductively coupled plasma mass spectrometry (ICP-MS) was employed. However, when using this device, it is important to first completely dissolve the cells and their content and to remove carbon to minimize matrix effects. In addition, during this sample preparation, complete recovery of boron is envisaged, which is difficult because of the volatility of boron. Because low boron concentrations will be determined also a low background signal of boron, which implicates the prevention of contamination with boron during sample treatment, is important. Sample preparations based on acidic digestion and UV digestion were performed on cell suspensions. These were evaluated with total organic carbon and ICP-MS measurements. From the results of these analyses, it was concluded that the ideal method is a short sealed acidic digestion combined with UV digestion. In this way, low carbon contents, low detection limits and high recoveries of boron can be obtained. Employing this method, it was possible to quantitatively determine associated boron in exposed cells. The result is important for future in vitro BNCT research, but it may also prove its importance for ICP-MS elemental analysis of cells.
