Book excerpt: Type 2 diabetes (T2DM) currently affects more than three million African American adults with double the number expected by 2025. The most effective and safest treatment for T2DM is lifestyle change therapy, including healthful eating, monitoring of blood glucose, and physical activity. However, current lifestyle change interventions are limited in their scope to alter the behaviors of individuals to more healthful ones. These limitations may be attributed, in part, to a lack of attention given to enhancing an individual's psychosocial process variables, such as resilience, coping skills, selfleadership, and empowerment. Incorporating resilience education into lifestyle change therapies is a novel approach that addresses the behavior modification limitations of current interventions by aiming to enhance psychosocial process variables. Therefore, the purpose of this project was to conduct a six-month pilot study to determine the feasibility of our resilience and diabetes self-management intervention, The Diabetes Coaching Program: Transforming Lives Through Resilience Education, in a convenience sample of African American adults (n=16) with T2DM. The intervention included four weekly resilience and diabetes education classes and eight bi-weekly support group sessions. Survey data and blood samples were collected at baseline and at six months. Twelve participants completed the study (75% retention). Results indicated that higher perceived stress scores were associated with less resilience, fewer adaptive coping skills, lower selfleadership, lower diabetes empowerment and greater depressive symptoms. However, diabetes empowerment was the only psychosocial process variable to be significantly enhanced by the intervention at six months. Weight, BMI, HbA1c, total cholesterol, LDL cholesterol, blood pressure, and IGF-1 levels were significantly decreased at six months, whereas, lymphocyte proliferation and physical activity were significantly increased. These data indicate that our intervention has the potential to improve diabetes selfmanagement among African Americans with T2DM and increase positive health outcomes, though further studies are needed to confirm these findings. Additionally, several lessons were learned from conducting the pilot study that may be useful for improving the intervention for future studies, including: recruitment and retention strategies; cultural competency issues; the use of complementary and alternative medicine practices by African Americans with T2DM; and approaches for increasing participant self-assessment and goal-setting.

Social influences are commonly associated with diabetes-related outcomes. Purpose: The purpose of the study was to assess whether and how socio-contextual factors influence self-management and emotional distress in urban African American adults living with type-2 diabetes, as well as to assess whether social support facilitate relatedness need fulfillment within this population. The study also sought to gain an understanding of the psychosocial support needs and preferences of urban African American adults living with type-2 diabetes. Methods: The descriptive study employed a mixed-methods research design. African American adults were recruited to take a one-time written survey. Hypotheses one and two were tested using hierarchical linear regression, hypothesis three was assessed using VI a simple linear model, and hypotheses four and five were evaluated using path analysis. Four gender-stratified follow-up focus groups were also conducted, to confirm quantitative findings and gain insight into the support needs and preferences of urban African American adults living with type-2 diabetes. Study Variables: Independent study variables included social support, autonomy support, basic psychological needs (autonomy, competence, relatedness), and autonomous regulation. Primary dependent variables were diabetes self-management and diabetes-related emotional distress. Results: 155 eligible individuals (67 male, 88 female) were included in the quantitative analytic sample. 23 individuals (10 male, 13 female) participated in follow-up focus groups, yielding a total of 6 hours of audio-recorded focus group data. Autonomy and competence significantly predicted self-management and emotional distress in hypotheses one and two, respectively. Competence was the strongest predictor of selfmanagement and emotional distress among sample participants, followed by autonomy, but relatedness was not observed to be a significant variable in the prediction of study outcomes. Social support was observed to significantly predict relatedness need fulfillment in a simple linear model for hypothesis three. Data did not support hypothesis four and five. Qualitative findings contextualized the quantitative study findings. Conclusions: Future support programs should facilitate basic need fulfillment in urban African American adults living with type-2 diabetes. Relatedness is especially important in this collectivistic cultural group.

Diabetes is the leading cause of blindness, kidney failure, and non-traumatic lower limb amputations. African Americans are more likely to suffer from diabetes related complications than any other race. The purpose of this study was to test a six-week culturally tailored diabetes self-management program for African Americans. The diabetes self-management intervention was conceptualized as the foundation that supported the changes in diabetes knowledge, self-efficacy, symptom management, and self-management activities. Thus, the specific aim of this study was to increase knowledge about diabetes, self-efficacy, symptom management, and self-management activities in African American adults diagnosed with diabetes. A one-group pre- and post-test was the design used in this study. The six-week intervention focused on areas of diabetes self-management based on the American Diabetes Association (ADA) guidelines: (a) introduction to diabetes, (b) healthy eating, (c) being active, (d), medications, (e) glucose monitoring and complications, and (f) symptom management. An African American nurse led the intervention. Adult African Americans with diabetes who were age 18 years old and older were recruited from two predominantly black churches in the south-eastern part of the United States. Sessions were held at each church directly after the morning and evening worship services. A total of 32 participants were recruited and 28 participants completed the six-week program. Health history and biomarkers (BMI, HBA1C) were measured at baseline. The major concepts were measured at baseline and post intervention using the Spoken Knowledge in Low Literacy Diabetes (SKILLD) scale, Diabetes Empower Scale- Short Form (DES-SF), Diabetes Symptom Checklist- Revised (DSC-r) scale, and the Summary of Diabetes Self-Care Activities (SDSCA) scale. At baseline, many person had low diabetes knowledge, self-efficacy, symptom management, and diabetes self-management activities. Findings revealed statistically significant positive improvements in diabetes knowledge, self-efficacy, symptoms management, and diabetes self-management activities from pre-intervention to post- intervention. Study participants gained an awareness of the need or diabetes self-management education. Designing and implementing culturally tailored diabetes self-management programs in faith-based settings are practical and necessary to reduce the disparity in diabetes for African Americans. More diabetes self-management studies are needed that target, recruit, and retain an adequate representation of African Americans. There is a definite need to better understand the relationship of psychosocial variables to diabetes control in these two racial groups. The purpose of this two-group, comparative, descriptive study was to examine the relationships of social support, self-efficacy, and outcome expectations (beliefs that diabetes-related behaviors will lead to certain outcomes) to self-care behaviors and glycemic control in Caucasian and African-American adults with type 2 diabetes. A convenience sample of 91 Caucasian and African-American subjects receiving health care at three outpatient facilities participated in the study. All subjects completed four self-report measures: Social Support Questionnaire, Self-Efficacy Questionnaire, Outcome Expectancy Questionnaire, and the Diabetes Activities Questionnaire at the time of the outpatient visit. Long-term glycemic control was assessed by glycosylated hemoglobin analyses at the time of the outpatient visit. Approximately 1 in 10 adults in the United States have diabetes and a significant majority have inadequately controlled blood glucose, blood pressure, and cholesterol. Previous research shows that many adults with diabetes lack the knowledge, skills, behaviors, and support to properly manage their condition. Diabetes self-management education has become an important part of diabetes care guidelines as a way to improve the self-management of adults with diabetes in order to lead to improve patient health and reduce inappropriate health care use and medical costs. Despite this, it is unclear who is actually receiving diabetes self-management education in the US, overall and by mode of delivery and diabetes educator type, and whether education is associated with improved population health. Therefore, the objective of this dissertation research is to investigate the distribution of diabetes self-management education, patient sociodemographic factors that influence the receipt of that education, and its contribution toward diabetes outcomes, medical expenditures, and healthcare use in US adults. We achieved this research objective by completing three specific research aims. We used nationally representative data from the Medical Expenditure Panel Survey and the National Health and Nutrition Examination Survey. We found that although a substantial majority of individuals received some form of diabetes self-management education, receiving diabetes education was associated with race/ethnicity and income (Aim 1), and varied by mode of education delivery. Individuals who received diabetes self-management education were less likely to have glycemic control but were more likely to receive antiglycemic treatment (Aim 2). Receiving diabetes self-management education was associated with greater diabetes-related ambulatory and prescription use, depending on the mode of education delivery (Aim 3). Our findings highlight some strengths of the current state of diabetes management occurring in usual care, including the widespread delivery of some form of diabetes education and its association with greater ambulatory and prescription use. However, we also found evidence of the need for more effective diabetes education delivery systems in usual care in order to improve patient self-care behaviors and reduce inpatient and emergency room use.

Diabetes self-management education (DSME) is a cost-effective but underutilized intervention for patients (CDC, 2021). There have been few studies conducted to examine the effect of simulation as a patient education method. Purpose: This study aimed to examine the effect of a simulation-based educational intervention on diabetes self-care management. Methods: Subjects were adults diagnosed with diabetes with a glycated hemoglobin test (HbA1c) greater than or equal to eight, obtained within the past six months. The participants were randomized into the simulation or educational handout group. The instrument used was the Diabetes Self-Management Questionnaire (DSMQ), which assessed self-care interventions required for glycemic control (Schmitt, 2013). Both groups completed the DSMQ pretest and demographic survey, and then completed the DSMQ again at six weeks post intervention. Results: No formal comparisons were made due to low sample. The mean total DSMQ score was in 7.6 the simulation group and 5.9 in the handout group at pretest. The mean total DSMQ score was 9.1 in the simulation group and 7.5 in the handout group at posttest. Conclusions: The study suggested that immersive learning experiences may be effective in adults with diabetes, regardless of time since diagnosis. More formal education is needed for patients with type II diabetes, especially regarding nutrition. Further studies are needed to further explore the gaps on the use of simulation in patient education.

The results of this study supported the usefulness of Orem's Self-Care Deficit Nursing Theory (SCDNT) with adult African American diabetic populations and identified the need for further research to examine self-care in other African American populations.

Abstract: Objective: To systematically review the effect of self-management education on diabetic control in type 2 diabetes Research design and methods: PubMed was searched for English-language articles published between 2010 and 2013. All the studies were original articles selected manually and used randomized control trials generating results of self-management education in people with type 2 diabetes referring to diabetic control. Relevant data were divided and tabulated into factors of population characteristics, interventions and outcomes. Interventions were classified into three sections as collaborative information intervention, lifestyle intervention, and skills teaching intervention based on the patterns of education. Outcomes were categorized into glycemic control, cardiovascular disease (CVD) risk factors and nephropathy risk factors. Results: A total of 24 studies were identified of initial 41 articles for this review. Effects of self-management education on glycemic control were demonstrated to be positive both in short-term (10 months) follow-up, but more positive effects in short-term follow-up. The same effectiveness happens to CVD risk factors, including lipids, weight and blood pressure. On the other hand, with short-term follow-up, teaching skills intervention of self-management education is more effective than collaborative information intervention and lifestyle intervention on reducing glycemic control and CVD risk factors. Also with long-term follow-up, teaching skills intervention of self-management education had more effectiveness than collaborative information intervention and lifestyle intervention on reducing glycemic control. However, few studies including CVD risk factors in the long-term follow-up, so it is difficult to evaluate the effectiveness of on CVD risk factors with long-term follow-up. Conclusions: Evidences supports the positive effectiveness of self-management education with collaborative information intervention, lifestyle intervention and skills teaching intervention among type 2 diabetes patients on diabetic control, in both short-term follow-up and long-term follow-up, but short-term follow-up is more effective than long-term follow-up. Further research is needed to develop self-management interventions to maintain long-term follow-up effects on glycemic control, CVD risk factors and other diabetes complications. Diabetes self-management education has become an important part of diabetes care guidelines as a way to improve the self-management of adults with diabetes in order to lead to improve patient health and reduce inappropriate health care use and medical costs. Despite this, it is unclear who is actually receiving diabetes self-management education in the US, overall and by mode of delivery and diabetes educator type, and whether education is associated with improved population health. Therefore, the objective of this dissertation research is to investigate the distribution of diabetes self-management education, patient sociodemographic factors that influence the receipt of that education, and its contribution toward diabetes outcomes, medical expenditures, and healthcare use in US adults. We achieved this research objective by completing three specific research aims. We used nationally representative data from the Medical Expenditure Panel Survey and the National Health and Nutrition Examination Survey. We found that although a substantial majority of individuals received some form of diabetes self-management education, receiving diabetes education was associated with race/ethnicity and income (Aim 1), and varied by mode of education delivery. Individuals who received diabetes self-management education were less likely to have glycemic control but were more likely to receive antiglycemic treatment (Aim 2). Receiving diabetes self-management education was associated with greater diabetes-related ambulatory and prescription use, depending on the mode of education delivery (Aim 3). Our findings highlight some strengths of the current state of diabetes management occurring in usual care, including the widespread delivery of some form of diabetes education and its association with greater ambulatory and prescription use. However, we also found evidence of the need for more effective diabetes education delivery systems in usual care in order to improve patient self-care behaviors and reduce inpatient and emergency room use.

Diabetes is more prevalent in blacks compared to whites of all ages. The purpose of this project was to develop, implement, and evaluate a culturally competent, church-based diabetic self-management intervention program for African American women with type 2 diabetes. Twelve African American women with type 2 diabetes participated in a church-based education program. Pre- and post-tests were used to assess knowledge of diabetes. A three-month behavior diabetessurveyy was used to assess behavioral change after completing the education program. Post-test scores on the diabetes knowledge test changed from a baseline mean of 83.67 (SD= 10.48) to 97.08 (SD= 3.60). Theresultss indicate that with additional instruction and education, the participant's diabetic knowledge increased. Fifty percent (50%) of the participants made at least two behavioral changes on the 3-month behavioral survey. The project found that church-based interventions can be an important component of diabetes health education programs. Culturally competent approaches that integrate ethnic beliefs, values, and customs are beneficial when teaching diabetes education to African American women.

Book excerpt: The prevalence of type 2 diabetes continues to escalate, especially among vulnerable and underserved populations (CDC, 2008); however, the current landscape of literature has not kept pace with the trends of those most affected. Available data have often pertained to nonunderserved groups and only address part of patients' physical, mental, social, or spiritual lives. The literature review and research articles included in this dissertation integrate available knowledge pertaining to the comprehensive experiences of underserved groups with type 2 diabetes and specifically the understudied concept of satisfaction with life (SWL) among African American and Hispanic minority groups. The first article presented is a literature review written to synthesize studies of underserved populations with type 2 diabetes, including individuals who were of a minority ethnic group, socioeconomically disadvantaged, elderly, uninsured, and/or of lower educational level. The objective of this review was to amalgamate current evidence for underserved groups using a biopsychosocial-spiritual framework (Engel, 1977; Hodgson, Lamson, & Reese, 2007; Wright, Watson, & Bell, 1996). Themes and gaps in knowledge from approximately 1995 to the present were identified. Fifty-one studies were reviewed and grouped into three themes pertaining to overall health and disease management, including: psychological influences; marital, familial, and environmental influences; and spiritual influences. Researchers documented a number of psychological, social, and spiritual variables influencing glycemic control. In particular, depression and relational conflict adversely influenced glycemic control in some cases, whereas availability of social support or spiritual resources frequently assisted in better disease management. Lack of demographic data and attention to diverse groups, as well as, incomplete empirical knowledge prompt the need for further research of the influences of disease management among underserved groups. Three recommendations are suggested: 1) further exploration of the relationship between psychosocial-spiritual influences on diabetes management for uniquely underserved groups; 2) investigation of demographic differences or similarities; and 3) development and use of clinical models incorporating a multidisciplinary, collaborative stance in addressing diabetes for vulnerable populations. The second article extends current literature and aims to meet a number of the above mentioned recommendations. This article is focused on two groups with noted health disparities, African American and Hispanic minorities with type 2 diabetes. It was written to examine associations found between biomedical markers of disease management and psychosocial constructs and assess the correlates and predictors of variability for SWL. Data were collected from 142 minority patients (65 African American and 77 Hispanic) during their first visit within a collaborative care program. Pearson product moment correlations, multiple linear regressions, and one-way analysis of variance were used to examine research questions in this descriptive, quantitative study. Only one weak, significant relationship was noted for BMI and somatization; otherwise, no noteworthy relationships were documented between psychosocial or spiritual constructs and biomedical markers of disease management (HbA1c and body mass index). Psychological distress, social support, and spirituality accounted for 63% of the variance in SWL for African American participants and 29% of the variance in SWL for Hispanic participants. Social support was the strongest predictor of SWL for African American participants, while psychological distress was the strongest predictor of SWL for Hispanic participants. Social support moderated the relationship between psychological distress and SWL for Hispanic participants, while social support did not moderate this relationship for African American participants. Spirituality did not moderate the relationship between psychological distress and SWL for either ethnic group. Clinicians and researchers who work with and study minority patients with uncontrolled type 2 diabetes may see improvement in patients' SWL when screening for and treating psychopathology, interpersonal strain, and existential struggles. Attention to ethnic variation in research and clinical practice is important since dissimilarity existed in the determinants of SWL and moderators of psychological distress and SWL.

Book excerpt: Type 2 diabetes mellitus (T2DM) is a chronic and progressive medical condition that has reached pandemic levels, currently affecting 368 million people worldwide. With previous research demonstrating that T2DM is largely irreversible, the focus is now on developing and disseminating structured diabetes self-management education and support (DSME) programs to help patients live as healthfully as possible without exacerbating their illness. In recent years, several DSME interventions have demonstrated clinically meaningful changes in T2DM patients; however, there appears to be little translation of these programs from research into real world settings because these settings often lack clear blueprints for how to create or implement DSME programs that best fit their organizational needs and structure. Thus, the current study aimed to address this research-practice gap through the development of a brief, theoretically-driven, and behaviorally-focused group-based DSME curriculum for use in real world settings. Phase 1 of the study entailed concentrated efforts to join behavioral health researchers, community health leaders, and T2DM experts in combining the Common Sense Model of Self-Regulation (a prominent health behavior theory), cognitive behavioral principles, and current guidelines from the American Diabetes Association into a novel DSME curriculum. Phase 2 of the study then focused on implementing the resulting four-session curriculum, An Active Approach to Diabetes Self-Management, in local community centers and examining its early acceptability, feasibility, and effectiveness in producing clinical and psychosocial outcomes of interest. A total of 22 adults with T2DM, ranging in age from 35 to 87 years, were recruited from four community organizations and entered into four separate groups. Results indicate statistically significant improvements in participants' glycemic control, diabetes knowledge, and diabetes self-efficacy over the course of the study. In addition, results indicate that both participants and community liaisons were highly satisfied with the intervention. These findings contribute to the field's growing knowledge of how best to capitalize on clinical, research, and real world expertise to design thoughtful and practical DSME programs.

The purpose of this work was to explore relationships between diabetes distress, self-efficacy and resilience and outcomes of glycosylated hemoglobin, quality of life and health status. A cross sectional descriptive design was used for this pilot study of 78 individuals enrolled from an Endocrine clinic in the Midwest United States and a Primary Care clinic in the southeast United States. Data were analyzed using descriptive statistics to characterize the sample and model variables. Spearman's correlation was completed to identify relationships among variables. A stepwise building approach was used to identify significant interactions and determine predictors of the study outcomes. The results of this study confirm the presence of facilitators and barriers in type 2 diabetes mellitus self-management and their relationships with distal outcomes. The findings demonstrate that diabetes distress is a predictor of health status and quality of life. The findings of this study provide a link to other facilitator and barrier variables such as provider collaboration, diabetes self-management education, treatment regimen, ethnicity and years since diagnosis which can be incorporated into the comprehensive theoretical model. This study contributes to the understanding of the emotional aspect of diabetes as it relates to self-management of T2DM. Continuing this work will allow researchers to examine and better understand important factors of self-management. This ongoing work will hopefully lead to improved support in self-management efforts and better outcomes.
