Book excerpt: In this paper we analyze the impact of using a risk-adjusted comprehensive payment model, in support of In this paper we analyze the impact on total medical costs of using a risk-adjusted comprehensive payment model, to support a patient-centered medial home. We compare 2008-2010 insurance claims data on treatment and control practices from a network health plan in upstate New York. Practices in treatment group embraced a risk-based comprehensive payment model receiving risk-adjusted base payments and bonuses, compared with fee-for-service in the control group. We estimate the treatment effect using difference-in-differences, controlling for trend, payer type, plan type, and patient fixed effects. We weight to account for partial-year eligibility, use propensity weights to address differences in exogenous variability between control and treatment patients. Our estimation results suggest that medical spending in the treatment group appears to be 5.8 percent lower in year one and 8.7 percent lower in year two; the largest proportional two-year reduction in spending occurs in laboratory test use, 16.5 percent. We find that risk-based comprehensive payment model has notably dampened spending growth for the practices in the treatment group.

[...] For program participants in the first year of the program, there are statistically significant reductions of 25 per cent in the number of hospital visits and of 17 per cent in the number of emergency department visits, relative to what we observe in the age- and sex-matched controls. [...] This trend in per participant costs of the program is attributed to changes in the content and delivery of the program reflecting the experience gained through eight years of operating combined with the results of internal program evaluation and external research on the program.

Most assessments of health reform legislation have focused only on the federal budgetary impact. This study projects the effect of national reform on total national health expenditures and the insurance premiums that American families would likely pay. We estimate that the combination of provisions in the House and Senate bills would save $683 billion or more in national health spending over the 10-year period 2010 -- 2019 and lower premiums by nearly $2,000 per family. Moreover, the annual growth rate in national health expenditures could be slowed from 6.4 percent to 6.0 percent.
