Book excerpt: Hispanics are at increased risk of experiencing poor physical, psychological, and social health outcomes as a result of their diminished access to and use of healthcare services. Although disparities in access to medical care have decreased for all minority groups, they have risen among Hispanics. The objective of this study is to explore the different barriers Latino women experience in trying to access health care. The participants (N=10) in the study include Latina women, 18 years of age or older with limited English skills. An in-depth interview was conducted with each individual participant to understand the obstacles that prevent them from accessing medical care. The results revealed language, high cost of insurance, not having insurance, excessive waiting periods, and documented status are major barriers in accessing healthcare. This study contributes to the research within the Latino women in the healthcare system.

Book excerpt: As the population of older Americans grows, it is becoming more racially and ethnically diverse. Differences in health by racial and ethnic status could be increasingly consequential for health policy and programs. Such differences are not simply a matter of education or ability to pay for health care. For instance, Asian Americans and Hispanics appear to be in better health, on a number of indicators, than White Americans, despite, on average, lower socioeconomic status. The reasons are complex, including possible roles for such factors as selective migration, risk behaviors, exposure to various stressors, patient attitudes, and geographic variation in health care. This volume, produced by a multidisciplinary panel, considers such possible explanations for racial and ethnic health differentials within an integrated framework. It provides a concise summary of available research and lays out a research agenda to address the many uncertainties in current knowledge. It recommends, for instance, looking at health differentials across the life course and deciphering the links between factors presumably producing differentials and biopsychosocial mechanisms that lead to impaired health.

When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. This report focuses on what communities can do to promote health equity, what actions are needed by the many and varied stakeholders that are part of communities or support them, as well as the root causes and structural barriers that need to be overcome.

Book excerpt: Research demonstrated that women of minority groups are among the ones hardest hit when it comes to healthcare access due to issues such as gender, cultural, and linguistic discrimination. This study's purpose was to identify such disadvantages and their underlying discriminating structures and processes for Latina women in a local context. In order to identify barriers and their underlying structures, social indicators were formulated that included: infrastructure, profession and education, immigration and health policies, health insurance, and culture and language. The theoretical perspectives included Critical Theory and Feminist Theory, which provided a framework to identify oppressive structures and processes. Three methodologies were utilized to collect data: community mapping, focus groups and interviews, and a survey. The maps provided an overview on spatial issues for Latinas, such as their location in the county and the distance to service providers. The survey captured a small sample of Latinas and mainly represented patterns of barriers that were experienced most often due to low socioeconomic status. The focus groups and interviews represented the voices of service providers in the community about the barriers for Latina women. The results from the qualitative data, combined with the results from the other two methods presented the discussed indicators as barriers for Latinas in Louisville. The analyses also showed that these barriers were infiltrated by systems of domination (ethnicity and language, gender, citizenship/nationality, and socioeconomic status) and types of oppression (powerlessness, marginalization, exploitation, and cultural imperialism). The policy level was also discussed by utilizing concepts of Critical Theory. It was argued that modern law, including healthcare and immigration policies, lack the moral foundations that would justify the right to healthcare. The human rights were utilized as moral foundation to identify access barriers. The integration of the theoretical perspectives and the human rights made way for reflecting on the identified barriers from various viewpoints and to emphasize that the impact of barriers varies between different social groups. In the end, recommendations, gathered throughout the qualitative data collection, were discussed to demonstrate how social change could be implemented locally.

Care Without Coverage examines the real consequences for adults who lack health insurance. The study presents findings in the areas of prevention and screening, cancer, chronic illness, hospital-based care, and general health status. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.

As the Latino population is soon becoming the majority in California, it is important to understand what factors may create barriers and begin addressing them. The Introduction section presented three research questions to determine what factors are related to access to care. The Methodology section further discusses the overall survey details and the components extracted in order to perform the analysis. The study was limited to Latino adults, ages 18-85 years, in California. A multitude of variables will be tested against access to care, such as age, gender, citizenship status, income, language and transportation. To test each variable, lists of questions from the surveys were extracted to test in the results. The Results section indicated that all six variables were statistically significant when compared to access to care. The ranges of significance levels varied with each condition, however it was found that each were related and had some level of influence to access to care. The Discussion section provides recommendations for future directions in research. To determine whether these factors were ethnicity specific or were barriers in general, it was suggested to test each variable and compare the results with one or more ethnicities.

Book excerpt: Research has shown that although Latinos are disproportionately affected by many negative health outcomes, they are less likely to have access to healthcare when compared to other racial and ethnic groups. Literature has shown that barriers to healthcare tend to decrease the longer Latino immigrants reside in the United States. However, little is known if all immigrants experience decreased barriers consistently over time or if certain people are more at risk to experiencing more barriers than others. In addition, few available data focuses on how these factors affect Latino immigrants from diverse countries of origin. In an attempt to address the dearth of literature, the purpose of this study was to examine how demographic and healthcare utilization factors related to barriers to care among Mexican and Guatemalan immigrants in Cincinnati, a non-traditional destination for Latino immigrants. The current study uses a Community-Based Participatory Research (CBPR) co-researcher model to collect survey data from 401 immigrants from Mexico (n = 258) and Guatemala (n = 143). Results from our study indicated that contrary to previous research, longer length of residence was not associated with fewer barriers to healthcare for either group. Among Mexicans, women and those who have greater confidence of ones' own skills to manage their health outcomes (i.e., patient activation) reported fewer barriers to care. Among Guatemalans, having children and a clinic visit in the past year moderated the relation between length of residence and barriers to care. Our findings suggest that intervention efforts should focus on improving healthcare access in general, but emphasis should be placed on Mexican men and those with low patient activation while focusing on childless Guatemalans and those not interacting with the healthcare system. Our results highlight the growing need to consider the diverse experiences of Latino immigrants from varying countries in order to create more appropriately targeted interventions to eliminate health disparities.

This was accomplished by performing a quantitative study analyzing secondary data obtained from the California Health Interview Survey (CHIS). The results revealed that Latino immigrants with lower level of English proficiency had fewer doctor's visits, reported lower levels of acculturation, and the majority were born in Mexico whom reported having no health insurance in the entire year. The variables of gender, citizenship status, general health status, and insurance coverage within the past 12 months had a strong correlation with the utilization of the emergency room. The findings may be beneficial in providing awareness to the barriers Latino immigrants face while accessing health care services in the United States.
