Book excerpt: Background: There has been an increasing shift in patient care from the acute hospital setting to the community. Nurses play an essential role as part of the community health care workforce; however, only a limited number of baccalaureate nursing students tend to choose a career in community health nursing after graduation. There is currently a gap in knowledge surrounding nursing students' perception of a career in Community Health Nursing and the issues influencing their career choice upon graduation. Purpose: To explore issues that influence career choice in community health nursing from the perspective of baccalaureate nursing students. Research Methodology: The study was guided by a descriptive qualitative research approach. Individual semi-guided interviews and focus groups were conducted with 11 nursing students and a group of key stakeholders to share their thoughts on pursuing a career in community health nursing and the factors that enabled or hindered their decision making. Thematic analysis of the interview and focus group data generated relevant themes. Findings: Five major themes were revealed from study. These are 1) defining community health nursing, 2) the clinical practicum experience, 3) stereotypes of community health nursing, 4) societal trends and expectations, and 5) issues influencing career choice in community health nursing. Discussion and Implications: The personal and contextual factors influencing the perceptions and attitudes of students towards pursuing community health nursing were discussed. Existing literature was integrated into the discussion of the many factors that both motivated and hindered baccalaureate nursing students from pursuing community health nursing. The underrepresentation of new graduates in community health nursing calls for directed efforts by community health nursing organizations and the university to improve the situation. Conscientious efforts need to be made to provide students with knowledge and information surrounding the roles of community health nurses and the opportunities for nursing students and nurses in community health nursing settings. Conclusion: There is a need to increase awareness about community health nursing in order for nursing students to understand the importance and impact it has on the health status of communities and healthcare delivery infrastructure. Nursing education programs would be an ideal platform for this awareness-raising and facilitate student nurses decision to pursue community health nursing as a career.

Book excerpt: A critical goal of nursing education is to prepare a future generation of practitioners who can influence outcomes in a complex and changing health care environment. The empowerment of nurses has been associated with positive organizational and health care outcomes. Educational culture, as a major socializing control, has the potential to affect the empowerment of nursing students and their future nursing practice. The purpose of this study was to examine relationships between students' perceptions of organizational culture in their baccalaureate nursing program and the students' self-perceived empowerment. The sample consisted of 231 senior baccalaureate nursing students from 4 public institutions in Northeast Ohio. The Organization Culture Inventory (Cooke & Lafferty, 1983, 1986, 1987, 1989) and the Short Vincenz Empowerment Scale (Vincenz, 1990) were used to measure organizational culture and empowerment, respectively. Findings indicate that, overall, students perceived their baccalaureate nursing program as having a moderately high constructive culture and a moderately low defensive culture. Students also described themselves as having moderately high empowerment. A weak positive relationship was found between students' perception of a constructive organizational culture of their baccalaureate nursing program and their empowerment. A weak inverse relationship was found between students' perception of a defensive culture in their baccalaureate nursing program and their empowerment. Together, the two organizational culture variables explained 7.7% of the variance in student empowerment. An important measurement finding of this study was that two organizational culture concepts, passive-defensive and aggressive-defensive cultures, could not be differentiated in this sample. Although factors other than culture may account more for student empowerment in baccalaureate nursing programs, this study provides support for the existence of an emerging new generation of empowered nurses who have the potential to make a positive impact on the nursing profession and patient care.

Nurses should be fully engaged with other health professionals and assume leadership roles in redesigning care in the United States. To ensure its members are well-prepared, the profession should institute residency training for nurses, increase the percentage of nurses who attain a bachelor's degree to 80 percent by 2020, and double the number who pursue doctorates. Furthermore, regulatory and institutional obstacles-including limits on nurses' scope of practice-should be removed so that the health system can reap the full benefit of nurses' training, skills, and knowledge in patient care. In this book, the Institute of Medicine makes recommendations for an action-oriented blueprint for the future of nursing.

Book excerpt: "Today's nurses work and live in a multicultural society where they encounter patients whose backgrounds are different from theirs, and who need care from nurses who are both proficient in their work and knowledgeable about the role that culture plays in patient treatment. In this study, 45 student nurses enrolled in a baccalaureate program at a northeastern urban college completed a survey about their perceptions of care and culture including their relevance and application to the practice of nursing. Findings based on qualitative analyses indicated that parents and family were instrumental in students' learning about care and a combination of family and educators influenced their learning about culture. Responses revealed that while the importance of obtaining specific cultural information from patients through effective communication was noted, the use of a medical interpreter was considered important by only one student. This finding is problematic since students were unaware of the importance of utilizing interpreter services when caring for a patient who has limited knowledge of the English language. The findings of this survey suggest the need for further transcultural education on the baccalaureate nursing level specifically as it pertains to communication with patients of diverse cultures in the provision of care."--Abstract.

Accordingly, at the request of the Robert Wood Johnson Foundation, on behalf of the National Academy of Medicine, an ad hoc committee under the auspices of the National Academies of Sciences, Engineering, and Medicine conducted a study aimed at envisioning and charting a path forward for the nursing profession to help reduce inequities in people's ability to achieve their full health potential. The ultimate goal is the achievement of health equity in the United States built on strengthened nursing capacity and expertise. By leveraging these attributes, nursing will help to create and contribute comprehensively to equitable public health and health care systems that are designed to work for everyone.
