Book excerpt: At present lung cancer is the most common cancer in the world and responsible for over 1 million deaths worldwide. Advances in bronchoscopy have helped to increase detection rates of central type early lung cancer. These lesions are invisible on the basis of radiological methods. They usually show only subtle changes of the bronchial mucosa and are therefore sometimes difficult to identify by conventional bronchoscopy. The data showing the ability to increase the diagnostic rate for early lung cancer in the central airways by autofluorescence bronchoscopy are convincing. Video-chip-autofluorescence bronchoscopy seems to be one of the technologies with the largest impact on diagnostic bronchoscopy in the last decades.

Book excerpt: Doctoral Thesis / Dissertation from the year 2008 in the subject Medicine - Neoplasms, Oncology, grade: 1.00, , language: English, abstract: Centrally arising squamous cell carcinoma of the airway is thought to develop through multiple stages from squamous metaplasia to dysplasia, followed by carcinoma in situ (CIS), progressing to invasive cancer. It would be ideal to be able to detect and treat preinvasive bronchial lesions before progressing to invasive cancer. Advances in early diagnostic and treatment options have the potential to manage LC while still in an intraepithelial and/or microinvasive stage. WLB is one of the most commonly used diagnostic tools for LC. However, WLB is limited in its ability to detect small intraepithelial and microinvasive preinvasive lesions.AFB was developed to address this limitation by WLB. Indications for AFI examination; (1) Patients with known or suspected LC (2) Patients with abnormal sputum cytology (3) Patients after curative surgery for stage I LC(4) Current or former smokers with symptoms. A total of 74.7% patients had abnormal sputum cytology, 14.3% had known or suspected LC. All patients underwent conventional WLB followed by AFI examination; using the AFI device. Bronchoscopic-guided biopsy specimens were obtained. Sensitivity, specificity, and diagnostic accuracy of WLB versus that of AFI, were 58.3% Vs 88.9%; 46.7% Vs 71.9%; and 53.0% Vs 80.9%. with a very high significant difference (p value

Available in PDF, EPUB and Kindle. Book excerpt: This book covers all aspects of diagnostic and therapeutic bronchoscopy, which go beyond the techniques of inspection, simple lavage and biopsies of the tracheobronchial tree. In a first section, historical aspects as well as the modern use of both rigid and flexible instruments, the set-up of a bronchoscopy unit, anaesthetic techniques, and functional evaluation for patients undergoing interventional bronchoscopy are discussed. A diagnostic section on transbronchial needle aspiration and bronchoscopic ultrasound is followed by extensive coverage of all existing therapeutic techniques: foreign body removal, laser resection, electrocautery, argon plasma coagulation, cryotherapy, brachytherapy, photodynamic therapy, stenting, and a chapter on multi-modality treatment of advanced pulmonary malignancies. A fourth section deals with percutaneous image-guided procedures, percutaneous dilatational tracheostomy, and transtracheal oxygen therapy. The last section provides an in-depth look at emerging bronchoscopic technology, such as autofluorescence, virtual bronchoscopy, and gene therapy. Written by the world's leading authorities in the field and beautifully illustrated, this book presents the state of the art of interventional bronchoscopy. It is recommended reading for pulmonologists, thoracic or ENT surgeons, oncologists and physicians who take an interest in bronchoscopy.

To date, two volumes have been both published and well received by the scientific community. Several book reviews have also favorably described the series as an "excellent compilation of material which is well balanced from authors in both the US and Europe". Of particular mention we note the recent book review in JACS by Gary Baker, Los Alamos. In this 3rd volume we continue the tradition of publishing leading edge and timely articles from authors around the world. We hope you find this volume as useful as past volumes, which promises to be just as diverse with regard to content. Book excerpt: This book provides a detailed overview of the latest innovations in respiratory endoscopy, from both diagnostic and therapeutic perspectives; each chapter focuses on one disease and the techniques for early diagnosis as well as treatment. It comprehensively covers treatment and procedures, including simultaneous X-ray fluoroscopy and its use during bronchoscopic procedures. This fast-developing technology is essential for the medical management of non-malignant and malignant diseases of the chest, especially lung cancer. Respiratory Endoscopy describes the cooperation between all the members of the healthcare team, and as such is a valuable resource not only for medical staff, but also for radiological technicians and nursing staff who contribute significantly in the care of the patients undergoing these invasive procedures. By promoting teamwork and providing practical know-how, it will improve the success and safety of respiratory endoscopy procedures.
