Book excerpt: Contexte : Le diabète de type 1 de l'enfant est une pathologie dont l'incidence augmente en moyenne de 3.5% par an en France. Il se révèle dans plus de 40% des cas par une acidocétose qui peut, en l'absence de traitement, évoluer vers la mort. Il a été démontré que le retard au diagnostic est un des principaux facteurs favorisant sa survenue. L'objectif de l'étude est de définir les connaissances et habitudes des médecins généralistes du Nord face à une suspicion de diabète chez l'enfant. Méthode : Il s'agit d'une étude épidémiologique observationnelle descriptive où un questionnaire a été envoyé à un échantillon de médecins généralistes du Nord tiré au sort. Ce questionnaire comporte 22 questions avec des propositions de choix, regroupées en 13 questions principales, portant sur les connaissances des médecins vis-à-vis du diabète de l'enfant et sur leurs habitudes face à une suspicion de découverte. Résultats : Parmi les 300 questionnaires envoyés, 100 nous ont été retournés complétés, soit un taux de réponse de 33.33%. 54% des médecins adressent l'enfant aux urgences pédiatriques dès la suspicion clinique ou après réalisation d'une bandelette urinaire et/ou d'une glycémie capillaire au cabinet. Une nette majorité des médecins répondeurs a une bonne connaissance des signes classiques du diabète, même si seuls 40% des médecins citent l'énurésie secondaire, signe révélateur fréquent et typique chez l'enfant. 80% des médecins répondeurs réalisent une biologie pour poser le diagnostic de diabète et 40% n'utilisent pas le lecteur glycémique en leur possession comme test diagnostic. Plus d'un quart des médecins répondeurs (27%) adressent l'enfant dans un délai supérieur à 24h pour une prise en charge spécialisée. Conclusion : Il ressort de notre travail qu'une proportion insuffisante de médecins a une attitude adaptée face à une suspicion de diabète chez l'enfant, notamment à cause d'une mauvaise utilisation des examens paracliniques disponibles au cabinet. Une nouvelle campagne d'information serait utile afin de rappeler aux médecins généralistes du Nord la définition clinico-biologique du diabète de l'enfant, ainsi que l'absence de nécessité de réalisation d'une biologie avant d'adresser l'enfant, afin de ne pas retarder la prise en charge qui est une urgence thérapeutique.
