Book excerpt: La prise en charge de l'insuffisance rénale chronique terminale (IRCT) chez les patients en situation irrégulière n'a jamais fait l'objet d'études médico-économiques en France. Notre étude reprend tous les patients en situation irrégulière et qui présentaient une IRCT diagnostiquée dans les CHU de Rouen et d'Amiens et dans le CH de Creil entre août 2008 et décembre 2018. Ces patients ont été comparés avec des patients en situation régulière. Quarante-et-un patients en situation irrégulière et en insuffisants rénaux chronique terminaux ont été inclus. Il s'agissait principalement de femme, l'âge médian était de 37 ans et la néphropathie le plus souvent indéterminée. Il n'y avait pas de différence de mortalité entre les deux groupes et aucun décès n'était lié à une complication directe de l'IRCT. L'hémoglobine médiane à l'arrivée en France était à 9 g/dl [7,8-10,3] et l'hyperparathyroïdie secondaire était modérée avec une PTH à 311 ng/l [122-899]. On ne retrouvait pas plus d'hospitalisation parmi les patients en situation irrégulière. A la fin du suivi, 38% des patients en situation irrégulière vs 68% patients en situation régulière ont bénéficié d'une transplantation rénale (p=0,012). Notre étude montre une qualité de prise en charge satisfaisante de la maladie rénale chronique des patients en situation irrégulière. Des études prospectives réalisées dans des zones de plus fortes immigrations sont nécessaires.

Tous ces troubles vont être à corriger à l'aide de nombreux médicaments qui, associés, peuvent entraîner des interactions médicamenteuses. Une étude menée sur une population de patients hémodialysés montre que beaucoup de prescriptions médicamenteuses sont non-conformes.

Until the 1950s, chronic renal failure was considered to be an inexorably lethal condition. This is no longer the case. In addition, the disease, severe uremic syndrome, is now extremely rare, if existent at all, in industrialized countries. Physicians of my generation who saw patients hospitalized with hemor raghes, pericarditis, severe anemia, cardiac failure, "malignant hypertension," pruritus, vomiting, generalized edema, and convulsions are particularly grate ful for this progress. I well remember seeing such patients hospitalized in the last days or weeks of their lives and also remember the sense of impotence I suffered for the com plete lack of efficient measures I had at my disposal to manage their condition. Nowadays, hemodialysis, peritoneal dialysis, and kidney transplantation allow patients with chronic renal failure to survive for very long periods of time in a satisfactory condition. Why then is there still a sense of dissatisfaction and why should we study dietary management? The drawbacks of dialysis and transplantation are the main reasons, but the certainty that dietary therapy is complementary to dialysis and even better than dialysis in certain conditions, is also very important.

Of the four parasites (Plasmodium), only P. falciparum kilts although the other three cause debilitating disease with regular relapses and recrudescence. More than fifty species of Anopheles can fulfil the vector rote which is essential in transmission of the parasite between human beings. Climatic factors (temperature and rainfall), the environment and biogeographical particularities dictate the distribution of anopheline species and determine transmission rates. This is why it makes sense to tank about the biodiversity of malaria. Today, more than 90% of deaths from malaria occur in Tropical Africa which is home to only 10% of humanity. Every 30 seconds a child dies from malaria. This continent harbours the most effective vectors (An. gambiae and An. funestus, in particular) and the climate is highly conducive to transmission of the disease. Severe malaria is also seen in forest foci in Southeast Asia, Papua-New Guinea and the Amazon. In the rest of the tropical and subtropical world, P. vivax and/or P malariae cause less severe disease.
