Book excerpt: The impact of asthma self-management programs on medical resource utilization has been studied repeatedly. The impact of these programs on a child's perceived quality of life, however, has not been addressed. This descriptive, correlational study compared the perceived quality of life and number of hospitalizations, emergency room and clinic visits in asthmatic children ages 7 - 11 years who adhered to a self-management plan with those who did not adhere to a plan. Quality of life was measured using the Childhood Asthma Questionnaire, a valid and reliable quality of life measurement tool for children of different ages. There were no significant differences between the Adherence and Non-adherence groups relative to quality of life or medical resource utilization. Findings did support a significant decrease in medical resource utilization for both groups since the establishment of their self- management plan. These findings confirm the role of self-management education as a worthwhile and cost effective investment for children.

Book excerpt: According to the National Center for Health Statistics (2001) in 1998, 3.8 million children aged 0-17 years had an asthmatic episode. Nationally the asthma population is 6.3%. Arizona has exceeded the national asthma rates for 1991-1998 with an incidence of approximately 7% of the population. Asthmatic children require self-management instruction about asthma, medications, symptoms, and proper self-management. Poor self-management results from a lack of competence over asthma related self-management skills. This study defined self-management skill as those skills an asthmatic must have for competence in management. The purpose of this study was to test a self- management education program designed to enhance the self-management skill of inner city children with asthma in order to improve their health outcomes and quality of life. Measurement of effect in health outcomes and quality of life (QOL) were the indicators of competency in self-management skill. The pediatric asthma-specific questionnaire by Juniper and Guyatt (2001) examines QOL. Thurber and Blue (1994) developed the Asthma Self-care questionnaire to measure health outcomes. The pilot study design utilized a quasi-experimental design with pretest/posttest and non random assignment of subjects measured the differences in health outcomes and QOL of asthmatic children. Data collection was at three inner city Phoenix public schools. The analysis examined the * independent variable influence of self-management education on the dependent variable impact on quality of life and health outcomes.

The purpose of this pilot study was to evaluate the effects of an organized asthma self-management education (OSMAE) intervention in children 7 to 12 years hospitalized with mild to severe asthma. A one-group quasi-experimental design study was conducted pre and one-week post intervention to evaluate outcomes variables. Besides collecting baseline demographic data, to evaluate the efficacy of the OSMAE, data were collected on asthma symptoms, activity limitations, emotional function, and peak flow readings and ED visits. Six consenting English-speaking 7 to 12-year-old children diagnosed with asthma and hospitalized at least once were recruited.

The impetus for this book came from a recognition of the increasing importance of HRQOL assessments in the evaluation of treatment outcomes and the need to increase the utilization of HRQOL assessments in research and clinical applications with a range of pediatric populations. The need to develop a volume that describes new research and clinical applications concerning this topic stemmed from several recent developments. There is a continuing need for evaluations of the efficacy of medical treatments for children and adolescents, including those with chronic health conditions. To address these critical unmet needs in the field of HRQOL assessment, and to advance scientific methods and clinical applications in this field, a conference was held at Case Western Reserve University. The conference set out to summarize current information concerning the development and implementation of measures of HRQOL assessment, to identify and consider key conceptual and methodological issues in research concerning the measurement of HRQOL, and to recommend priorities to advance the state-of-the-art in research and clinical applications of QOL assessment in children and adolescents with chronic health conditions. This volume summarizes and synthesizes the information that was presented by the conference participants in a series of lively discussions and chapters that were based on the presentations.

Despite the remarkable morbidity of this condition, childhood asthmatics and their parents frequently know little about this condition, thus it is often under-treated. Under-treatment of asthma can lead to life long reduction in health and quality of life. With appropriate educational intervention, childhood asthma can be better managed. It is well known that children are motivated to play video games. With Kanfer's Self-Regulation theory as a conceptual basis, we can take advantage of this powerful childhood interest to improve their asthma control. This study analyzed the effectiveness of using a computer-based asthma training video game, Quest For The Code, as an educational tool in improving specific measures of asthma severity in 36 chronic asthmatic children living in a rural area of Northwest Florida. This study demonstrated significant improvements in peak flow and scores on the Asthma Control Test (A.C.T.) after the educational intervention. None of the selected demographic variables, marital status of parents, number of siblings in the family, age of subject, school grade, and number of hours Quest For The Code was played were associated with the outcome measures.

To the best of our knowledge, no study reviewed the literature on the effect of family asthma education on major asthma outcomes. This study aimed to explore the effect of family education programs on major asthma outcomes in children. Quasi-experimental studies and randomized controlled trials were conducted among children with asthma aged 6,Äì18 years and their parents were included. Pub Med, Science Direct, and Trip databases were used to extract data published in English from 2010 to 2021. Twenty-two studies were reported in this review. It was demonstrated that family empowerment interventions were effective in improving the quality of life of children and their parents, asthma symptom control, and pulmonary function. Family education that was specific to medication improved medication adherence, inhalation technique, and asthma control. Family asthma education enhanced asthma management and family functioning. This approach should be a cornerstone of pediatric asthma therapy. It helps health care professionals to build a strong connection and trustful relationship with children with asthma and their families.

Book excerpt: Asthma is the most commonly diagnosed chronic disorder in childhood and is linked with several problematic outcomes including frequent school absences, increased hospitalizations and decreased quality of life. Further, urban populations struggling with low socioeconomic status are disproportionately represented in prevalence statistics and suffer from increased functional morbidity relative to other children with asthma. These findings exist in the midst of largely effective pharmacological interventions. Asthma self-management programs (SMPs) target several behaviors linked to improved outcomes and are often used as an adjunct to medication management. SMPs have been employed using a variety of techniques and treatment targets in a wide range of settings. Data suggests that SMPs have a mild to moderate impact on functional morbidity outcomes and merit further research. In order to facilitate efficacious interventions to those at highest risk of problematic outcomes, common barriers impinging on program attendance must be alleviated. The school setting proves to be an ideal location to deliver SMPs due to their accessibility and available resources. The current project evaluates a school-based asthma SMP delivered in an urban setting. Results reveal statistically significant increases in quality of life and child reported knowledge as well as decreased utilization of urgent care outpatient treatment. Trends towards decreased agreement between children and caregivers on allocation of asthma management tasks reflect challenges with implementation of programs in the absence of frequent parental contact. School-based asthma SMPs afford several benefits to high-risk populations but also must reflect the need for family involvement in successful asthma management.
