It was initially used to protect people who might be exposed to anthrax where they worked, such as veterinarians and textile plant workers who process animal hair. When the U. S. military began to administer the vaccine, then extended a plan for the mandatory vaccination of all U. S. service members, some raised concerns about the safety and efficacy of AVA and the manufacture of the vaccine. In response to these and other concerns, Congress directed the Department of Defense to support an independent examination of AVA. The Anthrax Vaccine: Is It Safe? Does It Work? reports the study's conclusion that the vaccine is acceptably safe and effective in protecting humans against anthrax. The book also includes a description of advances needed in main areas: improving the way the vaccine is now used, expanding surveillance efforts to detect side effects from its use, and developing a better vaccine.

Book excerpt: Early detection is essential to the control of emerging, reemerging, and novel infectious diseases, whether naturally occurring or intentionally introduced. Containing the spread of such diseases in a profoundly interconnected world requires active vigilance for signs of an outbreak, rapid recognition of its presence, and diagnosis of its microbial cause, in addition to strategies and resources for an appropriate and efficient response. Although these actions are often viewed in terms of human public health, they also challenge the plant and animal health communities. Surveillance, defined as "the continual scrutiny of all aspects of occurrence and spread of a disease that are pertinent to effective control", involves the "systematic collection, analysis, interpretation, and dissemination of health data." Disease detection and diagnosis is the act of discovering a novel, emerging, or reemerging disease or disease event and identifying its cause. Diagnosis is "the cornerstone of effective disease control and prevention efforts, including surveillance." Disease surveillance and detection relies heavily on the astute individual: the clinician, veterinarian, plant pathologist, farmer, livestock manager, or agricultural extension agent who notices something unusual, atypical, or suspicious and brings this discovery in a timely way to the attention of an appropriate representative of human public health, veterinary medicine, or agriculture. Most developed countries have the ability to detect and diagnose human, animal, and plant diseases. Book excerpt: Federal agencies responsible for responding to the 2001 anthrax incidents adopted a targeted sampling strategy that they based on their best judgment at the time. They primarily collected samples from specific areas, such as mail-processing areas, using their judgment about where anthrax would most likely be found. Such judgments can be effective in some situations for example, in determining whether a facility is contaminated when information on the source of potential contamination is definitive. However, in the case of a negative finding, when the source of potential contamination is not definitive, the basic question Is this building contaminated? will remain unanswered. Therefore, in the case of a negative result, a different strategy, probability sampling, is needed. Probability sampling would have allowed agencies to determine whether the building was contaminated with some defined level of confidence. The federal agencies CDC, EPA, and USPS involved in sampling the postal facilities in 2001 to detect anthrax undertook several activities. These included development of a sampling strategy followed by collection of samples using a variety of methods, transporting and extracting, and analysis of the samples. Neither these activities nor the overall process was validated for anthrax testing. Consequently, the agencies were challenged by the limited information available for reliably choosing one method over another and the lack of information on the detection limit to use when evaluating negative results.

In 2005, assessing federal agencies' activities for detecting Bacillus anthracis in postal facilities, GAO reported that the test results of their sampling were largely negative. GAO found that the agencies had not used validated sampling methods and approaches that would have given a defined level of confidence for negative results. Consequently, GAO recommended several actions. This study, identifies the extent to which (1) DHS's actions have addressed GAO's recommendations regarding sampling; (2) the environmental sampling methods for B. anthracis spore detection in initial public health sampling and microbial forensic investigations have been validated; and (3) any challenges remain to completing validation.
