The workers who have participated in this project are indebted to the Academy for financing extensive investigations and visits to institutes abroad, the aims of which were to solve certain problems by international cooperation. The results presented by the collective effort of research teams, each studying here have been achieved a particular problem, but working together to the same end. Mutual discussion of findings and a regular exchange of information between the research groups have contributed to the progress of the investigations. Regular pUblication of details and mutual criticism ensured that the findings were uniformly interpreted. It is hoped that the fundamental aim of our long-term project, i. e. the selection of the right approach to the complex problem of vascular pathology, has been achieved. The new approach results from the recognition that since arteriosclerosis finally leads to various kinds of vascular injury, attention should be focussed on the events preceding the development of the lesions rather than on the lesions themselves. This claim has been confirmed by various model experiments in which vascular injury inflicted under different conditions uniformly led to morphologi cally verified arteriosclerotic change. The recent increase in the incidence of civi lization diseases of the vascular system has resulted in a world-wide expansion of arteriosclerosis research.

Book excerpt: A vast literature has been concerned with arteriosclerosis and yet, many aspects of pathogenesis and of the mechanism of development of the arteriosclerotic vascular lesion remain only poorly understood. In recent years, our knowledge of the earliest stages of arteriosclerosis have greatly improved. By now, we have learned to relate morphologic changes to disturbances in function. It has been of particular impor tance that components of the arterial wall could be analyzed in regard to dysfunction, for example, in the endothelium or the vascular smooth muscle. The interaction of the different morphological components of the vascular wall could thus be much bet ter understood. Likewise, the interaction between the arterial wall and the flowing blood could be much better described, including the intimate relationship between platelets and the endothelium, the coagulation system and the endothelium, the granulocytes and the endothelial cell layer, as well as processes of migration of blood cells into the subendothelial space. The recognition of functional and morphological disturbance has attained clinical significance not only because the arteriosclerotic diseases have quantitatively reached the dimensions of an epidemic, that is, of a magnitude never been witnessed. It is also because of the development of new drugs that interfere with the atherogenic process and thereby prevent the development of the disease or halt its progression. It is also becoming increasingly possible to inhibit the occurrence of complications in existing arteriosclerotic lesions in manifest disease, i. e.

The significance of some of these observations made it clear that there was need for intensified research on the connective tissue components of the arteriosclerotic lesion and that arteriosclerosis research workers could benefit from a more comprehensive view of the subject. Because of their experience in the field of arteriosclerosis and their interest in stimulating new directions for research on the lesion, the Committee on Coronary Artery Lesions and Myocardial Infarctions of the Council on Arteriosclerosis, American Heart Association, planned an International Workshop on Arterial Mesenchyme and Arteriosclerosis. The Workshop brought together scientists expert in connective tissue research and research on arteriosclerosis who presented the current status of knowledge in their areas of expertise. The Workshop was held April 2-3, 1973 at the Royal Orleans Hotel, New Orleans, Louisiana and was attended by more than 170 people. The twenty papers and discussions presented in this volume summarize the proceedings of the Workshop and represent a comprehensive review of the role of arterial mesenchyme in arteriosclerosis.
