Book excerpt: Introduction : en dépit d’un cadre réglementaire strict incluant une visite médicale annuelle et la signature d’un certificat médical d’absence de contre-indication à la pratique (CACI), les accidents et la mortalité en plongée-loisir augmentent, notamment chez les « séniors » (> 50 ans). Matériel et méthode : une revue systématique de la littérature, analysant 31 articles publiés en français ou anglais après 2004, a permis de préciser les éventuels facteurs de risque intrinsèques ou environnementaux favorisant les accidents de décompression et les œdèmes pulmonaires d’immersion, exclusivement en plongée-loisir à l’air, et de s’interroger sur l’habilitation depuis 2014 de tout médecin à signer ce CACI. Résultats : le risque d’accident lors d’une plongée-loisir augmente avec l’âge ; on recense plus d’accidents graves et plus de séquelles après 50 ans. Trois facteurs de risque intrinsèques sont corrélés à l’âge : une prévalence accrue des maladies cardiovasculaires, une masse grasse plus élevée et une sénescence physiologique du système cardiovasculaire. Tout âge confondu, l’exposition au froid et la profondeur de la plongée sont des facteurs de risque d’accidents. Conclusion : les agences de plongée américaines imposent, indépendamment de l’âge, une limitation de la plongée à l’air à 40 mètres de profondeur, avec exclusion des profils de plongée exigeant des paliers de décompression. La Fédération française d'études et de sports sous-marins (FFESSM), moins sécuritaire, autorise ces profils, avec une profondeur maximale autorisée de 60 mètres. Ces deux approches de la plongée-loisir n’ont pas les mêmes exigences d’aptitude. Nous suggérons que le CACI rédigé par un médecin généraliste non spécialisé en médecine de plongée ne concerne que la plongée « à l’américaine » et que le CACI dédié à la plongée « à la française » nécessite l’avis d’un médecin spécialisé, surtout après 50 ans.
