Dans cette thèse, nous montrons la pertinence des analyses linguistiques pour une meilleure compréhension des troubles aphasiques, et plus particulièrement de l'agrammatisme. Les analyses linguistiques repèrent des dysfonctionnements en langue (liés à la structure de la langue), afin de connaître l'état de compétence en langue des patients. Notre réflexion s'articule autour de deux axes principaux : un axe théorique en linguistique fondamentale et un axe pratique en linguistique appliquée. L'objectif étant de conserver un lien étroit entre la recherche fondamentale et ses applications cliniques, à savoir le diagnostic et la rééducation. Le premier axe s'intéresse aux dysfonctionnements langagiers de neuf aphasiques de Broca agrammatiques à travers les niveaux phonologique (notions de système et de traits phonologiques) et morphologique (morphologie flexionnelle). Ici notre travail de linguiste réside essentiellement dans la construction et la validation de grilles d'analyse permettant d'appréhender les erreurs phonologiques (G.A.P.) et morphologiques (G.A.M) produites à l'oral par des patients agrammatiques. Ce type d'analyses ouvre ainsi des pistes de travail pour le clinicien, en vue d'une rééducation orthophonique spécifique. Dans le deuxième axe, nous proposons des perspectives de remédiation à partir des principaux résultats exposés dans cette thèse. D'une part, nous évaluons un outil linguistique de remédiation (C.O.L.O.É.) auprès de quatre patients agrammatiques. D'autre part, nous nous interrogeons sur la pertinence d'une comparaison entre rééducation clinique en langue et apprentissage d'une langue seconde, à l'aide du concept d'interlangue emprunté au domaine de l'acquisition des langues secondes. L'ensemble des travaux présentés ici s'intéresse de manière exclusive à la production du langage : nous analysons les formes produites à l'oral par des locuteurs aphasiques. Une telle approche ne peut se réaliser sans des épreuves de langues explicites (situations contraignantes), permettant de comparer la production du patient avec une cible. Les exercices contrôlés présentent trois avantages majeurs : ils constituent des outils d'évaluation rapides, ils permettent le repérage systématique des unités linguistiques, ils sont tout à fait adaptables à la remédiation orthophonique. Un des objectifs de cette recherche participe en effet à l'amélioration des outils d'évaluation (bilan linguistique) et à la constitution d'outils de remédiation prenant en compte les résultats scientifiques et la pratique orthophonique.

Nous vérifions la validité des principales hypothèses émises dans la littérature aphasiologique sur la nature de l’agrammatisme à partir d’un protocole élaboré sur la base des prédictions qui en émanent tout en tirant avantage des propriétés grammaticales du basque qui permettent d’approfondir l’examen du déficit verbal agrammatique. Nous décrivons sous forme quantitative et qualitative les résultats recueillis auprès des deux sujets agrammatiques et sujets contrôles correspondants, en soulignant les aspects linguistiques qui caractérisent le comportement agrammatique et le distinguent du processus linguistique normal. Enfin nous tentons de proposer une analyse « cohésive » de la nature procédurale postulée du déficit linguistique sous-jacent, dans un cadre neuro-psycho-linguistique interdisciplinaire, en rapprochant des travaux issus de la linguistique, de la psycholinguistique et de la neuroimagerie dans le but de fournir un cadre interprétatif adéquat à l’identification d’un déterminisme commun.

Les récentes avancées en neuropsychologie cognitive et psycholinguistique nous ouvrent la possibilité d’interpréter cognitivement les troubles aphasiologiques. Le modèle de traitement lexical de Hillis et Caramazza représente les modules activés lors d’une tâche langagière. Chaque module est sensible à des variables psycholinguistiques précises (longueur syllabique, fréquence lexicale...). Une répartition équitable de ces variables au sein des épreuves évaluant le langage permet de mettre en évidence leurs effets, d’en inférer les modules perturbés et ainsi de mettre en œuvre un programme de rééducation pertinent. Dans ce but, les épreuves de dénomination orale d’images 36 et de répétition de mots 32 ont été mises au point. Elles sont composées d’items répartis équitablement selon plusieurs variables psycholinguistiques favorisant une interprétation cognitive des résultats. Nous avons terminé la normalisation de ces épreuves ainsi que celle d’une épreuve de compréhension orale de phrases issues de l’Examen Long du Langage (UCL-ULG, non publié) qui permet d’analyser le traitement de différents types de phrases. Ce travail a permis d’établir des scores et des temps seuils grâce aux performances de 145 sujets contrôles, répartis en trois classes d’âge et en trois niveaux socio-culturels. Ceux-ci sont nécessaires pour détecter la présence d’un déficit significatif chez les sujets aphasiques. Les études de cas confirment l’utilité de ces épreuves pour déterminer le(s) module(s) langagier(s) déficitaire(s) en référence au modèle de Hillis et Caramazza. Ainsi, nous nous rendons compte que les atteintes cognitives peuvent être différentes alors que le diagnostic sémiologique est le même ; à l’inverse, elles peuvent être semblables alors que le diagnostic sémiologique est différent.
