Devant l’expansion de cette maladie chronique, des guides de recommandations ont été élaborés. Une récente étude menée en 2007, appelée Etude Nationale Témoin REprésentative des personnes Diabétiques, menée en partenariat entre la Caisse Nationale d’Assurance Maladie, l’Institut Nationale de Veille Sanitaire et l’Association Nationale de Coordination des REseaux de Diabétiques, soulève le problème de l’insuffisance du suivi des patients diabétiques de type 2 en France. Une étude bas-normande en 1999 avait permis d’évaluer les freins au suivi des patients diabétiques de type 2. Une enquête prospective a été réalisée fin 2009 et proposée à tous les médecins généralistes de l’agglomération de Caen (soit 187 médecins) afin d’évaluer leur point de vue sur le suivi des diabétiques de type 2. Les médecins généralistes connaissent les recommandations, estiment être bien informés mais ne les jugent pas toujours applicables en pratique. Malgré un nombre d’adhésion au réseau local élevé, il ne semble pas une aide. Peu de médecins paraissent convaincus par le contrat (CAPI) récemment proposé par la CNAM et une contrepartie financière ne semble donc pas une solution pour l’avenir. Si la formation du médecin traitant reste d’actualité, l’éducation du patient envers sa maladie doit constituer un objectif prioritaire afin d’améliorer son autonomie. «Les recommandations ne sont que des recommandations ; elles sont utiles pour un objectif de santé publique mais c’est au praticien de les adapter à son patient selon les critères multiples qui lui sont imposés».

La méthode a consisté à la diffusion d'indicateurs de suivi aux médecins généralistes de l'Isère et à l'évaluation de leur pratique de 2000 à 2003 par la CPAM. Les actes remboursés ont été sélectionnés par le système informatique MIAM. 960 généralistes ont inclus 5049 patients diabétiques. Les résultats ont été statistiquement significatifs (évolution 2000 versus 2003) : HbA1c quadrimestrielle : 63% à 71%, créatinémie annuelle : 77% à 85%, microalbuminurie annuelle : 23% à 34%, bilan lipidique annuel : 40% à 57%, suivi ophtalmologique : 47% à 50%. En conclusion, les résultats ont été encourageants mais insuffisants. Le système de santé doit faire face à cette situation d'où l'intérêt des réseaux de soins.

Le but de cette étude était d'identifier les difficultés d'applications pour le suivi du diabète de type 2 chez le sujet âgé en médecine générale. Méthode : une enquête qualitative par entretiens individuels auprès de douze médecins généralistes, a permis, après analyse d'inspiration phénologique de décrire les principales difficultés rencontrées en médecine générale dans l'application de ces recommandations. Résultats : les médecins connaissent les recommandations et ne remettent pas en cause leur validité. Ils souhaitent les appliquer mais se heurtent au manque de temps, aux problèmes de la compliance et d'autonomie du patient. Ils conçoivent la prise en charge des sujets âgés comme moins rigoureuse et plus centrée sur la relation médecin-malade. Ils envisagent d'amélioret leur prise en charge par l'éducation, la collaboration entre soignants, l'amélioration de la formation médicale et de l'outil informatique, voire la création de postes d'assistants médicaux. Conclusion : la sous application des recommandations pour le suivi du diabète de type 2 chez le patient âgé s'explique par des contraintes relationnelles, sociales et situationnelles. La prise de décisionen médecine générale peut parfois être davantage déterminée par ces contraintes que par l'intérêt scientifique des recommandations, dont les médecins restent néamoins convaincus

Méthode : Deux groupes de patients ont été comparés au cours de l'année 2008. Le groupe expérimental était constitué de 237 patients diabétiques de type 2 suivis par 41 médecins adhérents au réseau et le groupe témoin de 73 patients diabétiques de type 2 suivis par 15 médecins du Val d'Oise non adhérents au réseau. Résultats : L'adhésion au réseau permet une meilleure application des recommandations émises par la Haute Autorité de santé. Les améliorations observées portent surtout sur le suivi des patients avec la meilleure fréquence de réalisation des examens de dépistage : dosage de l'hémoglobine glyquée (81% vs 34%), bilan lipidique (92% vs 73%), créatininémie (93% vs 73%), recherche de protéinurie (52% vs 23%), réalisation du fond d' œil (71% vs 38%) et de l'électrocardiogramme (61% vs 25%). Conclusion : Le RDVO constitue une structure intéressante pour les patients diabétiques de type 2 puisqu'elle améliore leur prise en charge.

Book excerpt: "Hypoxaemia is a major contributor to child deaths that occur worldwide each year; for a child with pneumonia hypoxaemia increases the risk of death by up to 5 times. Despite its importance in virtually all types of acute severe illness, hypoxaemia is often not well recognized or well managed, more so in settings where resources are limited. Oxygen therapy remains an inaccessible luxury for a large proportion of severely ill children admitted to hospitals in developing countries. This is particularly true for patients in small district hospitals, where, even if some facility for delivering oxygen is available, supplies are often unreliable and the benefits of treatment may be diminished by poorly maintained, inappropriate equipment or poorly trained staff with inadequate guidelines. Increasing awareness of these problems is likely to have considerable clinical and public health benefits in the care of severely ill children. Health workers should be able to know the clinical signs that suggest the presence of hypoxaemia and have more reliable means of detection of hypoxeamia. This can be achieved through more widespread use of pulse oximetry, which is a non-invasive measure of arterial oxygen saturation. At the same time oxygen therapy must be more widely available; in many remote settings, this can be achieved by use of oxygen concentrators, which can run on regular or alternative sources of power. Having effective systems for the detection and management of hypoxaemia are vital in reducing mortality from pneumonia and other severe acute illnesses. Oxygen therapy is essential to counter hypoxaemia and many times is the difference between life and death. This manual focuses on the availability and clinical use of oxygen therapy in children in health facilities by providing the practical aspects for health workers, biomedical engineers, and administrators. It addresses the need for appropriate detection of hypoxaemia, use of pulse oximetry, clinical use of oxygen and delivery systems and monitoring of patients on oxygen therapy.
