Book excerpt: This book presents an Asian perspective on how the treatment of colorectal cancer can be optimized and standardized in ways that take into account technological advances and the trend towards individually tailored therapy. Readers will find careful, well-illustrated descriptions of the standard surgical techniques for rectal cancer and colon cancer that have contributed to recent improvements in 5-year survival rates in the Asia-Pacific region, where the incidence of colorectal cancer has been rising alarmingly due to lifestyle changes. The vital role now being played by minimally invasive laparoscopic and robotic options receives detailed scrutiny. Extent and timing of surgery, patient safety, risk of complications, and unresolved issues are all discussed. Furthermore, the use of surgery within the context of multimodal management including chemotherapy and radiotherapy is explained and an integrated approach for stage IV and recurrent disease is described. The book will serve as a valuable reference for young surgeons who are in training, experienced practitioners who want to enhance their knowledge and skills, and all others who wish to learn about this field.

Available in PDF, EPUB and Kindle. Book excerpt: In recent years, interest in the management of anterior skull base tumors has been fostered by the introduction and subsequent rapid expansion of indications of transnasal endoscopic techniques. In parallel, extraordinary progress has been made in all the other disciplines which are involved in the complex process of managing anterior skull base tumors, leading to substantial improvements in diagnosis and treatment. The anterior skull base can be involved in a large variety of tumors of a varying nature and histology, which in the majority of cases originate from adjacent anatomic sites. In fact, primary lesions of the anterior skull base mainly include osteoma and other tumors originating from bone and cartilage. Tumors involving the anterior skull base have their origin prevalently in the sinonasal tract, but lesions developing on the intracranial site, such as meningiomas, may also extend caudally to encroach upon this area. All these lesions are rare, which means that the pertinent literature does not include prospective studies or treatment guidelines based on a high level of evidence. In view of the extreme histological variability of lesions involving the anterior skull base, much emphasis has been placed on addressing the different nuances of treatment in relation to histology, especially for malignant tumors. The chapters focusing on surgery provide divergent views on selection criteria for a specific surgical technique, which is the aim of this publication.

Available in PDF, EPUB and Kindle. Book excerpt: The synthesis during the present decade of different arts and sciences to form oncology as a mUltidisciplinary subject is of profound importance for coordinating the clinical and research efforts to control a number of diseases that cause high mortality and morbidity in the human race and to elucidate their causation. These diseases traditionally have been grouped together under the general term of cancer, without any scientific reason and irrespective of many differences existing between them. The word cancer, because it is synonymous with diseases that cause suffering and death, naturally generates fear in people throughout the world. Cancer fortunately has a changing face caused by the realization that these diseases are different diseases with variable etiology and prognosis; they need different kinds of treatment, and even prevention is a practical proposition. The time has therefore come to delete the term cancer from our terminol ogy as unscientific and unhelpful and to substitute oncological diseases governed by the system of knowledge designated oncology. This conforms also with the designations used for other groups of diseases. In the divisions of oncology an important place is held by surgical oncology because many oncological diseases require surgical treatment alone, or in combination with radiotherapy and chemotherapy. Combina tion therapy is being used effectively for an increasing number of these diseases, and this trend will become more pronounced as chemotherapy develops. The results achieved with available chemicals and present dosage schedules are impressive.

Book excerpt: Although there has been a slow but steady decrease in incidence, gastric cancer remains the second leading cause of cancer death worldwide. Several aspects of the oncological and surgical management are still controversial and so gastric cancer represents a challenge for the surgeon. This book aims to delineate the state of the art in the surgical and oncological treatment of gastric cancer, describing the new TNM staging system, the extent of visceral resection and lymphadenectomy focusing on the different open and minimally invasive surgical techniques and discussing intraoperative chemohyperthermia and neoadjuvant and adjuvant treatment. Operative endoscopy and endoscopic ultrasonography are also discussed, as these now have an important role in both diagnostic work-up and palliative care of gastric cancer patients. Only a multidisciplinary approach involving the surgeon, gastroenterologist, and oncologist can produce the comprehensive and integrated overview that today constitutes a winning strategy for the optimization of results. What we hope we have achieved is a flexible, up-to-date, exhaustive publication, rich in illustrations and consistent with evidence-based medicine.
