According to some estimates, as many as 20% of adult clients terminate psychotherapy prematurely. Even experienced practitioners using the best evidence-based techniques cannot successfully promote positive, long-term change in clients who do not complete the full course of treatment. This book helps therapists and clinical researchers identify the common factors that lead to premature termination, and it presents eight strategies to address these factors and reduce client dropout rates. In fact, 5-10% of adult clients deteriorate during psychotherapy. Although not exclusively due to treatment failures per se, almost a fifth of clients terminate their therapy prematurely and findings suggest that that between 20 and 30% of clients do not return after the first session with half terminating after just two sessions. Therapeutic failures could include a range of negative therapy outcomes, such as harm, deterioration, client non-response, premature termination, or dropout, as well as process factors, such as negative therapy experiences, impasses, or alliance ruptures. Investigating therapeutic failures holds the key to improving the effectiveness of psychotherapy as well as understanding some of the fundamental conditions that need to be in place for the change mechanisms of psychotherapy to take effect. Although psychotherapy has made many strides over the last few decades to improve research rigour and to promote evidence-based practices, it is a profession that is still growing. The highest rate of attrition occurs after the first session. High rates of termination typically continue through the third or fourth sessions. This period is associated with the formation of the therapeutic alliance. This study attempts to predict which client-therapist dyads are likely to terminate prematurely using short paper and pencil instruments administered to clients and therapists before and after the first session. I divided the therapy dyads into three groups: Early premature terminators attending one session, late premature terminators attended between two and four sessions, and continuers attended five or more sessions. I drew the sample from a community mental health training clinic. Sixty-eight dyads participated in the study. Thirty-five dyads provided complete data sets. I assessed interpersonal variables with the INTREX short form questionnaires based on the Structural Analysis of Social Behavior (SASB). Clients and therapists completed the introject forms before the first session. After the first session, the clients and therapists rated each other's interpersonal behavior on the relationship forms. I hypothesized that hostility non-autonomous behavior, excessive focus on the therapist, non-complementary interactions, and conflicted patterns of communication would predict premature termination. The results showed that hostility, autonomous behavior and excessive focus on the therapist predicted premature termination. A discriminant analysis using the INTREX variables was able to correctly classify 85.7% of the dyads. A discriminant analysis using the pre-session INTREX variables was able to correctly classify 83.0% of the first session terminators versus those attending two or more sessions.

Rates of premature termination from counselling and psychotherapy remain high despite a considerable body of research into possible predictors of this phenomenon. Few studies have explored clients' experiences of premature termination in depth. Clients often report dissatisfaction as a reason for premature termination, and this experience is under-researched. From practitioners' perspectives, little is known about indicators of dissatisfaction, and how to manage premature termination if it occurs.The methodology used provides a wide-angled lens on clients' experiences, as well as a detailed exploration of the experience of dissatisfaction in therapy leading to premature termination. As such, the study provides insights into how practitioners could identify the presence of dissatisfaction in therapy as well as offering understanding about the possible diversity of clients' needs if premature termination does occur.
