La sphère médicale n'échappe pas à cette exigence. En échographie, cette démarche qualité a débuté par l'échographie du premier trimestre avec l'établissement du score de Herman pour l'étude de la clarté nucale et le dépistage de la trisomie 21. Aucune étude n'a à ce jour comparé la qualité biométrique et morphologique de l'échographie obstétricale du troisième trimestre à celle du deuxième trimestre. Matériel et méthode : Nous avons réalisé une étude de non infériorité, multicentrique, en aveugle, réalisée du 5 juin 2012 au 10 octobre 2013 à partir d'une série continue de 200 patientes primipares à bas risque. L'objectif était de montrer que le score global de qualité n'était pas inférieur à l'échographie obstétricale du troisième trimestre par rapport au score global de qualité de l'échographie du deuxième trimestre chez une même patiente, à la marge consentie de perte de qualité de un point. Un test de Student apparié, en hypothèse unilatérale, avec alpha à 5 % est réalisé. Le score global s'élève à 38 points et comporte : le cliché du BIP/PC, la coupe de la CA, la longueur fémorale, la coupe du rachis, la coupe des reins, la coupe 4 cavités et la coupe des 3 vaisseaux. Résultats : L'analyse a porté sur 103 binômes d'échographie T2-13. Dans notre étude 79,6% des patientes avaient moins de 30 ans et seul 2,9% de la population était obèse. Le liquide amniotique était en quantité normale aux deux termes d'examen. La présentation foetale majoritaire était céphalique avec le dos latéral aux deux échographies. Les patientes obtenaient à l'échographie T2 une moyenne de 32,37 2,62 sur 38 points au total pour une médiane de 33. A l'échographie T3, elles obtenaient une moyenne de 31,80 ± 3,27 pour une médiane de 33. Soit p = 0,1316. Cependant, la différence entre les moyennes du score global de T2 et T3 est proche (différence de 0,57), ceci est plutôt en faveur d'une non infériorité. Il en est de même pour les moyennes des sous scores biométriques, celles-ci ne sont pas statistiquement significatives mais leurs différences respectives sont inférieures à 1. Ceci est également en faveur d'une non infériorité de la T3 par rapport à la T2. Il en est de même pour la coupe 4 cavité et 3 vaisseaux. Le score qualité du rachis diffère de plus de 1 point en défaveur de la T3, par contre le score moyen de la coupe des reins est supérieur à la T3. Conclusion : Bien que les résultats de notre étude ne soit pas significatif pour une non infériorité de l'échographie T3 par rapport à l'échographie T2 pour une même patiente avec une marge consentie de perte de qualité de 1 point, les différences des scores moyens sont proches et inférieurs à 1, la plus petite différence. Ces résultats sont en faveur d'une non infériorité et vont dans le sens du maintien de l'échographie du troisième trimestre pour le dépistage des malformations foetales et la réalisation de la biométrie foetale de dépistage. Une nouvelle étude comportant un nombre supérieur de patientes est nécessaire pour obtenir un résultat statistiquement significatif.

Unlike the first trimester in which vacuum aspiration is universally con sidered to be the method of choice, in the second trimester of pregnancy the clinician is faced with alternatives; one method may be more appropriate than another in a particular circumstance and no single method is unequivocall~ accepted as best. Neither do second trimester terminations form a neatly defined single category.
