In western society this has been a subject of secrecy and taboo. Throughout our history homosexuals have been persecuted and forced to hide their sexual identity. The effects of this on an individual should be something of concern to professionals in the mental health field. While in the past there has been little knowledge of homosexuality, and no self-expression on the part of homosexuals, there has, in recent years, been growing information and expression. The effect of this change on mental health workers is what this paper will examine. An historical survey will present the forces which shaped the current attitude of the mental health profession about homosexuality. I will present a general examination of the social ideology toward homosexuality in western society. I will also review the history of those homosexuals who have banded together for companionship, reform, and protest. This will be a context against which I will examine the attitudes of a specific group of people who care for the mentally ill--nurses and aides at a state mental hospital. The questionnaire I used to examine their attitudes surveys a broad range of possible attitudes about homosexuality. The responses will demonstrate what changes have occurred in society's attitudes toward homosexuality. It will show, specifically, what the attitudes are of those who care for economically disadvantaged, severely disturbed homosexuals.

Book excerpt: Male homosexuality was illegal in England and Wales from 1533 until 1967 and, along with transvestism, was considered an antisocial "sexual deviation" that could be "cured". Nurses were involved in administering treatments to cure these individuals. This study used oral history interviews with fifteen nurses, along with documentary sources, to examine the meanings that nurses attached to these treatments, and represents the first attempt to examine nurses' perceptions on providing such treatments. The study also conducted oral history interviews with seven patients and explores their experiences of receiving these treatments to obtain a better understanding of the topic in question and claim a 'history from below' which allows historians to see historical practice from a new perspective. The period examined by this thesis was 1935 to 1974. It begins with the publication of the first official report on the use of aversion therapy to treat homosexuality. This publication, along with prejudicial attitudes towards homosexuals and transvestites in the media and in literary, medical, sociological and legal discourses, provided some momentum for the use of aversion therapy to cure these individuals. The period ends in 1974 with the seventh printing of the American Psychiatric Association Diagnostic Statistical Manual version II, which removed homosexuality as a category of psychiatric disorder in the USA. None of the patients in this study reported that the treatment had been effective and all were left feeling emotionally troubled by it. The study explored a number of influences that may have motivated nurses to administer these painful and distressing treatments. Nurses' work was largely constrained by the asylum-type conditions in which they worked, and the character and quality of patient care was largely influenced by the medical staff, who appeared to have overriding control of both the institution and the nurses working within it. In addition, due to their limited knowledge base, nurses believed that it was pertinent for the well-being of a patient that nurses obey medical orders. They took on the status offered to them of obedient order-takers. Nevertheless, from accounts gathered during this study, some nurses covertly undermined their superiors and engaged in subversive behaviours to avoid participating in this aspect of clinical practice. The thesis offers a hitherto undiscovered insight into the role of mental nurses caring for patients receiving aversion therapy for sexual deviation. In doing so, it provides insights into the way nurses may behave when a particular set of social, political and contextual factors are at play. As the first study to focus on exploring the nurses' role in caring for sexually deviant patients, it provides in-depth historical analysis of this subject and related issues as well as a basis for further historical analysis in this area. It is envisaged that this study might also act as a reminder of the need for nurses to ensure that their interventions have a sound evidence base, and that they constantly reflect on the moral and value base of their practice and the influence that science, societal norms and contexts can have on changing views of what is regarded as "acceptable practice".

Book excerpt: The general aim of the project was to describe the situation of lesbians and gay men in a nursing environment by studying the attitudes of nursing staff and students, and the experiences of gay nursing staff in their work environment and of gay patients and partners in their encounters with nursing. The study for papers I and II had a descriptive, comparative design. Nurses, assistant nurses, and nursing students completed the Attitudes Toward Homosexuality Scale (ATHS), the Causes of Homosexuality Questionnaire (CHQ), and the Affect Adjective Checklist (AAC), along with the Nursing Behaviour Questionnaire (NBQ). In general, the participating nursing staff and students expressed positive attitudes, though some subjects reported very negative attitudes toward gay people. The participants also expressed a full spectrum of emotions from delight to anger. The studies for papers III and IV had a descriptive, explorative design. Here, the data collection was performed by semi-structured interviews. The informants were gay nursing staff, gay patients and partners of patients. The gay personnel reported fear and concern about heterosexuals' reactions when these informants "come out," and were constantly assesing the risk being open about their sexual orientation among their workmates. Nearly all patients and partners described several situations where heteronormative assumptions were communicated by the nursing staff. Almost all informants offered recommendations for nursing staff to facilitate communication. According to the presented findings, probably only a small minority in Swedish nursing have negative attitudes toward lesbians and gay men. However, the informants told of heteronormative language and behaviour in nursing contexts that were percieved as insensitive, insulting and humaliating. Nursing staff need to learn how to communicate in a more natural way and to be aware of the norms they communicate through the language and behaviour they use.
