In commercial imaging systems, misalignment between computed tomography (CT) and PET images can introduce biases in the activity distribution. Recently, several reconstruction algorithms have been proposed, which made direct use of single scattered photons in the activity reconstruction. However, the realistic dataset contains single and higher order scattered photons, and current scatter reconstruction methods do not distinguish them. In this study, a novel reconstruction algorithm that is capable of processing higher order scattered photons was developed. A restricted attenuation correction method was created to avoid overcorrecting for scattered photons. The simulation outcomes have shown that the proposed methods can, under ideal energy resolution, reconstruct images that are qualitatively and quantitatively better than those obtained using existing algorithms, and that the methods show promise for use under more realistic clinical conditions.

Book excerpt: Positron emission tomography (PET) is a powerful metabolic imaging modality, which is designed to detect two anti-parallel 511 keV photons origniating from a positron-electron annihilation. However, it is possible that one or both of the annihilation photons undergo a Compton scattering in the object. This is more serious for a scanner operated in 3D mode or with large patients, where the scatter fraction can be as high as 40-60%. When one or both photons are scattered, the line of response (LOR) defined by connecting the two relevant detectors no longer passes through the annihilation position. Thus, scattered coincidences degrade image contrast and compromise quantitative accuracy. Various scatter correction methods have been proposed but most of them are based on estimating and subtracting the scatter from the measured data or incorporating it into an iterative reconstruction algorithm. By accurately measuring the scattered photon energy and taking advantage of the kinematics of Compton scattering, two circular arcs (TCA) in 2D can be identified, which describe the locus of all the possible scattering positions and encompass the point of annihilation. In the limiting case where the scattering angle approaches zero, the TCA approach the LOR for true coincidences. Based on this knowledge, a Generalized Scatter (GS) reconstruction algorithm has been developed in this thesis, which can use both true and scattered coincidences to extract the activity distribution in a consistent way. The annihilation position within the TCA can be further confined by adding a patient outline as a constraint into the GS algorithm. An attenuation correction method for the scattered coincidences was also developed in order to remove the imaging artifacts. A geometrical model that characterizes the different probabilities of the annihilation positions within the TCA was also proposed. This can speed up image convergence and improve reconstructed image quality. Finally, the GS algorithm has been adapted to deal with non-ideal energy resolutions. In summary, an algorithm that implicitly incorporates scattered coincidences into the image reconstruction has been developed. Our results demonstrate that this eliminates the need for scatter correction and can improve system sensitivity and image quality.

Book excerpt: Positron emission tomography (PET) is an imaging modality that can generate quantitative estimates of spatio-temporal distribution of radiotracers within the body in vivo. PET images are often segmented into regions of interest (ROI) to obtain time activity curves (TACs) which can be further analyzed to estimate kinetic parameters. However, segmenting PET images is not trivial because PET has relatively low spatial resolution, high noise, and lacks of anatomical information. In this dissertation we apply level set methods to PET image segmentation and reconstruction. Firstly, we propose an image segmentation method for 4-dimension dynamic PET images. We consider that voxels inside each organ have similar time activity curves. We develop a multiphase level set method that utilizes both the spatial and temporal information in a dynamic PET data set. Both computer simulation and real data experiments show that the proposed method results in smoother segments with less number of misclassified voxels compared to the dynamic clustering method. Secondly, we apply the level set method to PET image reconstruction to improve PET image quality. We propose a new approach to maximum a posteriori (MAP) reconstruction of PET images with a level set function based anatomical prior by taking advantage of anatomical information offered by PET/CT scanners. The image prior models both the smoothness of PET images and the similarity between functional boundaries in PET and anatomical boundaries in CT with no smoothing being applied across functional boundaries in PET images to reduce spill-over effect. Level set functions are used to represent smooth and closed functional boundaries. We conduct computer simulations to evaluate the performance of the proposed method. Compared with existing methods, the new method achieve better bias-variance performance for both phantoms. Finally we extend the anatomical boundary guided level set prior to direct MAP reconstruction of dynamic PET using Patlak kinetic model. The new method has the advantages of accurate modeling of noise prorogation from image reconstruction to kinetic parameter estimation and better delineation of functional boundaries using spatio-temporal information. We use nested algorithm to accelerate the optimization procedure. Computer simulation shows that the proposed method can improve quantitative accuracy of parametric image compared to other existing methods.
