Book excerpt: Physical activity and sedentary behaviour are important markers of health and quality of life, and predictors of functional decline. However, the factors that influence movement patterns in older adults, especially for those residing in the growing assisted living (AL) setting, are poorly understood. I acquired measures from 114 AL tenants of movement patterns from waist-mounted accelerometers worn for at least 3 days. On average, participants spent 86% of their waking hours in sedentary behaviour and 13.84% in light physical activity. The time spent sedentary was higher in males than females, and correlated with scores on the Timed-Up-and-Go and Modified Fall Efficacy Scale, but not with the Montreal Cognitive Assessment or Geriatric Depression Scale. These results indicate that both physical function and psychological factors influence sedentary behaviour in AL tenants. Future research should examine whether interventions targeted at intrinsic or environmental factors decrease sedentary behaviours.

Older adults accumulate large amounts of ST and low amounts of physical activity, which may compound the health risks. There is little research examining strategies to reduce ST among older adults, especially within assisted living residences (ALRs). The purpose of this study was to develop and pilot-test an evidence-informed intervention to reduce ST within ALRs. Stand to Strengthen is a flexible, multi-level intervention that can be delivered in a variety of ALRs. A 6-week feasibility trial using Stand to Strengthen was conducted to examine changes in function, quality of life (QoL), and ST patterns. There was a trend towards less self-reported weekday ST post intervention (p = 0.086) but no significant differences in other variables overall or for higher function participants. Lower function participants demonstrated significant, clinically meaningful improvements in physical function post-intervention (p=0.035, Cohen's d = 0.89). This study demonstrated that a multi-level intervention to reduce ST may delay functional declines as older adults transition to assisted living.

Book excerpt: There is overwhelming evidence linking increased physical activity with positive changes in cognitive functioning and brain health. Much of what we know about these interrelationships comes from aerobic exercise training studies with older adults and children. This literature has paved the way for the neuroscientific investigation of mechanisms responsible for exercise-induced cognitive and brain health enhancement, a list that ranges from molecular changes to systemic changes in executive control and neural connectivity. A new perspective has also emerged that aims to understand executive control processes that may underlie the regulation of health behavior. In accordance with this view, physical activity falls under the umbrella of health behaviors that require a substantial amount of executive control. Executive control is a limited resource, and the aging process depletes this resource. People who regularly exercise are said to have higher “self-regulatory control”—planning, goal-shielding and impulse control—than irregular exercisers. The successful maintenance of physical activity participation in lieu of daily cognitive stressors likely reflects an adaptive resistance to control failures. Indeed, a handful of studies have shown the relationship between greater executive control and subsequently higher levels of physical activity. However, little is known about the neural correlates of physical activity adherence or sedentary behavior, with the view that neurocognitive factors have an antecedent and reciprocal influence on these behaviors. No research has focused on the brain networks responsible for the self-regulation of physical activity, which likely overlaps with structures and functions playing critical roles in the regulation of other health behaviors. Interdisciplinary investigations are needed to explain the extent to which physical activity self-regulation and self-regulatory failure is dependent upon, or under the influence of executive control processes and brain networks. Understanding the degree to which self-regulatory resources may be enhanced, restored, and trained will have enormous implications for basic science and applied fields. It is also of great import to understand whether or not physical activity self-regulation is a domain-specific behavior associated with specific brain networks, or to determine the extent to which regulatory network-sharing occurs. The aim of this Frontiers Research Topic is to curate contributions from researchers in social and cognitive neurosciences and related fields, whose work involves the study of physical activity behavior, self-regulation and executive control. For this Research Topic, we, therefore, solicit reviews, original research articles, and opinion papers, which draw theoretical or empirical connections related to sustained physical activity behavior, self-regulatory strategies, cognitive performance, and brain structure and function. While focusing on work in the neurosciences, this Research Topic also welcomes contributions in the form of behavioral studies, psychophysiological investigations, and methodological innovations. Effective interventions like structural exercise programs that limit functional decline and promote independent capabilities are important to allow older adults to remain in an assisted living facility and delay nursing home placement. The purpose of this dissertation was to investigate the effects of self-efficacy theoretical-based Staying Active, Healthy Aging (SAHA) program using Wii Fit exergames on physical function, fear of falling, cognition, depression, and quality of life in assisted living residents. The factors that influence assisted living residents to exercise were explored as well. The study was a quasi-experimental pre/post-test design with a controlled trial. The Wii exergame group participated in Wii Fit gaming activities and received health education. Four mechanisms of Bandura's self-efficacy theory (Bandura, 1997) were used to enhance residents' exercise self-efficacy, including enactive mastery experiences, vicarious experiences, verbal persuasion, and emotional or physical feedback. The control group only received the health education intervention. The study showed that participants in the Wii exergame group had significantly decreased depressive symptoms and had significant improvements in balance and mobility. The education group showed no significant improvement in any of the outcome measures. Five themes emerged that facilitated participants' desire to exercise: (1) health and mobility; (2) increased alertness; (3) elevated mindset; (4) social interaction; and (5) structured program. Barriers to engage in exercise included: (1) age- or health-related impairments to exercise; and (2) unpleasant experience related to exercise. In conclusion, findings provide support for integrating self-efficacy theory into exergames as a mechanism to encourage older adults to engage in exercise. In addition, exergames show promise as a potential tool to improve and maintain physical and psychosocial health for older adults.

Three studies were completed that were designed to answer questions relating to the association between sedentary behavior and various health outcomes in older adults, how the interaction between sedentary behavior and physical activity affects these associations, a description of time spent in various sedentary behavior domains by older adults, and how age and participation in physical activity affects the dose-reposonse relationship between sedentary behavior and health outcomes. We found that, in older adults, time spent in sedentary behavior is positively associated with biomarkers linked to increased risk of cardiovascular disease and type II diabetes even in those who are active enough to meet the physical activity recommendations for public health. Also, the manner in which sedentary behavior is accumulated is an important predictor of physical function in older adults, whereby taking fewer breaks and spending time in longer bouts of sedentary behavior is associated with worse performance on tests of function. Lastly, the presence of a linear relationship between sedentary behavior and odds of having metabolic syndrome suggests that there is no threshold of sedentary behavior that needs to be met before the risk of harm begins to rise for that particular outcome. Taken together, the findings from these three studies contribute to the growing body of evidence to suggest that the amount of time adults spend in sedentary behavior can have a significant impact on their health. What we found confirms the recommendation by the Physical Activity Guidelines for Americans to "avoid inactivity" in addition to accumulating adequate moderate to vigorous activity, because it appears that increasing amounts of total time and time spent in longer bouts of sedentary behavior are harmful, and participation in physical activity does not protect against this harm. Hopefully, these findings will foster interventions to reduce sedentary behavior and meaningfully contribute to future revisions of the Physical Activity Guidelines with more specific information concerning strategies to avoid excessive sedentary behavior in order to promote health.
