Book excerpt: Financial services are important for women who are starting and growing a business, but in Pakistan microfinance providers (MFPs) are not reaching Pakistan’s businesswomen. Only 59 percent of microfinance clients are women, yet the majority of these loans are passed on the male members of the household – husbands, fathers, and sons. The practice of passing on loans to male household members is quite widespread; women may be bearing all the transaction costs and risks of accessing loans, but are not the final beneficiaries. Second, a very low proportion of female microfinance clients are entrepreneurs. The report explores why businesswomen in Pakistan may not be using microfinance products to meet their start-up and working capital requirements, in spite of identifying access to finance as a key constraint to their business operations. Against this backdrop, access to finance remains the biggest challenge for a woman who wants to start or grow a business. Yet less than a quarter of the entrepreneurs identified through business development service providers were currently borrowing from microfinance lenders. Even among those entrepreneurs that borrow, dissatisfaction is high. Why? Women borrower-entrepreneurs are not able to access individual loan products, but instead are consistently relegated to group lending. But group loans are very costly for a woman who is running a business, and the loans are too small to fulfill working capital needs. Businesswomen are rarely given the opportunity to access individual loan products, which are usually offered exclusively to male borrowers, and women are not given opportunities to graduate from group loans to individual loans over time. Lending practices often are discriminatory, requiring husbands’ permission, male guarantors, and unmarried women are rarely considered as potential clients. Although MFIs understand that women’s inclusion is integral to the objectives of microfinance, the practice of passing on loans raises serious issues about consumer protection for women clients, and the best and most effective solutions to these challenges could and should come from the sector itself. Designing better products that reach the needs of emerging women entrepreneurs could prove to be good business, achieving double bottom-line objectives. Investing in financial literacy and education of both men and women borrowers can help curb the demand for pass-through loans and help lower risks associated with deceptive practices.

Book excerpt: As the work on a revised edition of the Diagnostic and StatisticaL ManuaL (OSM-llIR) progressed, a great controversy grew over the inclusion of a new diagnostic category, "Premenstrual Phase Dysphoric Disorder." Some nosologists and scientists who study premenstrual syndrome (PMS) felt that, while a specific psychiatric disorder does exist, it occurs relatively rarely. The disorder can be characterized by recurrent periods of dysphoria on a monthly basis, in synchrony with the menstrual period. "PMS" already exists as a diagnosis in leD 9, the international medical nomenclature. The category for DSM-IIIR was to be a specific psychiatric disorder concentrating on the dysphoric reaction, and not including all of the physical and mental symptoms that people have ascribed to this condition. Much of the controversy that ensued had little to do with the diagnostic category or the condition itself. Rather, it concerned feelings voiced by feminist groups that the new diagnostic category would be misleading, that it would inappropriately label women as mentally ill, and that it would be affixed not only to the dysphoric disorder, but everything else that happens psychiatrically to women.

Book excerpt: Psychosomatic diseases are a group of diseases closely related to psychosocial factors but mainly manifested by somatic symptoms, involving respiratory, digestive, endocrine, and other systems. As a result, the lack of consensus on its diagnosis has plagued clinical treatment in internal medicine, surgery, and psychiatry for decades. In recent years, research on the pathogenesis of psychosomatic diseases has made significant progress. For example, Franz Alexander believes that unresolved subconscious conflicts are the main cause of psychosomatic disorders. The subconscious psychological conflict is caused by the changes in the functional activities of the autonomic nervous system, acting on the corresponding special organs and patients with susceptible qualities. Similarly, mental and psychological factors affect gastrointestinal sensory and motor functions through the autonomic nervous system, brain-gut axis, and neuroendocrine system. Meanwhile, gastrointestinal symptoms also affect emotions and behaviors through the brain-gut axis. In addition, the Cannon-Bard theory of emotional physiology and Pavlovian theory of higher neural activity types from quantitative research methods to study the relationship between conscious psychological factors, such as emotions, and measurable physiological and biochemical changes. In clinical work, the treatment of psychosomatic diseases has gradually shifted from emphasizing physical treatment to comprehensive treatment principles, that is, taking into account the psychological and behavioral aspects of the physical treatment of the primary disease. The main purpose of physical treatment of the primary disease is to control or relieve symptoms. To consolidate the treatment of psychosomatic diseases and reduce the recurrence of psychosomatic diseases, combining physical therapy with necessary psychotherapy would potentially obtain a more comprehensive curative effect.
