Numerous and scattered, these beneficial multipurpose systems combine productive with domestic water uses. However, their environmental and health impacts remain insufficiently documented. This report combines data from different sources into syntheses and national maps, with a focus on water-related diseases. The mitigation of negative impacts requires an integrated approach to specifically identify the enhancing and limiting factors that influence environmental impacts and the transmission of diseases around reservoirs. Public awareness campaigns need to accompany the promotion of preventive and curative measures and the development of alternative water sources for domestic supply.

The variation in health may present as differences in the occurrence and spread of diseases, the distribution of and access to healthcare facilities, and/or in health outcomes among the population. Reasons for these variations range from biology to the population’s levels of exposure and susceptibility to elements in their environment, including the social interactions taking place within the environment. The field of health and medical geography focuses on the spatial patterns and processes underlying these variations and provides pathways for understanding and addressing them. More specifically, the sub-discipline of health and medical geography focuses on, among others, how places (their characteristics and processes that go on in them) and environmental factors underlie and/or influence disease patterns, exposure and susceptibility to diseases, health variations, health behavior, health outcomes, and the provision of and access to healthcare services. This volume documents perspectives and applications in health and medical geography in Africa for academics, students, health practitioners, and development policymakers.

Book excerpt: At the present time, malaria is responsible for a million deaths a year, with 500 million reported cases of the disease and 2.5 billion people at risk of contracting it. The distribution and severity of the disease vary with the causative agents, vectors and environment. Of the 4 possible parasites, only P. falciparum causes fatal forms; the three others have debilitating effects related to frequent disease recurrence and reviviscence. More than 50 species of anophele are involved in the obligatory transmission of the parasite from man to man. Climate, environment and biogeography condition the distribution of anophele species and modulate the intensity of transmission. This is what is known as the biodiversity of malaria. At the present time, more than 90% of P. falciparum malaria deaths occur in tropical Africa where only 10% of the world's population lives. A child dies of malaria every 30 seconds. This continent is home to the most effective vectors and the climate favours transmission. Severe cases also arise in the forested areas of South East Asia, New Guinea and the Amazon region. Throughout the rest of the tropical and subtropical world, the disease caused by P. vivax and/or P. malariae is less serious.

Book excerpt: Only 4 percent of arable land in sub-Saharan Africa is irrigated, using just 2 percent of the available water resources. Furthermore, 18 percent of the area equipped for irrigation is not utilized at all and the intensity of use varies between 50 percent and 80 percent. This highlights the huge potential available for intensifying and expanding irrigated area, provided that the investments required can be successfully mobilized. However, it must be noted that if investments in irrigation are to yield satisfactory returns, investments must also be made in a series of related activities. Current global figures for the amount of private investment in irrigation confirm that good returns can indeed be achieved. Prospects for sub-Saharan Africa would be far more favorable if public development assistance, particularly foreign direct investments, did not show declining trends.
