Dans d'autres pathologies chroniques, une collaboration médico-pharmaceutique et des soins pharmaceutiques structurés ont montré un impact clinique positif. Les interventions initiées à l'hôpital, poursuivies en ville et maintenues dans la durée ont plus d'impact que des démarches isolées et sur le court terme. A l'étranger, des études rapportent l'impact positif de l'accompagnement par le pharmacien de ville, en collaboration avec d'autres professionnels de santé, sur l'adhésion médicamenteuse des patients. Concernant la polyarthrite rhumatoïde, la revue de la littérature montre que les interventions visant à optimiser l'adhésion médicamenteuse présentent des résultats modérés. Néanmoins, la poursuite de l'évaluation de nouveaux modèles d'interventions est encouragée. L'objectif de ce travail était double. Dans un premier temps, il s'agissait d'évaluer les perceptions et le ressenti des pharmaciens de ville sur l'intérêt et la faisabilité d'entretiens pharmaceutiques d'accompagnement de patients atteints de polyarthrite rhumatoïde. Dans un second temps, il s'agissait d'élaborer un protocole proposant une intervention pluriprofessionnelle pour améliorer l'adhésion médicamenteuse dans la polyarthrite rhumatoïde. Une analyse qualitative par entretiens semi-dirigés auprès de 15 pharmaciens de ville a permis de souligner des difficultés de recrutement des patients pour les entretiens actuellement possibles à l'officine (Anti-Vitamine K et asthme essentiellement), la complexité de l'organisation et du financement, une faiblesse du lien ville-hôpital mais une motivation des pharmaciens pour élargir le dispositif à d'autres pathologies. Concernant des entretiens destinés à des patients atteints de polyarthrite rhumatoïde, les pharmaciens les verraient sous forme d'entretiens structurés préférentiellement à l'officine, en lien voir « prescrits » par les médecins pour un partage d'information optimal. Une formation préalable adaptée à leurs attentes ainsi qu'un financement de ces entretiens devaient être envisagés pour motiver les pharmaciens à cette activité. Enfin, l'importance d'une évaluation rigoureuse du dispositif d'entretiens a été avancée. En prenant en compte ces éléments, nous avons élaboré le protocole d'une étude multicentrique et randomisée visant à évaluer l'impact d'une intervention pluriprofessionnelle, initiée à l'hôpital et poursuivie en ville, reposant sur des soins pharmaceutiques structurés (conciliation médicamenteuse, analyse de prescriptions, entretiens d'informations à l'hôpital et en ville sur les traitements de la polyarthrite rhumatoïde). Le critère de jugement principal serait l'adhésion médicamenteuse des patients atteints de polyarthrite rhumatoïde. Cette intervention pourrait entrainer une diminution de l'iatrogénie médicamenteuse et des recours aux soins par une meilleure adhésion médicamenteuse des patients.

Ces traitements, présentent, comme pour toute substance médicamenteuse des effets indésirables. Dans le cas des molécules responsables d'actions profondes sur diverses voies immunologiques, les aspects de tolérance et de sécurité sont d'autant plus importants qu'ils sont considérés comme étant essentiel car très influant dans l'observance des patients PR. Ce travail de thèse consiste en l'amélioration des connaissances sur les effets indésirables des thérapies médicamenteuses en se basant essentiellement sur la revue bibliographique. Cette revue a concerné, principalement, les méta-analyses, Cochrane, revues systématiques puis les essais randomisés contrôlés. Nous avons accordé une grande importance aux données issues d'études de post-commercialisation ainsi qu'aux études de cohortes observationnelles à grande échelles. Les événements indésirables rencontrés par les patients peuvent être considérés comme mineurs, En ce qui concerne le risque plus grave de cancer après expositions aux traitements PR, globalement, il semble admis que les patients atteints de PR sont concernés par un risque plus élevé de tumeurs malignes par rapport à la population générale.
