L’allaitement maternel offre de nombreux avantages à l’enfant mais aussi à la maman. De plus, la composition du lait maternel varie au cours de la lactation et de la tétée, de manière à être constamment adaptée aux besoins du nourrisson.Les femmes ont droit à un congé maternité de dix semaines après l’accouchement, ce qui complique l’allaitement maternel jusqu’aux six mois recommandés par l’OMS. La reprise de l’activité professionnelle est la cause de quasiment 37% des motifs de sevrage, même si le code du travail prévoit une heure d’allaitement dans la journée. Cependant, allaiter et travailler restent possibles à condition d’organisation. L’utilisation d’un tire-lait avant même la reprise du travail permet de réaliser des stocks de lait que la maman congèlera. Après la reprise, le principe de base est que, lorsque l’enfant est avec sa mère, il a une alimentation au sein, lorsqu’il est chez l’assistante maternelle ou à la crèche, il est nourri avec des biberons préparés à partir des stocks de lait maternel réalisés par la maman. Ce système d’utilisation de tire-lait conjugué à la congélation du lait maternel permet ainsi à la maman active d’allaiter malgré son activité professionnelle.

Pour les mamans qui souhaitent suivre ces recommandations, cela peut signifier poursuivre l'allaitement audelà de la reprise d'une activité professionnelle. Ce qui demande de l'organisation et surtout du soutien. Nous avons donc souhaité réaliser un guide pratique destiné à ces mamans. Nous avons utilisé la méthode décrite par l'HAS en 2008 pour l'élaboration d'un document écrit d'information destiné aux patients et aux usagers du système de santé. Nous avons effectué une recherche bibliographique afin de sélectionner les informations validées utilisées pour le contenu initial de la fiche. Nous avons ensuite travaillé en collaboration avec des mamans, des sages-femmes, des médecins généralistes et des puéricultrices : le document initial a été soumis pour discussion à des groupes de travail puis validé lors d'entretiens individuels. Deux groupes de travail et cinq entretiens individuels ont été réalisés. Les items abordés dans le document ont été : composition des repas en l'absence de la maman, conservation et utilisation du lait maternel, matériel nécessaire, droit du travail, difficultés possibles, contacts en cas de questions supplémentaires. La partie "difficultés" a été ajoutée lors du premier groupe de travail. Les autres modifications issues de ce groupe ont porté sur la mise en évidence de certaines informations jugées importantes et l'ajout des recommandations de l'OMS concernant la durée de l'allaitement. Les modifications apportées par le second groupe ont concerné les tournures des phrases, l'ajout de la nécessité de conserver au moins trois tétées par jour et de la notion de confiance en soi. Les entretiens individuels ont témoigné d'une globale satisfaction. Il a été décidé de diffuser le document sous forme papier et sur Internet. La prochaine étape sera de diffuser le document. Il faudra ensuite évaluer l'impact du document et actualiser les données

En outre, sur 10 ans, au moins 91 millions d' enfants supplementaires auraient ete traites pour emaciation severe et 105 millions de nourrissons additionnels auraient ete beneficiaires d'un allaitement matemel exclusif au cours de leurs premiers six mois de vie. L'atteintede l'ensemble de ces cibles permettrait d'eviter au moins 3,7 millions de mortalites infantiles. Chaque dollar investi dans ce paquet d'interventions genererait des rendements econorniques de4 a 35 dollars, ce qui fait du financement de la nutrition precoce l'une des interventions de developpement les plus rentables. Certaines des cibles - particulierement celles qui sont axees sur la reduction du retard de croissance chez l'enfant et de I'anemie chez la femmeparaissent ambitieuses et demanderont des efforts concertes de financement, de mise a l' echelle et d' engagement continu; toutefois, l' experience tiree de plusieurs pays indique que leur atteinte reste possible. Les investissements au cours des 1000 premiers jours decisifs de la petite enfance sont inalienables, transportables et porteurs de retombees tout au long de la vie - non seulement pour les enfants directement concernes, mais aussi pour nous tous - sous forme de societes plus robustes, qui agissent comme moteur des economies a venir.

This “bleeding of the nation” gave birth to a vast movement for child welfare that paved the way for a medicalization of childbearing. In Babies for the Nation, basing her analysis on extensive documentary research and more than fifty interviews with mothers, Denyse Baillargeon sets out to understand how doctors were able to convince women to consult them, and why mothers chose to follow their advice. Her analysis considers the medical discourse of the time, the development of free services made available to mothers between 1910 and 1970, and how mothers used these services. Showing the variety of social actors involved in this process (doctors, nurses, women’s groups, members of the clergy, private enterprise, the state, and the mothers themselves), this study delineates the alliances and the conflicts that arose between them in a complex phenomenon that profoundly changed the nature of childbearing in Quebec.
