Despite the prevalence of alcohol-related risk and problems, trauma centers do not currently provide screening and effective brief intervention as part of routine care. Because excessive drinking is a significant risk factor for injury, it is vital for trauma centers to have protocols in place to identify and help patients. They are in an ideal position to take advantage of the teachable moment generated from an injury by implementing screening and brief intervention (SBI) for at-risk and dependent drinkers. Brief alcohol interventions conducted in trauma centers have been shown to reduce trauma recidivism by as much as 50%. Such interventions also reduce rates of arrest for driving under the influence and cut health care costs. For these reasons, routine care in trauma centers should include screening patients for alcohol misuse, providing brief interventions for patients who screen positive, and, when needed, referring patients to specialty assessment and treatment.

Book excerpt: Alcohol is the sixth leading risk factor for disability and premature death all over the world, and one of the leading causes of premature mortality in western societies; it is a leading risk factor for death in young and middle-age males. Heavy drinking accounts for about two thirds of the burden of disease attributable to alcohol. In the early 1980s, screening and brief interventions (SBI) in primary health care settings were proposed as effective strategies to identify risky drinkers and to help them reduce their drinking. Since then, a growing body of evidence, including several meta-analysis and Cochrane reviews, has shown the efficacy and effectiveness of SBI in primary health settings. However, demonstrating the effectiveness of SBI has not been insufficient to facilitate its general implementation in the routines of primary health care physicians, and in fact the dissemination of SBI has proven to be a difficult business. Qualitative and quantitative research has identified most of the facilitators and barriers for its implementation, and publicly funded research has been earmarked to address the dissemination problems worldwide. Some examples are the World Health Organization Phase III and Phase IV studies on the identification and management of alcohol-related problems in primary care, EU funded projects (PHEPA, AMPHORA, ODHIN, BISTAIRS), the UK SIPS trials and the SBIRT developments sponsored by the Substance Abuse & Mental Health Services Administration (SAMHSA) in the USA. The efficacy and effectiveness of SBI in primary health is now well established, but there are still some questions that remain unsolved: which practitioners should deliver them; what length should they be; is there a need for booster sessions; is there added value of a motivational approach? These questions, together with other relevant aspects of SBI, need ongoing research. In recent years, SBIs have been tested in settings other than primary health care, including hospitals, accident and emergency rooms, criminal justice, colleges and universities, social services and pharmacies. In some of those areas, the evidence is scarce (for example, pharmacies) while in others it is very promising (for example, students and hospitals). New technologies have also offered the possibility of online tools, and, in the last few years, different digital-based applications have been tested successfully as new ways to deliver effective SBIs to larger amounts of people. Brief interventions have also spread to drugs other than alcohol. This book aims to be an update of the state-of-the art of brief advice. It is a compilation of articles published by some of the most relevant researchers in the field in Frontiers in Psychiatry between 2014 and 2016.

Book excerpt: Alcohol Screening, Brief Intervention (SBI) has been widely implemented in medical facilities since 2010 when it became mandatory in all level 1 trauma centers. Many studies have evaluated the effectiveness of SBI by following up on patients screened to be at-risk drinkers. Most of these studies have reported a large loss to follow-up as their main limitation but few have tried to investigate the characteristics of those non-compliant to follow-up. This retrospective study analyzed demographic characteristics, Alcohol Use Disorder Identification Test (AUDIT) and readiness to change scores for 190 lost-to-follow-up and 221 completed follow-up patients who had undergone SBI at the University of California, Irvine (UCI) Emergency Department between June 2006 and May 2007. Comparing the association between baseline characteristics and completing follow-up, those 30 and older have lower odds of completing follow-up in comparison to those 18-29 years of age. AUDIT and readiness to change scores were not significantly different between two groups. Those lost to follow-up had experienced more negative consequences of alcohol than those completed follow-up. Using linear regression, after adjusting for sex, language, ethnicity, and readiness to change scores, experiencing more negative effects of alcohol was still associated with less completed follow-up. These results demonstrate differences between those lost and those compliant to follow-up in such studies, which suggest a need for a more tailored approach to patients. This can result in more effective motivational intervention and commitment to change for those lost to follow-up, which also improves the internal validity, and generalizability of study results.

It has been well documented that there is a strong connection between alcohol and traumatic injury. Researchers have also found that interventions for alcohol abuse are effective in reducing recurrent episodes of traumatic injury. Assessment of brief interventions of trauma patients was used to determine the current practice in a selected level III trauma center. This descriptive study utilized a retrospective chart review to evaluate the initiation and implementation of brief interventions. The theoretical framework for this study was based on the Quality Assurance Model Using Research (QAMUR). Data from 150 charts were analyzed. Findings revealed that brief interventions were provided for only 17.1% of trauma patients. These findings demonstrate a need for further research in the development of standardized guidelines and protocols to impact the reduction of repeated traumatic injury as the result of alcohol misuse.

Book excerpt: Those who misuse alcohol in the United States do not regularly seek treatment on their own to reduce use and avoid consequences of misuse. Because of the association between alcohol misuse, alcohol-related risk behaviors, and injury; alcohol misuse in the United States has serious societal and individual repercussions. To alleviate these problems, health care professionals; including doctors, nurses, and social workers; have an opportunity to screen injured patients for alcohol misuse and provide brief interventions. Although some brief intervention research has demonstrated reductions in alcohol misuse and other injury-related behaviors, other evidence indicates that brief alcohol interventions are not equally effective for all injured patients. Moreover, screening and brief alcohol interventions are not reimbursed in most states, leaving providers and medical centers uncompensated for providing services. A possible way to address these challenges is to target intervention services to patients who are most likely to make positive changes. Therefore, this dissertation used mixture modeling to identify subclasses of injured patients based on their past injury-related consequences and risks of alcohol misuse in order to describe which subgroups made the greatest reductions in drinking in the year following discharge from a Level-1 trauma center. This dissertation also identified which subclasses of patients made the greatest behavioral improvements for injury-related consequences and risks of alcohol misuse during the year following discharge from the trauma center. Patients with profiles that contained high probabilities of multiple consequences and risks and those with histories of alcohol-related accidents and injuries reported some of the largest improvements in drinking and injury-related consequences and risks following discharge. Those classes that made the fewest changes had profiles that consisted of fighting and taking foolish risks while drinking or that consisted of low probabilities of risks or consequences of alcohol misuse. This dissertation provides tentative evidence for targeting intervention services to injured patients. Further research should verify which subclasses of patients are most likely to reduce alcohol misuse and other alcohol-related risk behaviors in order to more effectively target brief alcohol interventions, increase cost savings, and improve the health and behavioral health outcomes for injured patients who misuse alcohol.

Book excerpt: Background: Alcohol screening and brief alcohol interventions (SBI) are ranked the third highest prevention priority for US adults. Although the US Preventive Services Task recommends routine SBI, the optimal screening interval is unknown and brief alcohol interventions (BI) have been challenging to implement. Objectives: To 1) evaluate the yield of repeat annual alcohol screening for patients who initially screen negative on 1-4 prior negative alcohol screens in the population as a whole and in subgroups of patients; and 2) estimate the prevalence of BI with alcohol misuse before, during, and after implementation of a national performance measure (PM) and dissemination of an electronic clinical reminder (CR) for BI. Methods: 1) Using VA electronic medical record data for more than 360,000 outpatients (2004-2008) the probability of a positive final screen after 1 -4 prior negative screens was estimated using adjusted logistic transition regression models that included prior negative screen scores as explicit predictors. Results are also presented as the number needed to screen (NNS) for one patient to screen positive on the final screen. 2) Among 6,788 VA outpatients whose medical records were reviewed for quality improvement (7/06-9/08) the adjusted prevalence of documented BI (advice and feedback) across the 4 phases of BI implementation was estimated using a generalized log-linear regression. Results: Age, gender and prior negative screen scores were the strongest predictors of a positive final screen. As the number of prior negative screens increased from 1-4, the probability of a positive final screen decreased and those who consistently reported no drinking the past year had the lowest probabilities. Depending on prior negative screen scores, age, and gender, the NNS ranged from 3 - 159. Among patients with alcohol misuse, the prevalence of BI increased significantly over successive phases of BI implementation, from 5.5% during the baseline year to 29.0% after CR dissemination. Conclusions: Extending the screening interval beyond a year may be appropriate for some patient groups with a low likelihood of a positive screen in settings that lack the resources to screen and follow-up on all patients annually. The VA's strategy of implementing BIs with a PM and CR meaningfully increased documentation over a 1-year period.
