Book excerpt: On January 26, 2020, a Sikorsky S-76B helicopter, N72EX, being operated by Island Express Helicopters Inc. as a Title 14 Code of Federal Regulations (CFR) Part 135 on-demand flight, crashed into terrain in Calabasas, California. The pilot and the eight passengers died. One of the passengers was retired basketball player Kobe Bryant. Safety issues include the pilot's preflight weather and flight risk planning, the flight's entry into instrument meteorological conditions and the pilot's inadequate adverse weather avoidance, the pilot's spatial disorientation, influences on the pilot's decision to continue the flight into adverse weather, Island Express' incomplete implementation of its safety management system (SMS), the benefits of a mandatory SMS, the benefits of flight simulation devices for pilot training in adverse weather avoidance, the benefits of a flight data monitoring program, and the value of crash-resistant flight recorder systems in preventing future accidents. As a result of the investigation, the National Transportation Safety Board makes two new safety recommendations each to the Federal Aviation Administration and Island Express.

Book excerpt: On March 23, 2004, about 1918:34 central standard time, an Era Aviation Sikorsky S-76A helicopter, N579EH, crashed into the Gulf of Mexico about 70 nautical miles south-southeast of Scholes International Airport (GLS), Galveston, Texas. The helicopter was en route to the drilling ship Discoverer Spirit. The captain, copilot, and eight passengers aboard the helicopter were killed, and the helicopter was destroyed by impact forces. The flight was operating under the provisions of 14 Code of Federal Regulations Part 135 on a visual flight rules flight plan. Night visual meteorological conditions prevailed at the time of the accident. The National Transportation Safety Board determines that the probable cause of this accident was the flight crew's failure to identify and arrest the helicopter's descent for undetermined reasons, which resulted in controlled flight into terrain.

About 3.2 miles north of the runway 19R threshold at ADW, during an instrument landing system approach, the helicopter impacted terrain and crashed. The pilot, one flight paramedic, one field provider, and one of two automobile accident patients being transported were killed. The helicopter was substantially damaged when it collided with trees and terrain in Walker Mill Regional Park, District Heights, Maryland. The National Transportation Safety Board determined that the probable cause of this accident was the pilot's lack of experience and lacking support of supporting institutions.

Book excerpt: “Surviving is a true blessing that few get to experience.” Being strapped in the seat of a plane as it plunges into a nosedive midair is everyone’s worst nightmare. Bracing for Impact’s compilers and contributors know. They have both lived out that fear and survived, albeit badly hurt, in their own plane crashes. In this collection of true-life survivor tales, people from all walks of life—a freelance writer, a crew member of the Lynyrd Skynyrd band, a naval flight surgeon, a teenager, and a newlywed on her honeymoon, among others—recount their traumatic narrow escapes as engines stalled, fuel ran out, hazardous weather conditions descended, and landings did not go according to plan. In the face of death, as life flashed before their eyes—or not, as some wryly note—these survivors encountered the terrific split of before and after the crash. Their lives, though preserved, would change forever. Perhaps more significant than the crash itself is how each story plays out in the aftermath of the ordeal. In heart-wrenching, unrelenting honesty, these stories explore the wide spectrum of impacts on survivors—ranging from debilitating fear, post-traumatic stress disorder, and substance abuse, to a renewed sense of urgency, where survivors swear to live each day to the fullest and rededicate their lives to helping others. Including the 1977 story of the Lynyrd Skynyrd plane crash that killed lead singer Ronnie Van Zant, guitarist Steve Gaines, and vocalist Cassie Gaines, Bracing for Impact is as much a horrific account of air disasters as it is a celebration and recognition of the people who survived them. Fans of the 2016 Clint Eastwood film Sully starring Tom Hanks will enjoy this edge-of-your-seat read!

Book excerpt: In 1966, Pan American Airways reached the zenith of its wealth & influence. Its pilots were lords of the sky; Skygods. Under aviation pioneer Juan Trippe's autocratic control, Pan Am bought jet airliners before its competitors & made record profits. It was the first U.S. airline to order the supersonic transport; it accepted reservations for the first service to the Moon. Then Pan American Airways fell to earth. In Skygods, Robert Gandt, a Pan Am pilot for 26 years, gives an inside account of the great airline's unprecedented demise. He interviewed hundreds of former Pan Am airmen & executives. He reveals how Pan Am's captains, in Navy-style uniforms, once commanded their ships like petty tyrants. They were the best & brightest in airline industry, but there were disturbing stories of captains who allowed stewardesses to land their aircraft, flew them at the wrong altitude & in the wrong direction & who tragically disappeared, often without a trace. All was not well either in the Pan Am Building, the massive landmark in New York where a succession of impulsive & short-sighted CEOs combined to preside over the demise of a great airline. Pan An bought a domestic airline it did not need; bought aircraft it did not need & operated half-empty planes on low-density routes. It sold the entire Pacific network for a bargain price & sold precious assets to meet its payrolls. And then came the Lockerbie tragedy. This is a fascinating account of what can go wrong with a pillar of strength of the U.S. industry, when its leaders lose their sense of direction & when their star employees-the Skygods-discover that they are mere mortals.
