Book excerpt: On July 8, 2006 at 22:44 UTC, as it was landing at Irkutsk airport, an А-310 airplane, registration F-OGYP, operated by Sibir Airlines AS Flight C7 778, ran down the runway, overran the runway threshold and, at a distance of 2140 m and on a magnetic azimuth of 296° from the aerodrome reference point, collided with barriers, broke apart and burst into flames. As a result of the accident 125 individuals died, including both pilots and 3 of the cabin crew; 60 passengers and 3 cabin crew suffered physical injuries of varying degrees of severity. The actions of the crew from the onset and in the development of an emergency situation revealed shortcomings in the professional training of both the airplane captain and the co-pilot. The real cause of the accident was pilot error due to lack of training and experience.

Book excerpt: The Lockheed 1011 registered A6-BSM, operated by Star Jet and chartered by Olympic Airlines, arrived on 4 July 2005 at Terminal 1 at Paris Charles de Gaulle airport. Departure was delayed because the forward hold door could not be closed. A mechanic tried to close the door manually with a hammer and a chuck. Some passengers, worried about the apparent state of the cabin and the noise, asked to disembark, and this led to a mass movement. The airplane took finally off at 16h17. Shortly after departure the crew noticed problems with engine number 3. The captain requested the SEVERE DAMAGE procedure and returned to the airport. The French Bureau d'Enqu tes et d'Analyses pour la s curit de l'aviation civile (BEA) investigated the incident. BEA found out that the aircraft suffered from many problems, such as leaking fuel, malfunctioning safety features and lacking maintenance. The flight crew was not properly licensed, the captain was too old to fly in Europe. The Lockheed Tristar was a flying coffin.

Only after the crash of USAir 427 in 1994 and a similar incident with Eastwind 517 in 1996 the NTSB was able to pinpoint the cause of this crash: jammed rudder. The Boeing 737 has a history of rudder system-related anomalies, this finally solved the mystery of sudden jamming of the rudders of this aircraft.

Kennedy International Airport, New York, to Miami International Airport, Miami, Florida, crashed at 2342 eastern standard time in the Everglades, approximately 18 miles west northwest of Miami International Airport. The aircraft was destroyed. There were 163 passengers and a crew of 13 aboard the aircraft, 99 people died in the crash. The flight was diverted because of problems with the nose landing gear The aircraft climbed to 2,000 feet while the crew attempted to correct the problem. Surviving passengers and crewmembers stated that the flight was routine and operated normally before impact with the ground. The National Transportation Safety Board determines that the probable cause of this accident, was preoccupation with a malfunction of the nose landing gear position indicating system distracted the crew's attention from the instruments and allowed the descent to go unnoticed.

Book excerpt: On 28 November 2008, a Boeing 777-200ER, operated by British Airways as flight BA38, on its way from Beijing, China to London (Heathrow), suffered on approach to Heathrow Airport an in-flight engine rollback. At 720 feet agl, the right engine ceased responding to autothrottle commands for increased power and instead the power reduced to 1.03 Engine Pressure Ratio (EPR). Seven seconds later the left engine power reduced to 1.02 EPR. This reduction led to a loss of airspeed and the aircraft touching down some 330 m short of the paved surface of Runway 27L at London Heathrow. The investigation identified that the reduction in thrust was due to restricted fuel flow to both engines. It was determined that the restriction occurred most probably in the Fuel Oil Heat Exchangers. The investigation identified the forming of ice in the fuel system as probable cause. The aircraft was destroyed, but there were no casualties.

Book excerpt: The Boeing 737 has a history of rudder system-related anomalies, including numerous instances of jamming. A number of accidents and incidents were the result of the airplanes' unexpected movement of their rudders. During the course of the four and a half year investigation of the crash of USAir Flight 427 near Aliquippa, Pennsylvania, killing 132 people, the NTSB discovered that the PCU's dual servo valve could jam as well as deflect the rudder in the opposite direction of the pilots' input, due to thermal shock, caused when cold PCUs are injected with hot hydraulic fluid. This finally solved the mystery of sudden jamming of the rudders of this aircraft.

This book tells the story of how engineers and scientists finally uncovered the defect that had been engineered into the plane. One of its novel features is that it portrays the complex interaction of different experts and opposing interests in investigating and solving the mystery of this single crash.

Available in PDF, EPUB and Kindle. Book excerpt: This fascinating story explains how aviation crashes are investigated, and what goes on behind the scenes to improve safety. It is also the untold saga of how one maverick scientist battled the bureaucracy to save lives. Federal officials hired him to prevent an anticipated bloodbath from airline deregulation. He soon introduced innovations, such as Crew Resource Management training, which dramatically reduced airline accidents. However, when he dared expose lies to Congress, officials used the sky marshals to harass him. They then ignored his other programs, which contributed to countless unnecessary deaths -- including JFK Junior's. Becoming a military safety guru, his important tasks included training Air Force One crews, and going undercover to discover why a mysterious Soviet airliner crash killed an African president. But he was fired for blowing the whistle on the Pentagon cover-up of the worst fratricide since Vietnam. Congress and other important organizations have often sought his advice on civil and military aviation problems.

Book excerpt: On 28 December 2014 an Airbus A320-216 aircraft registered as PK-AXC was cruising at 32,000 feet on a flight from Juanda Airport, Surabaya, Indonesia to Changi Airport, Singapore with total occupants of 162 persons. The Pilot in Command (PIC) acted as Pilot Monitoring (PM) and the Second in Command (SIC) acted as Pilot Flying (PF). The Flight Data Recorder (FDR) recorded that many master cautions activated following the failure of the Rudder Travel Limiter which triggered Electronic Centralized Aircraft Monitoring (ECAM) message of AUTO FLT RUD TRV LIM SYS. The crew tried repeatedly to reset the computers but the autopilot and auto-thrust disengaged and the flight control reverted to Alternate Law. The investigation showed that the loss of electricity and the RTLU failure were caused by a cracked solder joint. All occupants of the plane were killed in the accident.

Book excerpt: On 25 January 2010, at 00:41:30 UTC, Ethiopian Airlines flight ET 409, a Boeing 737-800, on its way from Beirut to Addis Abeba, crashed just after take-off from Rafic Hariri International Airport in Beirut, Lebanon, into the Mediterranean Sea about 5 NM South West of Beirut International Airport. All 90 persons on board were killed in the accident. The investigation concluded that the probable causes of the accident were pilot errors due to loss of situational awareness. Ethiopian Airlines refutes this conclusion. Other factors that could have lead to probable causes are the increased workload and stress levels that have most likely led to the captain reaching a situation of loss of situational awareness similar to a subtle incapacitation and the F/O failure to recognize it or to intervene accordingly. Ethiopian Airlines refutes the investigation. According to the airline the final report was biased, lacking evidence, incomplete and did not present the full account of the accident.

Book excerpt: During the night of 04th May 2007, the B737-800, registration 5Y-KYA, operated by Kenya Airways as flight KQA 507 from Abidjan international airport (C te d'Ivoire), to the Jomo Kenyatta airport Nairobi (Kenya), made a scheduled stop-over at the Douala international airport (Cameroon). The weather was stormy. A number of departing planes decided to wait for the weather to improve. Kenya Airways, however, decided to depart. Shortly after take-off at about 1000 ft, the aircraft entered into a slow right roll that increased continuously and eventually ended up in a spiral dive. On the 5th May 2007 at approximately 0008 hrs, the airplane crashed in a mangrove swamp South-South/East of Douala. All 114 people on board were killed and the airplane was completely destroyed. The airplane crashed after loss of control by the crew as a result of spatial disorientation, after a long slow roll, during which no instrument scanning was done, and in the absence of external visual references in a dark night.

This book was released on 2014-04-22 with total page 306 pages. Available in PDF, EPUB and Kindle. Book excerpt: On January 15, 2009, about 1527 eastern standard time, US Airways flight 1549, an Airbus Industrie A320-214, N106US, experienced an almost complete loss of thrust in both engines after encountering a flock of birds and was subsequently ditched on the Hudson River about 8.5 miles from LaGuardia Airport (LGA), New York City, New York. The flight was en route to Charlotte Douglas International Airport, Charlotte, North Carolina, and had departed LGA about 2 minutes before the in-flight event occurred. The 150 passengers and 5 crewmembers evacuated the airplane via the forward and overwing exits. One flight attendant and four passengers were seriously injured, and the airplane was substantially damaged beyond repair. The National Transportation Safety Board determines that the probable cause of this accident was the ingestion of large birds into each engine, which resulted in an almost total loss of thrust in both engines and the subsequent ditching on the Hudson River.
