Book excerpt: The WHO World report on ageing and health is not for the book shelf it is a living breathing testament to all older people who have fought for their voice to be heard at all levels of government across disciplines and sectors. - Mr Bjarne Hastrup President International Federation on Ageing and CEO DaneAge This report outlines a framework for action to foster Healthy Ageing built around the new concept of functional ability. This will require a transformation of health systems away from disease based curative models and towards the provision of older-person-centred and integrated care. It will require the development sometimes from nothing of comprehensive systems of long term care. It will require a coordinated response from many other sectors and multiple levels of government. And it will need to draw on better ways of measuring and monitoring the health and functioning of older populations. These actions are likely to be a sound investment in society's future. A future that gives older people the freedom to live lives that previous generations might never have imagined. The World report on ageing and health responds to these challenges by recommending equally profound changes in the way health policies for ageing populations are formulated and services are provided. As the foundation for its recommendations the report looks at what the latest evidence has to say about the ageing process noting that many common perceptions and assumptions about older people are based on outdated stereotypes. The report's recommendations are anchored in the evidence comprehensive and forward-looking yet eminently practical. Throughout examples of experiences from different countries are used to illustrate how specific problems can be addressed through innovation solutions. Topics explored range from strategies to deliver comprehensive and person-centred services to older populations to policies that enable older people to live in comfort and safety to ways to correct the problems and injustices inherent in current systems for long-term care.

Book excerpt: The Social Security Administration (SSA) administers two programs that provide disability benefits: the Social Security Disability Insurance (SSDI) program and the Supplemental Security Income (SSI) program. SSDI provides disability benefits to people (under the full retirement age) who are no longer able to work because of a disabling medical condition. SSI provides income assistance for disabled, blind, and aged people who have limited income and resources regardless of their prior participation in the labor force. Both programs share a common disability determination process administered by SSA and state agencies as well as a common definition of disability for adults: "the inability to engage in any substantial gainful activity by reason of any medically determinable physical or mental impairment which can be expected to result in death or which has lasted or can be expected to last for a continuous period of not less than 12 months." Disabled workers might receive either SSDI benefits or SSI payments, or both, depending on their recent work history and current income and assets. Disabled workers might also receive benefits from other public programs such as workers' compensation, which insures against work-related illness or injuries occurring on the job, but those other programs have their own definitions and eligibility criteria. Selected Health Conditions and Likelihood of Improvement with Treatment identifies and defines the professionally accepted, standard measurements of outcomes improvement for medical conditions. This report also identifies specific, long-lasting medical conditions for adults in the categories of mental health disorders, cancers, and musculoskeletal disorders. Specifically, these conditions are disabling for a length of time, but typically don't result in permanently disabling limitations; are responsive to treatment; and after a specific length of time of treatment, improve to the point at which the conditions are no longer disabling.

Book excerpt: Every year workers' low-back, hand, and arm problems lead to time away from jobs and reduce the nation's economic productivity. The connection of these problems to workplace activities-from carrying boxes to lifting patients to pounding computer keyboards-is the subject of major disagreements among workers, employers, advocacy groups, and researchers. Musculoskeletal Disorders and the Workplace examines the scientific basis for connecting musculoskeletal disorders with the workplace, considering people, job tasks, and work environments. A multidisciplinary panel draws conclusions about the likelihood of causal links and the effectiveness of various intervention strategies. The panel also offers recommendations for what actions can be considered on the basis of current information and for closing information gaps. This book presents the latest information on the prevalence, incidence, and costs of musculoskeletal disorders and identifies factors that influence injury reporting. It reviews the broad scope of evidence: epidemiological studies of physical and psychosocial variables, basic biology, biomechanics, and physical and behavioral responses to stress. Given the magnitude of the problem-approximately 1 million people miss some work each year-and the current trends in workplace practices, this volume will be a must for advocates for workplace health, policy makers, employers, employees, medical professionals, engineers, lawyers, and labor officials.

This aging population is predicted to place a huge financial burden on healthcare systems around the world. Understanding healthy ageing is a key research priority, along with a better understanding of the pathophysiology of ageing that occurs in a number of age related diseases, such as arthritis. By gaining a better understanding of healthy musculoskeletal ageing we can provide better care and new therapies for common musculoskeletal problems. This Research Topic is intended to bring together basic researchers and clinicians working in the broad area of musculoskeletal ageing. The topic includes mechanisms of healthy ageing in the musculoskeletal system, which we define as skeletal muscle and the synovial joint, particularly constituent structures including articular cartilage, subchondral bone tendon and ligament. A particular focus of this Research Topic is dietary modulation of musculoskeletal ageing.

Book excerpt: Estimated costs associated with lost days and compensation claims related to musculoskeletal disorders-including back pains and repetitive motion injuries-range from $13 billion to $20 billion annually. This is a serious national problem that has spurred considerable debate about the causal links between such disorders and risk factors in the workplace. This book presents a preliminary assessment of what is known about the relationship between musculoskeletal disorders and what may cause them. It includes papers and a workshop summary of findings from orthopedic surgery, public health, occupational medicine, epidemiology, risk analysis, ergonomics, and human factors. Topics covered include the biological responses of tissues to stress, the biomechanics of work stressors, the epidemiology of physical work factors, and the contributions of individual, recreational, and social factors to such disorders. The book also considers the relative success of various workplace interventions for prevention and rehabilitation.

Book excerpt: Some of the most serious consequences of aging are its effects on skeletal muscle. ‘Sarcopenia’, the progressive age-related loss of muscle mass and associated muscle weakness, renders frail elders susceptible to serious injury from sudden falls and fractures and at risk for losing their functional independence. Not surprisingly, sarcopenia is a significant public health problem throughout the developed world. There is an urgent need to better understand the neuromuscular mechanisms underlying age-related muscle wasting and to develop therapeutic strategies that can attenuate, prevent, or ultimately reverse sarcopenia. Significant research and development in academic and research institutions and in pharmaceutical companies is being directed to sarcopenia and to related health issues in order to develop and evaluate novel therapeutics. This book provides the latest information on sarcopenia from leading international researchers studying the cellular and molecular mechanisms underlying age-related changes in skeletal muscle and identifies strategies to combat sarcopenia and related muscle wasting conditions and neuromuscular disorders. The book provides a vital resource for researchers and practitioners alike, with information relevant to gerontologists, geriatricians, sports medicine physicians, physiologists, neuroscientists, cell biologists, endocrinologists, physical therapists, allied health and musculoskeletal practitioners, strength and conditioning specialists, athletic trainers, and students of the medical and biomedical sciences.

Book excerpt: Musculoskeletal disorders (MSDs) are associated with various degrees of disability affecting participation in activities of daily living (ADL) and the overall quality of life (QoL) of affected individuals. MSDs pose a significant burden to health systems, social care systems and caregivers. Such disorders encompass a diverse group of conditions affecting bones, muscles and joints, including degenerative conditions, inflammatory rheumatic diseases, fragility conditions and several types of bodily pain. Common musculoskeletal degenerative conditions in the elderly population include osteoporosis and osteoarthritic changes in the joints and spine. Furthermore, MSDs are common in multimorbidity. Therefore, it is important to utilize the proper diagnostic tools and the adequate therapeutic modalities that are carried out by a multiprofessional team, and if needed in certain conditions, by an interprofessional team as well. This approach is vital for the optimal treatment course and outcomes. Additionally, special consideration should be given to frailty, particularly in elderly persons with multiorgan dysfunction, where the important component is sarcopenia, resulting in a loss of muscle mass and a decline in strength, thereby leading to functional deterioration. In this Special Issue, the current and experimental diagnostic and therapeutic modalities are presented, as well as preventive measures for improving the functioning and overall quality of life for individuals with MSDs.

Available in PDF, EPUB and Kindle. Book excerpt: How can we safely and effectively treat musculoskeletal disorders in this fast-growing population? Musculoskeletal Drug Therapy for Geriatric Patients is an essential reference for pharmacists, physicians, and nurses who plan treatment for elderly patients suffering from post-menopausal osteoporosis, osteoarthritis, rheumatoid arthritis, and gout or hyperuricemia. The medical cost of these common ailments is staggering: osteoporosis alone causes more than 1.5 million fractures each year with annual costs to the health system of approximately $14 billion. The human cost--in pain, disfigurement, restricted movement, and lost work--is incalculable. Today pioneering research is being done, and new drugs have become available to alleviate patients’suffering. Musculoskeletal Drug Therapy for Geriatric Patients offers detailed discussions of the newest drug treatments and recent findings on older medications. For example, many authorities agree that acetominophen should be frontline therapy of degenerative arthritis in the elderly, yet the most common adverse drug reaction of geriatric patients in long-term care is for NSAID gastropathy. This book suggests safer options, including the newest COX-2 inhibitors and disease-modifying anti-rheumatic drugs. Musculoskeletal Drug Therapy for Geriatric Patients offers clear strategies for the assessment and treatment of these painful, debilitating disorders in the elderly. Including tables and charts that comprehensively list test results, Musculoskeletal Drug Therapy for Geriatric Patients provides solid advice on the issues of medical management of these common diseases. This valuable book examines specific issues, including: the newest COX-2 inhibitors and how they work the evaluation, diagnosis, and management of osteoporosis osteoarthritis and its drug therapy a scholarly assessment of geriatric rheumatoid arthritis and its treatment. dosage, contraindications, and possible negative effects of various medications determining the need for chronic prophylactic therapy in patients with gout or hyperuricemia Although none of these musculoskeletal diseases can yet be cured, effective drug therapy can keep them in check. Musculoskeletal Drug Therapy for Geriatric Patients will help you give your patients the most powerful new treatments with the fewest side effects, thereby improving their quality of life.
