This dissertation aims to develop novel acquisition methods and post-processing tools for the robust assessment of intracranial and aortic hemodynamics using PC-MRI and to study their relationship with aging and neurodegeneration. The associations between cerebrovascular health, arterial stiffness, aging, and neurodegeneration are poorly understood. 4D flow MRI is a noninvasive, quantitative imaging technique well-suited for the comprehensive assessment of cerebrovascular anatomy and function. However, its use in large-scale studies and clinical translation requires automation and robustness to accurately visualize and quantify hemodynamic parameters of interest. Currently, no adequate freely-available analysis tools exist and commercial post-processing packages are targeted toward cardiac applications. Here, I develop and validate a method to improve 4D flow MRI streamlines for generating 'virtual injections', enabling visualization of complex blood flow patterns in the brain. In addition, I introduce and validate an automated tool designed for simplified, robust, and reproducible cranial 4D flow MRI analysis, reducing post-processing times and interobserver variability. This tool is subsequently used to establish normative intracranial hemodynamics in one of the largest 4D flow studies to date from 759 older adults. Blood flow rates and pulsatility indices are tabulated in all major intracranial vessels and show a strong age dependency. This tool is also used to assess the relationship between cerebrovascular hemodynamics and white matter microstructure in cognitively unimpaired and Alzheimer's disease subjects. We find that white matter integrity is reduced in Alzheimer's disease and that white matter microstructure correlates with cerebral blood flow and pulsatility in cognitively unimpaired subjects. Furthermore, aortic stiffening, caused by aging or cardiovascular disease, can propagate harmful pulsatility into the cerebral microvasculature. PC-MRI methods have been developed to probe aortic stiffness by measuring pulse wave velocity (PWV), however, temporal resolution and breath-holds are methodological limitations, particularly in older adults. A free-breathing, radial, 2D-PC sequence with advanced image reconstruction is introduced to overcome these limitations and is validated in a custom-manufactured aortic phantom. Obtained PWVs are repeatable and comparable to standard breath-hold sequences. The image acquisition is further advanced by using a simultaneous multislice acquisition and reconstruction methodology to acquire two PC slices simultaneously which reduces scan time, improves physiological consistency, and maintains image quality. Together, these novel post-processing and acquisition techniques provide a means to quantitatively and comprehensively assess both intracranial and aortic vascular health. Using these techniques, a strong relationship between aging, aortic stiffness, cerebral blood flow and pulsatility, and white matter integrity was observed in older adults. These methods and results will help elucidate the pathophysiological contributions of vascular health in the context of aging and neurodegeneration.

Book excerpt: Magnetic resonance imaging (MRI) has shown to be a valuable tool for studying the human brain, allowing in-vivo visualization of structures and anatomy in great detail, especially at Ultra-High field strengths (≥ 7T). MRI is not limited by anatomical and structural information. It can study the brain’s anatomy, functionality, connectivity (functional and structural), and chemical metabolism. Functional MRI (fMRI), for instance, enables the investigation of brain function mechanisms in-vivo with a non-invasive advantage compared to other tools. The present thesis focuses on advanced MRI techniques for ultra-high field strength (≥ 7T), specifically for neuroscience applications. Combined with the higher field strength, these techniques provide better imaging quality and precise brain activity measurement. For example, high-quality anatomical T1 weighted images are essential for several MRI applications, notably, to serve as an anatomical reference in fMRI and gray matter segmentation. Unfortunately, increased field strength also induces non-uniformities in the transmit field (B1+) that can compromise image contrast non-uniformly. One of the goals of the present thesis was to investigate new strategies to overcome this issue. Regarding the functional brain investigation, the gradient-echo (GRE) is the typical method of choice for fMRI applications. Despite its high sensitivity to deoxyhemoglobin variations and widespread availability, the gradient-echo (GRE) BOLD signal is predominantly driven by the large draining vessels resulting in a limited spatial specificity, especially for 7T or higher field strength applications in which the BOLD sensitivity (susceptibility effect) is higher compared to lower static field scanners. In this context, we investigated an alternative fMRI method called vascular space occupancy (VASO) that promises higher spatial specificity than the typical GRE BOLD. To achieve the aim of this thesis, we used four approaches (chapters 2-5).

Book excerpt: This handbook describes methods for processing and analyzing functional connectivity Magnetic Resonance Imaging (fcMRI) data using the CONN toolbox, a popular freely-available functional connectivity analysis software. Content description [excerpt from introduction] The first section (fMRI minimal preprocessing pipeline) describes standard and advanced preprocessing steps in fcMRI. These steps are aimed at correcting or minimizing the influence of well-known factors affecting the quality of functional and anatomical MRI data, including effects arising from subject motion within the scanner, temporal and spatial image distortions due to the sequential nature of the scanning acquisition protocol, and inhomogeneities in the scanner magnetic field, as well as anatomical differences among subjects. Even after these conventional preprocessing steps, the measured blood-oxygen-level-dependent (BOLD) signal often still contains a considerable amount of noise from a combination of physiological effects, outliers, and residual subject-motion factors. If unaccounted for, these factors would introduce very strong and noticeable biases in all functional connectivity measures. The second section (fMRI denoising pipeline) describes standard and advanced denoising procedures in CONN that are used to characterize and remove the effect of these residual non-neural noise sources. Functional connectivity Magnetic Resonance Imaging studies attempt to quantify the level of functional integration across different brain areas. The third section (functional connectivity measures) describes a representative set of functional connectivity measures available in CONN, each focusing on different indicators of functional integration, including seed-based connectivity measures, ROI-to-ROI measures, graph theoretical approaches, network-based measures, and dynamic connectivity measures. Second-level analyses allow researchers to make inferences about properties of groups or populations, by generalizing from the observations of only a subset of subjects in a study. The fourth section (General Linear Model) describes the mathematics behind the General Linear Model (GLM), the approach used in CONN for all second-level analyses of functional connectivity measures. The description includes GLM model definition, parameter estimation, and hypothesis testing framework, as well as several practical examples and general guidelines aimed at helping researchers use this method to answer their specific research questions. The last section (cluster-level inferences) details several approaches implemented in CONN that allow researchers to make meaningful inferences from their second-level analysis results while providing appropriate family-wise error control (FWEC), whether in the context of voxel-based measures, such as when studying properties of seed-based maps across multiple subjects, or in the context of ROI-to-ROI measures, such as when studying properties of ROI-to-ROI connectivity matrices across multiple subjects.
