Available in PDF, EPUB and Kindle. Book excerpt: Rheumatology is concerned with the diagnosis and therapy of rheumatic diseases. These diseases can be classified into degenerative arthropathies, systemic conditions and connective tissue diseases, inflammatory arthropathies and soft tissue rheumatism. Rheumatoid arthritis, septic arthritis, spondyloarthropathies, etc. are categorized as inflammatory arthropathies. Tennis elbow, low back pain and Golfer's elbow fall within soft tissue rheumatism. The incorporation of biologics such as inhibitors of JAK-STAT signaling pathway, TNF-alpha and certain interleukins are one of the most important developments in this field. Autoimmune diseases are a principal cause of many rheumatic disorders. Therefore, much of research in rheumatology is focused in this direction. Clinical trials of monoclonal antibody therapies for the treatment of arthritic disorders are another emerging focus in rheumatology. This book elucidates the concepts and innovative models around prospective developments with respect to rheumatology. It presents researches and studies performed by experts across the globe. It is meant for students who are looking for an elaborate reference text on rheumatology.

Available in PDF, EPUB and Kindle. Book excerpt: Many advances have occurred in the last 5 years especially in understanding genetic susceptibility factors, the role of immune response in chronic arthritis and the relationship between cellular processes that underlie inflammation and tissue damage. All these are dealt with in depth in the individual contributions. Genetics studies included illustrate how studies of HLA genes and genes controlling the autoantibody repertoire may be involved. Environmental factors are believed to be important and studies both in humans and in experimental models on the role of viral and bacterial agents are covered. The fascinating relationship between microbes, dysregulated immune responses and oncogenes are also examined. Since autoimmunity is the key aspect of the immunological response in rheumatoid arthritis, the role of antigens is re-examined as is the part played by immune T and B cells. New areas of clinical application of basic science is well presented in this book, forexample, in examining how markers of cartilage destruction may become important in monitoring disease activity. A chapter reviewing possible future novel therapies is included.

Book excerpt: At present we may be at the cross-roads in the therapeutic approaches we have for the treatment of the 100 or more rheumatic conditions. This is be cause we now recognise that although some advances have been made with the development of a large range of non-steroidal and steroidal drugs during the past two decades or so, we now recognise that many, if not all, of these have rather limited effects on many of the disease processes which underlie the manifestations of the various rheumatic states. Advances in molecular bi- 010gy in the past 5-10 years have enabled these tools to be applied extensive ly for developing further our understanding of the rheumatic disease processes. In some cases these molecular tools (e. g. ,),-interferon, interleukin- 2, T-cell antibodies) have been directly employed as therapies themselves. While the outcome from trials with such agents in rheumatoid arthritis in particular has not been as would have been hoped, these results as with cy closporin A and low-dose methotrexate in the therapy of rheumatoid arth ritis have given us important indications for the approach employing what are generally described as "immunomodulators" to control this disease. But this may not be the same type of approach which is desirable for all types of rheumatic conditions. Indeed, even the way which the present range of drugs and other therapies are applied may not be the most effective and safe means of treating different types of arthritic conditions.

Book excerpt: Commemorative publication pays tributeto the American College of Rheumatologyand its membership Commemorating the remarkable achievements in rheumatology since the founding of the American College of Rheumatology (ACR), this unprecedented collection of essays records the history of rheumatology during this important era and pays tribute to its members upon the ACR's 75th anniversary. From developments in therapy to clinical trials to the revolution in genetics, this book describes important advances that have led to our current body of scientific knowledge and established treatment paradigms that have improved outcomes for patients with rheumatic diseases. The contributors represent leaders in the field, including ACR Masters, past ACR presidents, and other distinguished clinicians, researchers, and educators. Covering all aspects of rheumatology, the essays review such topics as: * History of rheumatology * Advances in treatment * The revolution in modern science * Practice of rheumatology: economics, organization, and structure * Patient outcomes * Funding of research * Role of ACR in the political process All the essays are written from a personal perspective, offering diverse andengaging points of view. In addition to exploring the impact of advances in clinical and basic research as well as in practice, the authors offer their forecasts for the future, including innovations in the treatment of rheumatic disease and the organization of healthcare. For seventy-five years, the ACR has been dedicated to advancing the missionof rheumatology in improving the lives of patients with rheumatic disease, promoting innovative clinical and basic research, and advancing the training of physicians and other healthcare professionals. The members of the ACR include physicians, research scientists, nurses, physical and occupational therapists, psychologists, and social workers among other disciplines. This anniversarycollecti

Book excerpt: This new companion to Hochberg et al.'s Rheumatology masterwork focuses on the momentous recent advances in our understanding of the genetics and immunology of psoriatic and reactive arthritis, and their implications for diagnosis and management. Leading international authorities explore new concepts in genetic and pathogenic mechanisms and early diagnosis; provide comprehensive, well-illustrated coverage of clinical features; evaluate the very latest therapeutic options, including biologics; and discuss clinical outcome measures. Examines the very latest pathogenic mechanisms and diagnostic criteria, providing the necessary tools for early diagnosis and monitoring of disease progression. Offers comprehensive coverage of the clinical manifestations of psoriatic and reactive arthritis, including a 'mini atlas' of color plates.Examines the pros and cons of the available therapeutic options, including the very latest biological as well as traditional therapies.Includes practical discussions of clinical outcome measures, delivering ongoing tools for assessment of function, disease activity, and quality of life.
