Desde o início dos anos 2000, 46 milhões de pessoas nos países estudados recebem cobertura de programas de saúde com direitos explícitos a atendimento. Essas reformas foram acompanhadas por um aumento do gasto público com saúde, financiado em grande medida pelas receitas gerais que priorizam ou visam explicitamente a população que não tem capacidade de pagamento. O compromisso político em geral se traduziu em orçamentos maiores e na aprovação de legislação que insula recursos para a saúde. Muitos países priorizaram o atendimento de saúde primária com boa relação de custo-benefício e adotaram métodos de pagamento que incentivam a eficiência e a prestação de contas por resultados, proporcionando à s autoridades do setor de saúde mais poder para induzir os prestadores a cumprirem as prioridades de saúde pública. Apesar do progresso, ainda persistem disparidades no financiamento e na prestação de serviços de qualidade entre os subsistemas de saúde. Cumprir o compromisso da cobertura universal de saúde exigirá esforços concertados para melhorar a geração de receitas de formal fiscalmente sustentável e aumentar a produtividade em termos de despesas. Em Rumo a uma Cobertura Universal de Saúde e Equidade na América Latina e Caribe: Evidência de Países Selecionados, os autores mostram que a evidência da análise de 54 pesquisas domiciliares corrobora que os investimentos para ampliar a cobertura de saúde estão gerando resultados. Embora os pobres ainda apresentem resultados de saúde piores que os riscos, as disparidades diminuíram consideravelmente — em particular nos estágios iniciais da vida. Os países alcançaram altos níveis de cobertura e equidade na utilização de serviços de saúde materna e infantil. O quadro tem mais nuanças e não é tão positivo quando se trata da situação de saúde dos adultos e da prevalência de doenças e condições crônicas de saúde. A cobertura para doenças não transmissíveis não é alta e continua a pender mais para os mais abastados. A prevalência de doenças não transmissíveis não ocorreu como esperado, tendo em vista a queda na mortalidade; o melhor acesso a serviços de diagnósticos entre as pessoas mais riscas pode estar mascarando mudanças na prevalência efetiva. Gastos catastróficos com saúde caíram na maior parte dos países. No entanto, o quadro com respeito à equidade é mais misto e indica limitações na medição. Embora a taxa de empobrecimento em razão de gastos com saúde seja baixa ou em declínio, 2 a 4 milhões de pessoas nos países estudados ainda ficam abaixo da linha da pobreza em razões de gastos com saúde. Esforços de monitoramento sistemático da qualidade do atendimento de saúde na região são ainda incipientes. No entanto, a análise da literatura revela deficiências importantes na qualidade da saúde, bem como diferenças substanciais entre os subsistemas. Melhorar a qualidade do atendimento e garantir a sustentabilidade dos investimentos em saúde continuam a ser itens pendentes da agenda.

Book excerpt: Over the last decade the question of the relationship between organisations and society has been subject to much debate, often of a critical nature. The decade has seen protests concerning the actions of organisations, exposures of corporate exploitation and unfolding accounting scandals. At the same time ethical behaviour and a concern for the environment have been shown to have a positive correlation with corporate performance. The nature of corporate social responsibility is therefore a topical one for businesses and academics. There are however many different perspectives upon what is meant by corporate social responsibility and how this might be applied within organisations. This book explores some of these different perspectives based upon the experiences of different people in different parts of the world. There has been much written about globalisation – some of it positive and much of it negative. It is a subject which arouses definite opinions. Despite the fact that the word globalisation is part of the title of this book it is not our intention to contribute to this debate. Instead we use the word globalisation in its original sense to represent the ubiquity of the concern for Corporate Social Responsibility (CSR) which is the subject matter of this book. Specifically we are concerned with the social contract between an organisation and its stakeholders. It is apparent that any actions which an organisation undertakes will have an effect not just upon itself but also upon the external environment within which that organisation resides. In considering the effect of the organisation upon its external environment it must be recognised that this environment includes both the business environment in which the firm is operating, the local societal environment in which the organisation is located and the wider global environment. Effectively therefore there is a social contract between organizations and their stakeholders. Recognition of the rights of all stakeholders and the duty of a business to be accountable in this wider context therefore has been largely a relatively recent phenomenon. The economic view of accountability only to owners has only recently been subject to debate to any considerable extent. In the current environment there is a need to debate this issue and its implications. This book therefore recognises the international scope of the interest in corporate social responsibility both through the contributions made by the authors of the respective chapters, who come from various parts of the world, and also through the international importance of the perspectives offered by these contributors. In doing so the various authors demonstrate that corporations are a part of society just as much as each of us is as a individual. Furthermore they demonstrate that the issues and concerns are not local ones but are international is scope and concern us all. The contributions to this book provide a representation of the range of concern for this relationship and the range of topics which fall within the subject matter of CSR. Among the authors who have contributed to this book are representatives from every continent and from a wide range of disciplines. The topics which are considered in the various chapters are equally diverse.

Book excerpt: In recent decades there has been an exponential increase in large hydroelectric plants in Brazil, especially in the Amazon region. These large hydraulic structures impact the environment and the lives of people living in the places where they settle and require a special type of water governance. The dictatorial regime (1964-1985) created a "standard" for the construction of these great structures, through an institutional and legal framework, which benefited the Brazilian business elite but also, through the creation of a popular imagination, which shows itself lasting progress on the country's progress and development. The suspension of security, the fragility of institutional environmental structures, the disrespect for indigenous reserves, the lack of clarity about the concept of "affected population" and the non-payment of fair compensation were identified as one of the main challenges for a democratic water governance in the country. In the late 1970s, the Dam-Affected Movement (MAB) began its organization and is also studied in this research. The study is an important and insightful academic contribution to the understanding of the main bottlenecks of effective water governance in Brazil.
