Book excerpt: Objective: The aim of this study was to conduct a randomized in vivo clinical trial to assess the efficacy of a one time application of a phoactivated dye, methylene blue (PerioWaveTM) in treating chronic periodontal disease in humans. Material and Methods: Thirty five periodontally maintained subjects with moderate to severe chronic periodontitis were randomly divided into a control group with nonsurgical periodontal debridement (NSPD) only versus an experimental group with NSPD and antimicrobial photodynamic therapy (APT). All teeth in both groups received periodontal treatment comprising of scaling, root planing, ultrasonics and local anaesthesia. The experimental group had the qualifying sites (pockets depths greater than 4.4 mm and bleeding on probing (BOP)) treated with the PerioWaveTM system and compared to the control group. The primary endpoint was clinical attachment level (CAL) and secondary endpoints were pocket depth (PD) and BOP. The endpoints and plaque were evaluated at baseline, 6 and 12 weeks by a blinded examiner. Results: This study showed PerioWaveTM as a one time application of a cold diode laser with a wavelength of 670nm; a maximum power of 150mw; an average energy density of 20 J/cm2; a phenothiazine photosensitizer (methylene blue) in a concentration of 0.01% wt/vol; and at an optimal activation time of 60 seconds per mesiodistal surface had no statistical significance in the treated qualifying sites with regards to CAL (p=.69; p=.97) or PD (p=.14; p=.23) at 6 or 12 weeks after treatment over the NSPD group. However BOP did have a statistical significance over the control group at 6 weeks (p=.05) but not at 12 weeks (p=.47). Conclusions: In patients with advanced chronic periodontitis, CAL and PD did not show improvement with one time application of the PerioWaveTM photodynamic system over conventional NSPD. However, the lack of BOP did have a short time (6 weeks) reduction.

Abstract: Abstract Abstract The aim of this study was to evaluate if there is any adjunctive effect of hormone replacement therapy (HRT) on non-surgical periodontal therapy in postmenopausal women with moderate to severe chronic periodontitis. The study was carried out as a single-blinded longitudinal study lasting for 12 months. Postmenopausal women with or without any HRT (assigned to test and control groups, respectively) and having at least 4 sites with probing depth >=6 mm were recruited. A total of 30 patients, 15 in the test group (mean age = 51.0 years) and 15 in the control group (mean age = 54.1 years) participated in the study. The mean duration of menopause was 3.9 years for test subjects and 3.0 years in controls. However, only 6-month data were available for 27 of the 30 patients due to time constrains. Therefore, 3-month data from all 30 subjects and 6-month data from 27 subjects were analyzed and presented. Clinical parameters, including presence of supragingival plaque (PI%), bleeding on probing (BOP%), probing pocket depth (PPD) and probing attachment level (PAL) were recorded at baseline, 3-month and 6-month after non-surgical periodontal treatment. Standardized intra-oral radiographs in selected sites for evaluation of alveolar bone changes by digital subtraction radiography were also taken at the same chosen time intervals. After initial examination, oral hygiene instructions were given and full-mouth supra- and sub-gingival debridement were performed. After 6 months, full-mouth Pl% decreased from 75% at baseline to 22%, while the BOP% decreased from 67% at baseline to 26% at 6-month for the test group. The corresponding percentage changes of Pl% for the control group were from 75% to 18%, and 74% to 20% for Abstract BOP%, respectively. Throughout the study, the reduction of PPD, gain in PAL and increase in REC at different time points were found to be statistically significant (pThe findings from the present study suggested that there was no adjunctive clinical benefit of HRT in postmenopausal women when combined with non-surgical periodontal therapy during the treatment of moderate to severe chronic periodontitis. Scaling and root planing has been shown to be an effective treatment for chronic periodontitis. Although mechanical treatment significantly decreases the prevalence and levels of subgingival microorganisms, it does not necessarily eliminate all pathogens.Periodontal pocket provides a natural reservoir bathed by gingival crevicular fluid that is accessible for the insertion of the delivery device. Local delivery systems, when retained in the periodontal pocket, can release the antimicrobial agents at levels that can be delivered by systemic antibiotics with minimal side effects.
