As joint attention plays a key role in the development of social communicative skills such as language and theory of mind, its deficit can have a great impact on the life of children diagnosed with autism. Although many interventions have been developed to address this deficit in fields such as psycho-education and education, little is available in the field of music therapy. Moreover, of those interventions developed in music therapy, few involved external settings, such as homes and schools. This study used an intervention research methodology to provide appropriate guidelines for intervention with children with autism to address joint attention deficits. These guidelines include such elements as goals and objectives, including addressing joint attention responses and initiations, treatment duration, setting, intervention agent, specificities of sensory interventions with children with autism, and pertinent musical components. Results indicate that the inclusion of siblings and the use of improvisational music therapy could favor positive results. Potential implications for music therapy practice as well as areas for future research are discussed.

Book excerpt: Early intervention is a critical component of efforts to optimize outcomes for children with autism spectrum disorders (ASD) and their families. One promising target for early intervention is joint attention, an early developing social-cognitive competency that is foundational to many other social, communicative, and cognitive skills; and a core deficit in children with ASD. While joint attention interventions are gaining interest among researchers, many are limited by their failure to situate joint attention development within the caregiver-child relationship and to adequately examine child and caregiver outcomes and transactional processes. The purpose of this study was to examine changes in child and caregiver joint attention actions and transactions across the course of a parent-mediated joint attention intervention. The Child-Caregiver Joint Attention coding system was developed and applied to videotaped caregiver-child interaction sessions from all phases of the Joint Attention Mediated Learning intervention. Participants included five mothers and their toddler aged sons. Joint attention actions examined included gaze alternations, pointing, showing, joint attention responding, and joint attention initiating for both children and caregivers. Four of five children demonstrated increases in gaze alternations, joint attention responding, and joint attention initiating by the end of the intervention. Three caregivers demonstrated increases in gaze alternations and joint attention responding, and four displayed increases in joint attention initiating. There was no clear pattern of change across children or caregivers in pointing or showing. All participants, with the exception of one caregiver, responded to a higher percentage of opportunities for joint attention in the final intervention phase than in Baseline, suggesting that most participants became more responsive to their social partners by the end of the intervention. The findings of this study suggest that parent-mediated joint attention interventions have the potential to promote changes in both child and caregiver joint attention actions and transactional relationships. Future research should continue to examine outcomes for both children and primary caregivers and changes in child-caregiver transactions over the course of different types of joint attention interventions in order to inform intervention development and selection, and explore mechanisms for change.

Book excerpt: Joint attention, the ability to coordinate one's attention with that of another person (Dawson et al., 2004), and imitation, the ability to copy another person's behavior (Sevlever & Gillis, 2010), are two of the initial methods by which children learn from and interact with the world around them (Trevarthan, 1979). These two skills are related to the development of language, social skills, and play. Further, they seem to come naturally in typically developing children. For children with autism spectrum disorder (ASD), however, these skills are often delayed or entirely absent, thereby potentially leading to significant impediments in the acquisition of crucial functional skills (Dawson et al., 2004). Social orienting theory posits that children with ASD exhibit such deficits in joint attention and imitation because of their lack of attention to social stimuli and, as a result, the decreased attempts by others in their environment to engage them (Dawson et al., 2004). Current estimates report that the prevalence of autism spectrum disorder (ASD) averages one in every sixty-eight children (CDC, 2014). For children with ASD, research has repeatedly emphasized the importance of both early identification and early intervention (Koegel, Koegel, Ashbaugh, & Bradshaw, 2014; National Research Council, 2001). One early intervention model that is based on social orienting theory is the Early Start Denver Model (ESDM), which considers dyadic interactions between the therapist and the child as a prime avenue for learning not only basic social skills such as joint attention and imitation but also more complex social skills, language, and play (Rogers & Dawson, 2010).Data from 23 children participating in an ESDM program were examined in the current study; specifically, the researcher assessed whether children demonstrated significant skill growth over the first six months of intervention. Further, the researcher evaluated the relationship between joint attention and imitation as well as how these core areas correlated with additional skills, including language, social skills, and play. Finally, the researcher investigated whether level of joint attention and imitation at baseline predicted overall responsiveness to the intervention. Results showed that all children in the study made significant progress in all areas in the first six months of intervention. The hypothesized positive relationship between joint attention and imitation was supported; however, results did not indicate a trajectory wherein joint attention was acquired prior to imitation. Results supported the relationship between both joint attention and imitation skills and subsequent language, social, and play skills. Finally, baseline skill levels did not significantly predict overall performance.
