Book excerpt: The design and fabrication of implant-supported prosthetic structures requires the faithful reproduction of anatomical details and the correct positioning of the implants in the jaw. To do this, it is essential to take impressions using silicones or polyethers that will provide a working model that is as precise and exact as possible and will lead to a prosthetic structure with adequate passive fit.In recent years, the introduction of digital workflow has brought about a revolution in the field of dentistry, particularly in relation to the fabrication of prosthetic structures, whether tooth- or implant-supported. Digital techniques have allowed greater management of technical errors in metal-casting, making it easier to achieve a correct prosthetic fit. This study aimed to compare two impression techniques for analogue implants: the direct (or pick-up) technique with the elastomeric material polyether, and a digital scanner (True Definitionu00ae, 3Mu00ae ESPE: USA), in order to determine which is the more accurate technique. Twenty impressions were taken of a master model with each technique. Were measured the distances between contiguous analogues, intermittently positioned analogues and the most distal implants. Only significant differences were found between both techniques and the master model for the distance between distal analogues. After analizing the results of this study, for rehabilitations involving more than four implants, neither technique can be considered accurate although error falls within the tolerance limits established in the literature (30-150u00b5m).

Book excerpt: BackgroundThe accuracy and final perfect stress free fit of the final implant restoration is dependent on various factors. But one of the most important is the creation of accurate model. The contemporary approach uses both digital and material impression taking procedures. And still there are a lot of unbeneficial factors that can alter the accuracy of final prosthesisAim/HypothesisThe purpose of the study is to evaluate the influence accuracy of three different impression taking methods for bone level implants compared to intraoral scanningMaterials and Methods38 implants were scanned using scan flags and a digital model was created using the TRIOS intraoral scanner. All the patients were divided into two groups. For group one of 20 implants were transferred using open transfers and an open stock tray impression was taken with double layer double phase C-silicon. For the other 18 implants a double layer single phase additive silicon impression was taken. For both groups was created additional 3D printed custom tray and an impression was taken with polyether. All impressions were cast with dental plaster fourth grade and were scanned with scan flags consistent with the implant system. A digital comparison was made for each group compared to the digital model, acquired from the intraoral scanning. Results Showed some significant discrepancy in the group with the wash technique and the condensational silicon, less discrepancies were found in the group with additive silicon, and almost no significant discrepancies were found in none of the implants transferred with 3D printed custom tray. Conclusion Using a 3D printed custom tray impression taking approach could be a successful alternative to intraoral scanning.

Book excerpt: Several factors play a role in stability of impressions made from elastomeric impression materials. These include; polymerization shrinkage, loss of by-products during condensation, thermal contraction from oral temperature to room temperature, imbibition when exposed to water, disinfectant or high humidity and incomplete recovery from deformation due to viscoelastic behavior. An ideal impression material should be dimensionally stable over time to allow for pour at the convenience of the operator. Several studies evaluated the dimensional accuracy of elastomeric impression materials based on various factors including effects of repeat pour, temperature, humidity, disinfectants, impression techniques, and filler loading amongst others. Most of the previous studies did not use the standardized method described by the ADA specification for elastomeric impression materials ...

Book excerpt: This new book focuses on dental implants used in conjunction with other prosthetic devices in the general dentist's office, designed to help the partially or completely edentulous patient recover normal function, esthetics, comfort, and speech. Step-by-step procedures guide practitioners through challenging clinical situations and assist them in refining their technique. The information in this practical, highly illustrated book reflects the latest in continued research, diagnostic tools, treatment planning, implant designs, materials, and techniques. Prosthetic devices covered in this include complete dentures, bridges, overdentures, and various dental implant systems. A comprehensive chapter covering immediate load implants teaches dentists how to provide an edentulous patient with implants the same day surgery is performed. A thorough discussion of preimplant prosthodontic considerations takes the practitioner through the vital assessment steps necessary to plan treatment. Considerations for assessing the restorability of teeth adjacent to potential implant sites include abutment size, crown-root ratio, endodontic status, root configuration, tooth position, parallelism, root surface area, caries, and periodontal status. Fixed treatment planning options for the completely edentulous mandibular arches expands treatment options available to dentists, helping them to treat more patients. Material thoroughly explores the three dimensional concept of available bone and the implant treatment options for each type of bone anatomy, which enables practitioners to treat patients at any stage of edentulism. Comparisons of the periodontal indices for a natural tooth and an osteointegrated implant alert clinicians to fundamental differences in the support system. Basic biomechanics are discussed, demonstrating how these principles also relate to the scientific rationale for contemporary and future dental implant designs. A comprehensive discussion of bone density in an edentulous site explains this determining factor in treatment planning, implant design, surgical approach, healing time, and initial progressive bone loading during prosthetic reconstruction.

Book excerpt: Introduction: Studies demonstrated that the accuracy of intra-oral scanners can be compared with conventional impressions for most indications. However, little is known about their applicability to take impressions of multiple implants. Objective: compare the accuracy between digital impressions and different conventional printing techniques in implants in order to observe differences of the novel digital impression with the conventional techniques.Material and Methods: A stone cast of an edentulous mandible with 4 implants was fabricated to serve as master cast (control), two master models were made: one with parallel implants (Model 1)and another with uneven implants(Model 2). Model 1: 4 types of impressions were taken(Group A: tray impressions with reposition abutments; Group B: Open tray impression groups for dragging copings, without splinting; Group C: Open tray impressions for ferrules; Group D: Using 3M u2122 True Definition Scanner system).Model 2: 3 types of impressions were taken(Group E: tray impressions with reposition abutments; Group F: Open tray impression groups for dragging copings, without splinting; Group G: Using 3M u2122 True Definition Scanner system).Master casts and conventional impression casts were digitized with na extraoral high-resolution scanner. Standard tessellation language (STL)data sets from the seven groups of digital and conventional impressions were superimposed with the STL data set from the master cast to assess the 3D (global)deviations. To compare the master cast with digital and conventional impressions. Results: Statistical analysis was performed using the IBMu00ae SPSSu00ae Statistics Vs. 24, NY, USA.The normality of the distributions was analysed with an Kolmogorov-Smirnov test. The median comparison was performed using The differences between the median were analyzed using non-parametric Krustal-Wallis and Mann-Whitney tests with a significance level of p

Book excerpt: Production of exemplary crowns and fixed partial dentures depends upon the usage of impression materials that accurately reproduces prepared teeth and their relationship to adjacent oral structures. A leading dental manufacturer has introduced an improved elastomeric impression material, but the accuracy of the new material after repeat pours have not been verified. The present study was done to comparatively evaluate dimensional accuracy of vinyl polyether silicone and vinyl polysiloxane elastomeric impression materials after repeated pours at different time intervals. It Was concluded that, because of excellent dimensional stability, both impression materials can be repeatedly poured for two weeks as there is no significant dimensional change overall.

This book was released on 2017 with total page pages. Available in PDF, EPUB and Kindle. Book excerpt: Background After successful osseointegration and reentry procedure, the decision to perform an implant impression or not is based on clinical and radiographical assessment methods. Single radiographs focusing on the platforms of the implant and the related impression post are commonly used to confirm a correct fit between the components. However, information and knowledge on the ability to detect the presence or absence of a gap at this interface are lacking. Aim/Hypothesis To investigate the theoretically estimated radiographical gap detectability and the gap acceptability at the implant-abutment interface before impression taking. It was hypothesized that no difference would be observed between the two theoretically estimated gap values (detectable vs acceptable).Material and Methods A survey was performed at the Dental School of the University of Bern. Thirty prosthodontists were included and interviewed concerning their subjective theoretical estimations of (A) the radiographical gap detectability between an implant and the related impression post, and (B) the maximally acceptable gap size at this interface aiming to perform the implant impression. The estimated gap values for the detectability and the acceptability were analyzed with descriptive methods and comparisons performed using the nonparametric Mann-Whitney U-test. Results The gap size theoretically detectable at the implant-abutment interface was estimated significantly higher on radiographs than the gap size that would be clinically accepted by the dentist for the implant impression procedure (p=0.046). The estimated mean values u00b1 standard deviations were 155u00b5m u00b1 199u00b5m for A (radiographical gap detectability) and 65u00b5m u00b1 43u00b5m for B (gap acceptability at the implant-abutment interface). The estimations ranged between 10 u2013 1000u00b5m and 2 u2013 200u00b5m for A and B, with median values of 100u00b5m and 50 u00b5m, respectively. Conclusion and Clinical implications (500/500)The theoretically detectable misfit at the interface was estimated 2x higher on radiographs than the dentists would accept to perform the implant impression. In conclusion, radiographical assessment cannot be considered specific enough in relation to the acceptable gap estimation. Thus, additional and more accurate assessment methods are required to fulfill the estimated needs for implant impression decision making. A practical study is needed to confirm the results from this theoretical survey.
