Book excerpt: The design and fabrication of implant-supported prosthetic structures requires the faithful reproduction of anatomical details and the correct positioning of the implants in the jaw. To do this, it is essential to take impressions using silicones or polyethers that will provide a working model that is as precise and exact as possible and will lead to a prosthetic structure with adequate passive fit.In recent years, the introduction of digital workflow has brought about a revolution in the field of dentistry, particularly in relation to the fabrication of prosthetic structures, whether tooth- or implant-supported. Digital techniques have allowed greater management of technical errors in metal-casting, making it easier to achieve a correct prosthetic fit. This study aimed to compare two impression techniques for analogue implants: the direct (or pick-up) technique with the elastomeric material polyether, and a digital scanner (True Definitionu00ae, 3Mu00ae ESPE: USA), in order to determine which is the more accurate technique. Twenty impressions were taken of a master model with each technique. Were measured the distances between contiguous analogues, intermittently positioned analogues and the most distal implants. Only significant differences were found between both techniques and the master model for the distance between distal analogues. After analizing the results of this study, for rehabilitations involving more than four implants, neither technique can be considered accurate although error falls within the tolerance limits established in the literature (30-150u00b5m).

With the ever-increasing number of dentists involved in implant dentistry, it is essential to ensure that their treatment methods follow the highest standard. The ITI Treatment Guide series, a compendium of evidence-based implant-therapy techniques in daily practice, is written by renowned clinicians and provides a comprehensive overview of various therapeutic options. Using an illustrated step-by-step approach, the ITI Treatment Guide shows practitioners how to manage different clinical situations, with the emphasis on sound diagnostics, evidence-based treatment concepts, and predictable treatment outcomes. The second volume of the ITI Treatment Guide is devoted to the restoration of partially dentate patients. Central to this volume of the ITI Treatment Guide are loading protocols available to the clinician and the patient and how they relate to various treatment indications, including both single and multiple missing teeth in the posterior and anterior regions of the mouth. Among potential topics for upcoming volumes are implant placement in extraction sockets, loading protocols in edentulous patients, implant therapy in the esthetic zone in extended edentulous spaces, and many more.

Book excerpt: BACKGROUND AND AIMThe clinical fit of an implant prosthesis at the implant-abutment junction is directly dependent on the accuracy of impression and cast. Digital workflow for producing implant-supported restorations involves the usage of intraoral scanners (IOS). From IOS data 3D printed master model is often fabricated using the selected type of digital analogs. There is a lack of data regarding the effects of IOS, 3D printer, and digital analog type effect on the local and global accuracy of digital analog positions in 3D printed master model. Moreover, errors arising in each step/stage should be identified. The aim of the study was to estimate the effect of IOS, two 3D printers and 2 digital analog systems on 3D positions of the digital analogs comparing reference, IOS and 3D printed model data sets. The null hypothesis was that there are no statistically significant differences between the data sets.MATERIALS AND METHODSTwo Straumann BLT 4.1 mm RC implants were inserted in the reference model (REF) left quadrant, in the location of second premolar and second molar with 5u00b0 angulation. Three calibration spheres of 5 mm in diameter (u00b11 u00b5m) where placed on the left quadrant at the model base. Scan bodies (3Shape) were attached to the implants and model was scanned with Nikon Altera 10.7.6. industrial scanner (REF-stl). REF model was scanned 10 times with E3 (3Shape) scanner for validation. Ten digital impressions were taken with 3Shape Trios3 intraoral scanner. The closes to the overall average or IOS impressions STL file (IOS-stl) was selected for 3D printing. Asiga MAX and Next Dent 5100 3D printers, ELOS Print Model Analog and NT-trading DIM-ANALOG were used to produce 4 groups (n=10 each) of 3D printed implant models. Later they were scanned with a validated E3 scanner (3D-print-stl). Distance, angulation, rotation, vertical shift measurements were performed using Geomagic Control X 2018 software.CONCLUSIONSAsiga MAX 3D printer performed more accurately than NextDent 5100. ELOS Print Model Analog showed most accurate results both locally and globally than NT trading. Intraoral scanning had significant influence in overall error propagation. Further studies are needed to evaluate other factors.

Book excerpt: TitleDigital intraoral scanners vs traditional impressions: Fitting accuracy of screwed mesostructuresBackgroundPassive fit and marginal precision of prosthetic structures on osteointegrated implants are a requirement to minimize tensions and to avoid loss of marginal bone and finally lack of osteointegration. The impression process is a primary step to obtain sufficient accuracy. We use daily intraoral scanners for impressions to made implant restorations from several years and we appreciate the clinical outcomes for passive fitting (cemented sovrastructures) and precision of screwed ones.AimThe aim of this job is to investigate the marginal accuracy of custom CoCr milled mesostructures on implants acquiring impressions by intraoral digital scanners and compare them to ones obtained bytraditional impressions systems and to evaluate if the misfit is within the optimal measure of 25-50u00b5m.Material and methodsTwo pair of implants were rigidly fixed in maxilla resin models. The first two implant were internal hexagonal flat connection implants made by Winsix implant system ( Biosaf In srl). The second two were internal octagonal conical connection implants made by Geass (Geass srl). Three impressions were took of each model. The first one was a traditional impression made by polyether material with the single impression technique. The other two impression was acquired using CS3500 Carestream Intraoral Scanner and Trios3 intraoral scanner. The digital impressions were made following factory recommendations for implant cases workflow.Milled CoCr screwed mesostructures were commissioned on the three impressions of each model. The aim was to analyze the misfit of the three mesostructures obtained using Sheffield test. The misfit was evaluated by a radiograph taken perpendicularly to the long axis of the implants. ImageJu00ae software was used to digitally process the radiographic images and perform the measurements. ResultsWe eliminate, using an artificial model some intraoral variables as blood, saliva, patient movements but we simulate the natural architecture of mouth maintaining crestal anatomy, gum, implant distance. We investigate the misfit radiographically performing the Scheffield test: in this way we screwed firmly only one screw of the mesostructure and we observe the matching of the other implant-structure connection. The bigger misfit was appreciate on the two mesostructures obtained by traditional impressions, one of them has failed to comply with the established gap limit of 50 u00b5m.Very good fitting, within the limit of 50 u00b5m was found on three mesostructures: The one made by impression with Carestream intraoral scanner CS3500 on winsix implantsand the two performed by Trios3 intraoral scanner 3Shapedental on Winsix implants and Geass Way implantsThe fitting accuracy of the last mesostructure made by impression performed on Geass Way implants with CS3500 scanner looks a middle way.ConclusionsThe misfit of all the mesostructures obtained by digital intraoral scanners seems to be in the safety range, avoiding structural tensions involving implant after screwing. This study confirm the fitting accuracy of milled mesostructures obtainable by digital intraoral impressions. We expected a more homogeneous results in the group of mesostructures obtained by digital impressions, poor number of mesostructures investigated may be the reason of the discrepancy.More data are indispensable to complete the results obtained and to investigate the real accuracy obtainable .

This book was released on 2017 with total page pages. Available in PDF, EPUB and Kindle. Book excerpt: Background: Digital three-dimensional planning of dental implant treatment has proven its benefits to the implantation procedure. Hereby, 3D datasets of CBCT scans are able to be superimposed to optically obtained data of the oral situation in order to reproduce and consider the soft tissue situation during the planning process. The quality of this procedure is depending to the used scan datasets and their immanent failure. 416Aim: This study aimed to compare the accuracy of the superimposition of CBCT images with digital intraoral scanning datasets on the one hand and the accuracy of the superimposition with digitally scanned gypsum models on the other hand. Subsequently, a recommendation for the digital workflow should be given. 270Materials and methods: A highly precise polymethacrylate model of an upper jaw with full dentition was scanned with a CBCT unit (3D eXam, Kavo dental GmbH, Biberach, Germany) for the digital superimposition with digital intraoral scanning data and lab-side scanning data. In this context, the same polymethacrylate model was also scanned with a TRIOS (3Shape A/S, Kopenhagen, Denmark) digital intraoral scanner first and with the D640 model scanner (lab-side model scanning unit, 3Shape A/S) in a second step. Each of these two procedures was performed ten times to ensure statistically relevant results. Afterwards, all datasets were converted into STL-files. Files obtained with TRIOS were assigned to group 1 and files obtained with D640 model scanner to group 2. Each scan of both groups was superimposed to the CBCT dataset throughout two different ways of local fitting (local-best-fit). Firstly, every scan was superimposed to the CBCT image using the virtual structures of the teeth. Secondly, the hard palate was used to define the relation. Three-dimensional deviations of every image pair were calculated for every locus over the teeth. Statistical analysis was done via IBMu00ae SPSS Statistics. Results: This study clearly showed that the scanning procedure which is used to reproduce the oral hard and soft tissue situation leads to different deviations in the superimposition with a CBCT dataset. Using a digital intraoral scanning unit, mean deviation was limited to 0.075mm. The lab-side scanning revealed a mean deviation of 0.129mm. By comparing these two ways of local-best-fit strategies, the relation over the teeth showed a mean deviation of 0.039 mm and the relation over the hard palate revealed a mean deviation of 0.165mm. The combination of these two examined parameters lead to the lowest mean deviation for the TRIOS scan datasets superimposed to the CBCT scan by using the local-best-fit over the teeth with 0.022mm and the highest for the lab-side scan superimposed by the relation of the hard palate with 0.203mm. All differences between groups and local-best-fit relations and combinations were statistically significant. Conclusion and clinical implications: The present data implicate a huge impact of the scanning method of the oral hard and soft tissue on the accuracy of digital superimposition to corresponding CBCT images. As immanent failure has to be reduced in clinical processes such as digital implant planning, the superimposition of a CBCT dataset with digital intraoral scanning datasets can be recommended regarding this worklflow by using a local-best-fit relation over the virtual structure of the teeth.
