Book excerpt: Foreign-born Latinos are at particular risk for underutilizing mental health services (Alegria et al., 2007b). The purpose of this study was to examine the roles of acculturation and stigma about mental disorder in the use of mental health services among West Coast Latinos, including foreign- and U.S.-born. Variables measured included acculturation, stigma, mental health status, and past-year mental health service visits to three types of providers: (a) medical; (b) specialty mental health; and (c) non-clinical. I accounted for the influence of mental health status as I evaluated the following hypotheses: (1) Foreign-born Latinos will report lower levels of acculturation and higher levels of stigma than U.S.-born Latinos; and (2) lower levels of acculturation and higher levels of stigma will correlate with lower mental health services use. Results indicated that foreign- and U.S.-born Latinos had used services to the same extent in the year prior and reported similar levels of mental health functioning. There was no evidence of a relationship between acculturation, stigma, and mental health services use. However, having insurance and lower education were shown to be predictors of making a mental health visit (p= .014). Unexpectedly, more U.S.-born Latinos had made at least one past-year visit to a non-clinical source than foreign-born Latinos (p= .043). Overall, this study demonstrated that, while cultural and attitudinal factors did not play a significant role in the use of mental health services, socioeconomic factors of having health insurance and lower educational attainment did.

Book excerpt: The factors associated with the health and social service utilization behavior of the elderly have been widely studied over the last 20 years. Little is known, however, about what factors are related to the mental health behavior of the elderly. Moreover, no research has tested causal models of formal mental health use among the minority elderly, or has evaluated when Latino elderly turn to others outside of the family for help. This study examines the influence of Predisposing, Enabling and Need factors on the utilization of the professional, the physician, and the church for mental health help by the Mexican and Mexican American elderly. The three types of formal mental health behavior were analyzed by regression analysis. The results for Mexican Americans indicated that use of a professional was the strongest predicted criterion measure, accounting for 19% of the variance, followed by use of the church at 12%, and use of a physician at 10%. The results for the Mexican elderly indicated that use of the church was the strongest predicted criterion measure, accounting for 23% of the variance, followed by use of a professional at 10%, and use of a physician at 8%. The findings indicated that Need was the strongest predictor across service type with greater variability among the Enabling and Need factors among both groups. This study suggests that the use of health and mental health services are similar because of their emphasis on Need factors. However, the greater variability among Enabling and Predisposing factors suggests that we should consider the heterogeneity among both the Mexican and Mexican-American elderly when designing mental health programs. More important, because of the multidimensional nature of the criterion variable, the establishment of an interactive and cooperative formal mental health network is imperative.

Book excerpt: The studies within this dissertation contribute to the existing scholarship by investigating social capital and acculturation factors as intersecting predictors of aging immigrants' health, thereby providing a more comprehensive understanding of health heterogeneity among aging immigrants. Specifically, the current study investigated how aging Asian and Latino Americans' self-rated physical and mental health is impacted by both social capital and acculturation factors. It also investigated the effect of social capital as a potential moderator and mediator of the relationship between the acculturation process and self-ratings of physical and mental health. Analyses were based on a sample of Vietnamese, Chinese, Cuban and Mexican adults, 55 years and older, from the 2002 0́3 2003 National Latino and Asian American Study, a nationally representative household survey of Latinos and Asian Americans. Study 1 investigated the structural model of social capital among immigrants while proposing that definitions of social capital among immigrants should consider socio-historical factors such as perceptions of discrimination. Results from exploratory factor analysis revealed a four-factor structure from variables hypothesized to indicate social capital. Results from a second-order confirmatory analysis showed that perceptions of discrimination were not significant contributors to social capital but social support from family, social support by friends, and neighborhood cohesion were significant factors. An alignment analysis confirmed that the resultant index could be used to compare social capital across the four ethnic groups. Study 2 analyzed the pathways through which social capital indicators impacted the relationship between acculturation factors and self-reported mental and physical health among aging immigrants. Findings from Study 2 supported the mediation hypothesis that acculturation (measured by length of residence in the U.S.) would predict social capital, which would, in turn, predict individual ratings of both physical and mental health. These results highlight that social capital partially explains the interaction between self-rated physical and mental health among the sample of aging Latino and Asian American immigrants. Moderation hypotheses were not supported by the data. Results not only aid in better understanding the measurement of social capital and its role in the acculturation-health relationship, but also clarify pathways between self-rated health, acculturation and social capital.
