Book excerpt: Introduction : infirmiers, aides-soignants et médecins généralistes sont au coeur de la prise en charge à domicile lors de l'accompagnement d'un patient en soins palliatifs. Ces situations sont toujours complexes et nécessitent une grand implication des soignants afin de toujours agir dans l'intérêt du patient et de le garder acteur de ses soins et de sa vie. Les objectifs de cette étude étaient de connaître le vécu des soignants face à ces situations si particulières et ainsi de proposer des pistes d'amélioration afin de favoriser au maximum ces prise en charges extra hospitalières dans le respect des choix du patient. Matériel et méthode : nous avons réalisé une étude qualitative s'adressant aux médecins généralistes, infirmiers libéraux et aides soignants intervenant à domicile sur le secteur de la Haute-Lozère. Nous avons recueilli leurs expériences par des entretiens individuels semi-dirigés puis analysé ceux-ci avec une approche phénoménologique intuitive du verbatim. Résultats : ces prises en charge sont spécifiques et complexes, elles nécessitent des connaissances théoriques et pratiques qui ne sont malheureusement pas ou peu retrouvées dans la formation initiale des soignants. Ce sont des accompagnements pour lesquels un investissement important de chacun des soignants envers son patient est indispensable ainsi qu'une bonne coordination entre professionnels de proximité et équipes spécialisées (HAD, EMSP). Ces équipes représentent des atouts et des supports mais peuvent entraîner un délai de prise en charge supplémentaire, dû à leur sous effectif. Le milieu rural est synonyme d'un isolement géographique mais aussi d'un soutien familial important favorisant le maintien à domicile. Conclusion : aujourd'hui, le patient est au centre de la prise en charge médicale et est acteur de ses soins. Il existe cependant un trop gros écart entre le souhait de rester vivre à domicile en soins palliatifs et la concrétisation de ce souhait. Il serait important de parfaire la formation initiale des médecins, infirmiers et aides-soignants dans ce domaine, leur permettre la prescription de certains anxiolytiques, les soutenir en augmentant les effectifs de l'HAD Lozère et de l'EMSP et favoriser la création d'un réseau de soins palliatifs local.

The most extensive section sets out detailed guidelines for the selection and prescribing of non-opioid analgesics, opioid analgesics, drugs for neuropathic pain, and adjuvant drugs for the treatment of adverse effects, the enhancement of pain relief and the management of concomitant psychological disturbances. Information ranges from explanations of how specific drugs work, through the precautions to take in the presence of certain disorders, to a list of factors that influence the effectiveness of opioids. Concerning the use of opioids, readers are reminded that psychological dependence does not occur in cancer patients and that the only correct dose of morphine is the one that relieves the pain. Part two provides a guide to opioid availability. A discussion of the reasons why opioids continue to be underprescribed or difficult to obtain is followed by an explanation of the Single Convention on Narcotic Drugs.
