Ils diffèrent de ceux du sujet âgé par leur étiologie. En effet, on retrouve 51% d'AVC ischémiques artériels (AVCIA), 5% de veineux et 44% d'AVC hémorragiques (AVCH), contre respectivement 80%, 5%, 15% pour le sujet âgé. L'absence de cause identifiée (43,3%) caractérise la majorité des AVCIA. Les cardio-embolies par anomalies interatriales contrastent par leur fréquence avec le faible nombre d'étiologie athéroscléreuse. Pour les AVCH, la 1ère cause est les malformations vasculaires cérébrales (65%). La fréquence importante des facteurs de risque vasculaires dans la population AVCIA souligne l'intérêt majeur de leur prévention. Le meilleur pronostic des AVC du jeune et de nouvelles thérapeutiques rendent nécessaire une prise en charge rapide dans des unités neurovasculaires aiguës.

Book excerpt: Les Accidents Vasculaires Cérébraux Ischémiques (AI) restent un problème de santé publique, avec une incidence forte dans la population en âge de travailler. Environ un tiers des AI du sujet jeune restent de cause indéterminée, rendant la prévention secondaire inadaptée. La consommation de toxiques pourrait être la cause de ces AI. L'objectif de notre étude épidémiologique à inclusion prospective sur 2 ans et analyse rétrospective chez les AI de moins de 55 ans était de déterminer la prévalence de la consommation de toxiques et les caractéristiques démographiques et cliniques des patients consommateurs. Nous avons montré une prévalence de 13,8% de la consommation de toxiques chez les AI de moins de 55 ans, avec un lien inverse avec l'âge, statistiquement plus élevée chez les moins de 45 ans. Le premier toxique retrouvé est le cannabis (11,9%), le deuxième la cocaïne (5,9% des 18-35 ans), semblant sur-représentés par rapport à la population générale. La moitié des patients testés positifs aux opiacés prenait des traitements opiacés connus, mais aucun patient ne consommait d'amphétamine. L'interrogatoire sur la prise de toxiques manquait le plus souvent, en lien avec l'âge. Les consommateurs de toxiques étaient plus jeunes, tous tabagiques, sans différence pour les facteurs de risque habituels, le type ou le territoire des AI, le délai au dépistage urinaire ou le taux de thrombolyse. Il semble important de réaliser systématiquement un interrogatoire sur la prise de toxiques chez les AI jusqu'à 55 ans, d'améliorer les délais de dépistage urinaire et de réserver ces dépistages aux AI de moins de 45 ans, sans recherche d'opiacés.

The excep tional achievements of mountaineers who have climbed nearly all peaks over 8,000 m without breathing equipment raise the question of maximal adaptation ca pacity of man to low oxygen pressures. More importantly, the increase in tourism in the Andes and the Himalayas brings over 10,000 people to sites at altitudes above 4,000 and 5,000 m each year. At such heights several kinds of high alti tude diseases are likely to occur, and these complications require detailed medical investigations. Medical authorities need to inform both mountaineers and tourists as to how great a physical burden can be taken in the mountain environment without risk to health. Physicians need to know what kind of prophylaxis is to be employed at high altitudes to prevent the development of diseases and what therapeutic measures should be used once high altitude diseases have occurred. Moreover, the physical condition of the indigenous population living at higher altitudes such as the Andes and the Himalayas, who are exposed continuously to the stress of high altitude, requires our attention. We have become familiar with symptoms characteristic of chronic high-altitude disease: under special conditions this popu lation has a tendency to develop pulmonary hypertension, which is associated with pulmonary edema, pulmonary congestion, and right heart failure.
