Book excerpt: In Kazakhstan, the HIV epidemic is largely confined to the injection drug user (IDU) population. Cultural stigma and discrimination discourage this population from seeking information on HIV testing, treatment, and prevention. The present study was designed to assess perceived barriers to seeking and accessing voluntary counseling and testing (VCT) and to examine factors that could account for those barriers. Participants were selected by a respondent driven sampling technique and were administered a standardized, face-to-face survey. The final sample included 1,071 IDU in Kazakhstan in 2006. Within the sample population, the mean age was 32.4 years (SD= 7.618), 80.4% were male, and 70.1% had reported being previously tested for HIV. A logistic regression model was developed to determine the influence of twelve common perceived barriers to HIV testing behavior. Three distinct barriers including fear of a positive result, the perception of more immediate problems, and inconvenient testing center hours predicted testing behavior. This analysis hopes to contribute to the development of HIV preventive approaches that may aid in creating effective programming and outreach efforts within the IDU population in Central Asia.

Book excerpt: Despite wide acceptance of Voluntary Counselling and Testing (VCT) as a crucial entry point for the prevention, treatment, care and support of HIV-infected persons; the prevalence of testing in Ethiopia remains very low. A review of the literature identifies the set of factors that influence HIV testing of individuals to include socio-economic and demographic factors, knowledge of HIV/AIDS, risk perception, fear of stigma, social capital and the perceived benefit of VCT. By drawing assumptions based on the known literature, this mini-thesis explores the relationship between VCT usage and the above factors in order to explain the status of HIV testing in Moher and Aklil district, Gurage Zone, Ethiopia. The study hopes to address the dearth of material on VCT and contribute to it in a non-clinical and rural context.

Book excerpt: Abstract Purpose: The purpose of this project was to create awareness of the importance of HIV prevention and testing among clients of St. Anthony's Foundation. While the aims of this project were to test different recruitment strategies and to propose a protocol for ongoing HIV awareness and testing at St. Anthony's Foundation through St. Anthony Medical Clinic. Methods: A key informant interview was conducted to assess the acceptability and feasibility of HIV voluntary counseling and testing among clients of St. Anthony's Foundation. Strategies for recruitment of participants was developed and implemented. Two hundred and five individuals were approached for HIV counseling and testing, and HIV testing was carried out on those who accepted. Result: Of the 205 individuals spoken to, seven carried out HIV testing and there was 14.28% seropositive in the population, within the ages of 30-39, homosexual, had multiple sex partners and injection drug user. One hundred percent of participants reported ever having sex without a condom while 28.58% reported ever using PrEP for prevention. Discussion: Voluntary counseling and testing (VCT) is a good tool for knowledge of HIV status and prevention. However, good strategies for recruiting this population is important for the success of VCT in any given population. Conclusion: The importance of HIV voluntary counseling and testing among various populations in society can never be overemphasized. The HIV prevalence in this population shows that there are hidden positive cases among hard to reach populations.

Book excerpt: Over the past twenty years, many low- and middle-income countries have experimented with health insurance options. While their plans have varied widely in scale and ambition, their goals are the same: to make health services more affordable through the use of public subsidies while also moving care providers partially or fully into competitive markets. Until now, however, we have known little about the actual effects of these dramatic policy changes. Understanding the impact of health insurance-based care is key to the public policy debate of whether to extend insurance to low-income populationsand if so, how to do itor to serve them through other means.

Available in PDF, EPUB and Kindle. Book excerpt: The World Development Report 2006: Equity and Opportunitypresents a social development strategy organized around the themes of social inclusion, cohesion, and accountability. It examines equality of opportunities--a potentially important factor affecting both the workings of the investment environment and the empowerment of the poor--by building on and extending existing accountability frameworks presented in the 2005Report. TheReportis divided into three parts. Part I describes patterns of inequality in a range of variables both at the national and global level-incomes, educational achievements, health indicators, power, and influence. Part II highlights reasons why some levels of inequality in the variables presented in Part I may be too high-whether for intrinsic reasons or because they harm the attainment of competing values, such as the level of goods and services in the economy. Part III discusses policies that affect the relationship between equity and the development process at a national and global level. This section includes policies that could help reduce the levels of some intermediate inequalities and focuses on circumstances in which these polices form the basis for more rapid overall development and faster poverty reduction. Now in its twenty-eighth edition, theWorld Development Reportoffers practical insights for policymakers, business developers, economic advisers, researchers, and professionals in the media and in non-governmental organizations. It is also an essential supplement to economic and development courses in both academic and professional settings.
