The magnitude of these changes is important for decision-making and comprehensive treatment planning, with provisions for possible solutions to expected complications during prosthetic rehabilitation. Objectives: to review all English dental literature to assess the magnitude of dimensional changes of both the hard and soft tissues of the alveolar ridge up to 12 months following tooth extraction in humans. Materials and methods: An electronic MEDLINE and CENTRAL search complemented by manual searching was conducted to identify randomised controlled clinical trials and prospective cohort studies on hard and soft tissue dimensional changes after tooth extraction. Only studies reporting on undisturbed post-extraction dimensional changes relative to a fixed reference point over a clearly stated time period were included. Assessment of the identified studies and data extraction was performed independently by two reviewers. Data collected were reported by descriptive methods. Weighted means and percentages of the dimensional changes over time were calculated where appropriate. Results: The search provided 3954 titles and 238 abstracts. Full text analysis was performed for 104 articles resulting in 20 studies that met the inclusion criteria. In human hard tissue, horizontal dimensional reduction (3.79 0.23 mm) was more than vertical reduction (1.24 0.11 mm on buccal, 0.84 0.62 mm on mesial and 0.80 0.71 mm on distal sites) at 6 months. Percentage vertical dimensional change was 11-22 % at 6 months. Percentage horizontal dimensional change was 32% at 3 months, and 29-63% at 6-7 months. Soft tissue changes demonstrated 0.4-0.5 mm gain of thickness at 6 months on the buccal and lingual aspects. Horizontal dimensional changes of hard and soft tissue (loss of 0.1 mm to 6.1 mm) was more substantial than vertical change (loss 0.9 mm to gain 0.4mm) during observation periods of up to 12 months, when study casts were utilised as a means of documenting the changes. Conclusions: Human re-entry studies showed horizontal bone loss of 29-63% and vertical bone loss of 11-22% after 6 months following tooth extraction. These studies demonstrated rapid reductions in the first 3-6 months that was followed by gradual reductions in dimensions thereafter. Book excerpt: Exodontia, or extraction of teeth, has been a well-documented dental treatment that forms one of the foundations of dentistry. The steps associated with extracting teeth have changed little in the last century and these steps are largely part of the dogma of dentistry. One such step is that of socket compression post-extraction. Rationale for socket compression after extraction is manifold. They include: shorter healing times, fewer dry sockets and re-approximating walls that were stretched in the elevation and delivery stages of extractions. The purpose of this study was to determine if post-extraction ridge compression negatively affected alveolar ridge dimensions when compared to sites that are not compressed post-extraction. Secondary outcome measures will identify if socket compression/re-approximation affects the rate of soft tissue closure or occurrence of alveolar osteitis. In this study, 14 subjects were recruited. Eight subjects formed the compression group, while six formed the non-compression group. The subjects in the compression group received compression of their alveolar ridges after extraction to approximate their original pre-extraction width. The subjects in the non-compression group did not receive ridge compression. Each subject had pre-extraction and post-extraction CBCT scans along with post-operative follow up visits at 1, 2, and 4 weeks post-extraction. The present investigation found that with respect to changes in ridge width, sites that were compressed did not lose significantly more dimension than those that were not. With respect to ridge height, sites that were compressed did not lose significantly more dimension than those that were not. Sites that were compressed and sites that were not, healed at approximately the same rate, with respect to soft tissue closure. While the results showed a lack of statistical significance between both groups, there appears to be a trend towards the ridge compression group having a smaller ridge width. Such a trend was not noted with soft tissue closure, thereby invalidating the rationale for socket compression after extraction. One of the limitations of this pilot study is the small sample size. Further validation of these results must be done with a larger sample size in order to provide clinical guidance to dental practitioners.

Regenerative periodontal treatment goal is to restore functional periodontal support offering a valuable treatment alternative even for teeth with large periodontal destruction, which may be successfully treated and maintained in health for long periods. In most cases where teeth are extracted for periodontal reasons, implant therapy will demand large bone augmentation procedures. Lack of sufficient bone volume may prevent placement of dental implants. In extreme cases, large bone reconstruction is indispensable before implant placement can be performed. Although, most bone grafts are only able to fill and maintain a space, where bone regeneration can occur (“osseoconductive”), the ideal bone graft will also promote osseous regeneration (“osseoinductive”). Several bone augmentation procedures have been described, each, presenting advantages and shortcomings. Success of bone augmentation procedures depends on the presence of bone forming cells, primary wound closure over the augmented area, space creation and maintenance where bone can grow and proper angiogenesis of the grafted area. Factors that influence the choice of the surgical technique are the estimated duration of surgical procedure, its complexity, cost, total estimated length of procedure until the final rehabilitations may be installed and the surgeons’ experience. This special issue will have a definite clinical orientation, and be entirely dedicated to soft and hard tissue regenerative treatment alternatives, both in periodontal and implant therapy, discussing their rationale, indications and clinical procedures. Internationally renowned leading researchers and clinicians will contribute with articles in their field of expertize.
