The microscopic examination of the uterine scrapings showed malignant disease. The exact nature of the malignant disease was in doubt, but it was supposed to be sarcoma, as will be explained in the pathological report. When the fact of malignancy was established, I, of course, advised immediate radical operation and a few days later performed a vaginal hysterectomy. The uterus was turned over to Dr. F isch for further examination and I have asked him to present, in person, his findings in the case. The patient recovered without incident and returned to her home. She has regained her former strength and weight, feels well, does her own housework and, so far, there is no evidence of recurrence. Book excerpt: Excerpt from The New System of Gynaecology, Vol. 2 of 3 Acute Vulvitis. - Cause. - Acute vulvitis is generally gonorrhoeal in origin, but it is occasionally due to injury caused by a douche, the temperature of which was too high or the chemical it contained too strong, or to the septic infection of a wound of the vulva caused during labour or otherwise. Symptoms. - The patient complains of a profuse discharge and of heat, swelling, and pain in the vulva. Pain and swelling in the groins and scalding on micturition may also be present. Signs. - The labia are swollen, tender, and red, and there is a profuse purulent discharge covering the affected parts. The orifices of the urethra and Bartholin's ducts may be inflamed and red. The Special characters of gonorrhoeal vulvitis are described in the Article on Gonorrhoea (vol. I. P. Diagnosis. - The nature of the infecting organisms should be ascertained by a bacteriological examination Of a specimen of the discharge. Apart from the detection of the gonococcus, gonorrhoea may be suspected if the vulvitis was sudden in its onset and the inflammation very acute, so that the pain and swelling are particularly marked. A very profuse and green discharge is highly suggestive of gonorrhoea, and infection of the inguinal glands, urethra, and Bartholin's ducts all point strongly to such an origin. The complications mentioned in Vol. I. P. 574 are also liable to follow an attack of gonorrhoeal vulvitis. According to Levret, it equals in gravity the other eruptive fevers, and is almost always accompanied by abortions and premature delivery. Grisolle and Cazeaux, on the other hand, observed two cases in which the preg nancy pursued its course. Bourgeois agrees with Levret. Among fif teen cases observed by him, he noted eight abortions or premature births of the eight children, five were non-viable (born before seven months) three at the seventh or eighth month. In women who were only between the second and fifth month of pregnancy, the disease pursued its usual course, but it became more severe as the pregnancy was more advanced. The premonitory symptoms of abortion appeared towards the end of the disease; as a rule, delivery did not occur until from one to three days later, and Sometimes the disease had terminated. When women were attacked with the disease at the end of pregnancy, premature delivery took place at the outset, accompanied by fever and serious symptoms. The foetus was dead-born, or perished within a few hours or days after birth. The disease pursued its course and the women slowly recovered. In rare instances the children were born with measles. Bourgeois saw one case, the infant being born fifteen days before full term and living only three days. Gautier collected eleven cases, in six of which the children presented the morbillous eruption. Measles occurring during pregnancy, therefore, may predispose to the death of the foetus and to abortion; it does not endanger the mother. The prognosis during the puerperal state is less grave.
