However, over the ten years since the epidemic had taken hold, it was clear that paradigms were limited, input was uncritical and large cohorts were overlooked. In this text a series of contributions have been compiled to explore adolescent issues ranging from sexual behaviour and health education campaigns to HIV prevention and HIV/AIDS care. The chapters begin by giving an overview of adolescent problems, such as homelessness, pregnancy and gender, and explore why these problems are so often overlooked. We then move on to an examination of the facts and fictions associated with adolescent risk, challenging some of the basic current notions underpinning approaches to the subject at the time. Also included are particular focused studies of Australian adolescents’ beliefs about HIV and STDs and also the American adolescents’ perceptions of drug injection. Finally, the volume gives a focused view of those with HIV infection, with a review of findings of the time, neuropsychological and psychological factors. Book excerpt: Among those that face the greatest risk for contracting Human Immunodeficiency Virus (HIV) are black adolescents between the ages of 13 and 24. Nearly one-third of new infections occur among this demographic. Among all youth, black men have higher rates of contracting HIV in comparison to any other race/ethnicity. This study focuses on the roles of the family, primarily parental communication and relationship factors, and school, primarily peers, and teachers, and how they are associated with sexual health behaviors (condom use, HIV testing, etc.); their impact, in short, on shaping health behaviors. This dissertation study examines the following questions among Black adolescent males: (1) Are there mediational relationships between parent support, self-efficacy beliefs about sex, sense of belonging at school, parent relationships, parent attitudes about sex, peer knowledge, racial discrimination, and HIV sexual risk behaviors? (2) Are self-efficacy, sense of belonging, parent support, parent attitudes towards sex, parental relationships, and peer knowledge predictive of HIV testing (ages 14-26)? Using longitudinal data from the National Longitudinal Study of Adolescent to Adult Health (Add Health). Add Health is designed to explore health and social context from adolescence to adulthood (Carolina Population Center). Includes a nationally representative sample of U.S. adolescents from grades 7 through 12, over four waves (1994-95, 1996, 2001-02, 2008 ) and collects data on respondents' social environment, family environment, behaviors and choices, physical health, education, goals and achievements. The main analytic strategy to addressing research question was a multinomial analysis and a structural equation model. The results indicated that parent support positively predicted HIV testing among Black males at waves 1 and 3. Results also indicated that parent attitudes was negatively associated with males being tested at both waves 1 and 3. These results our important because the eco-developmental theory suggests that microsystems are the most influential with respect to adolescent HIV/Sexually transmitted disease risk behaviors now extending to HIV testing.

Book excerpt: Adolescent females in the United States and around the world are at an increase risk for human immunodeficiency virus (HIV). Understanding the factors that increase adolescent females vulnerability to HIV infection is necessary for the development of an effective HIV prevention program. The objective of this proposed research grant is to implement a community based HIV prevention program for adolescent females intended to change knowledge, attitudes, and behavior related to safe sexual practices. A quantitative experimental pre-test/post-test control group design will be utilized for the proposed research study. Two hundred and sixteen adolescent females aged 13 to 18 years of age who live in San Diego County will be recruited to participate in the study. The Health Belief Model (HBM) will be used as the framework for the intervention. Both the intervention group and the control group will receive six 60-minute sessions provided by four trained female healthcare facilitators. The intervention group will receive education on HIV infection and skill building activities necessary to practice safe sex. The control group will receive education on sexual health, healthy relationships, and keeping a healthy body. The intended outcome of the proposed study is to see if there is a change in knowledge, attitudes, and behavior related to safe sexual practices in adolescent females after completing a HIV prevention program. Outcomes of the intervention will be evaluated using four instruments: (1) HIV-Knowledge Questionnaire for adolescent girls (HIV-KQ AG), (2) Adolescents’ Attitudes: AIDS Precautions Scale (3) UCLA Multi-dimensional Condom Attitudes Scale (MCAS), and (4) An Adolescent and Young Adult Condom Self- Efficacy Scale. The proposed study is significant to nursing because the advanced practice nurse (APN) is a major provider in caring for the adolescent population. This gives the APN a valuable opportunity to provide HIV prevention education that is crucial for the adoption of lifelong attitudes, skills, and behavior that support the overall health and well being of adolescent females

Too often, prompted by technical and scientific progress, we have ignored even marginalized-the vital "human element" in health thinking and prac tice. Thus, for example, while family planning programs focused on providing a choice among safe and effective contraceptive methods (a supremely worthy goal), the central issue of sexuality and sexual behavior was generally neglected. Similarly, the enormous and important efforts to develop rapid and reliable diagnostic and treatment methods for sexually transmitted diseases helped divert attention away from the crucial issues of sexual practice. In short, we seem to have difficulty addressing the fundamental behaviors-including sex, drug taking and other intoxications, and violence-that are central to the major causes of preventable morbidity, disability, and premature mortality in the world today. Our collective reluctance to examine and understand ourselves is also expressed in the oft-repeated pipedream that scientific progress will "take care of" the HIV / AIDS pandemic by delivering a preventive vaccine, an effective cure, or both. Yet even a cursory glance at the relationship between scientific/ technical progress and health shows that meeting the scientific challenges is only one step toward effective application of the vaccine or drug. It is typical, not atypical, that hepatitis B vaccine is only now becoming relatively freely available to large populations in the developing world, more than a decade after the vaccine's licensure.
