Book excerpt: In this study, mental health therapists' (n = 42) perceptions about the personal characteristics essential to a client's successful therapeutic experience were examined to determine whether traditional gender role stereotyping was operative in the therapists' perceptions of clients. The client's sex and the therapist's sex, age, and years of experience in the field were investigated to determine whether these variables affected the therapists' perceptions of the personal characteristics necessary for success in therapy. An electronic survey for either a hypothetical male or female client composed of the Bem Sex Role Inventory (BSRI) (1974) 60 personal characteristics and 12 demographic questions was sent to participants. The participants were asked to rate the BSRI personal characteristics as to whether the characteristic was "essential," "very useful, but not essential," "somewhat useful, but not essential," or "not necessary" for the specified hypothetical client to have a successful therapeutic experience. Findings indicated therapists in this study did not endorse traditional gender role stereotypes for a hypothetical male or female client. There was no significant interaction found between item type and therapist gender or client gender, and neither therapist age nor therapist years of experience appeared to have an appreciable moderating effect on the therapists' ratings of item types. Lawshe's content validity analysis identified both "essential" personal characteristics for a client to have a successful therapeutic experience and personal characteristics judged to be "not necessary." Limitations of the study are discussed and suggestions for future research are offered.

Book excerpt: The aims of this research is to 1) quantitatively assess therapist perceptions of prototypically difficult and successful clients and assess whether characterizations of such clients vary as a function of therapist level of experience and sex, 2) examine whether clients who characterize themselves as more similar to the prototype show different rates of change in psychotherapy, and 3) evaluate whether clients' self-reported personality and attitudes change in psychotherapy become more or less similar to the prototype profiles. There were no differences in prototypical difficult and successful client profiles as a function of therapist sex or level of experience. Clients' improvements in psychotherapy were not moderated by clients' similarity to prototype difficult or successful profiles. There was some suggestion that clients' personality profiles 30 weeks into therapy were more like the prototype successful personality profile compared to their personality profiles at baseline (p = .058). Clients' attitude profiles appeared to move away from the prototype difficult attitude profile and towards the prototype successful attitude profile (ps 9́Þ .001). These results suggest that, how similar a client is to therapists' perception of a prototype difficult or successful client does not impact their progress in therapy, but clients change to become more like the prototype successful client and less like the prototype difficult client during the course of psychotherapy.

Book excerpt: Estimates indicate that as many as 1 in 4 Americans will experience a mental health problem or will misuse alcohol or drugs in their lifetimes. These disorders are among the most highly stigmatized health conditions in the United States, and they remain barriers to full participation in society in areas as basic as education, housing, and employment. Improving the lives of people with mental health and substance abuse disorders has been a priority in the United States for more than 50 years. The Community Mental Health Act of 1963 is considered a major turning point in America's efforts to improve behavioral healthcare. It ushered in an era of optimism and hope and laid the groundwork for the consumer movement and new models of recovery. The consumer movement gave voice to people with mental and substance use disorders and brought their perspectives and experience into national discussions about mental health. However over the same 50-year period, positive change in American public attitudes and beliefs about mental and substance use disorders has lagged behind these advances. Stigma is a complex social phenomenon based on a relationship between an attribute and a stereotype that assigns undesirable labels, qualities, and behaviors to a person with that attribute. Labeled individuals are then socially devalued, which leads to inequality and discrimination. This report contributes to national efforts to understand and change attitudes, beliefs and behaviors that can lead to stigma and discrimination. Changing stigma in a lasting way will require coordinated efforts, which are based on the best possible evidence, supported at the national level with multiyear funding, and planned and implemented by an effective coalition of representative stakeholders. Ending Discrimination Against People with Mental and Substance Use Disorders: The Evidence for Stigma Change explores stigma and discrimination faced by individuals with mental or substance use disorders and recommends effective strategies for reducing stigma and encouraging people to seek treatment and other supportive services. It offers a set of conclusions and recommendations about successful stigma change strategies and the research needed to inform and evaluate these efforts in the United States.

Book excerpt: This qualitative research study examined the impact of psychologists’ perceptions of their clients’ political views on the therapeutic interaction. A literature review including both current and classic studies was performed. Four themes emerged: the impact of Donald Trump on the therapeutic interaction, the nature of therapist self-disclosure about politics, therapists’ experience of political agreement and disagreement with clients, and the ethical concerns raised by political discussion between psychotherapists and their clients. To conduct this study, Moustakas’s (1994a) transcendental phenomenological approach was employed. The study relied upon semi-structured interviews with 13 fully-licensed, doctoral-level psychologists who practiced psychotherapy and were also registered voters in the United States. The interviews yielded four textural themes, including that participants believed that Donald Trump’s presence in the American political landscape generated significant impact on the therapeutic interaction, participants felt challenged when working with clients whose political views diverged from their own, participants focused on the client’s treatment goals rather than on politics, and participants varied in their management of political self-disclosure. The themes were then examined on a structural level, which include bodyhood, materiality, causality, relationship to self, relationship to others, spatiality, and temporality. The findings of this study showed that participants successfully navigated political discussions with clients, despite the fact that participants experienced challenges raised by clients’ political expression. These challenges did impact the therapeutic interaction. The results suggest that more formal training regarding political discussion in psychotherapy is warranted for clinicians.
