However, the medical treatment for such diseases differed significantly, depending on the class of patient. The wealthy received private medical treatment at home or, later, in a practitioner's consulting room. The middle classes might also pay for their treatment but, in addition, they could attend one of an increasing number of specialist hospitals. The working classes could get free treatment from charitable voluntary hospitals or dispensaries. For the abject poor who were receiving poor relief, their only option was to seek treatment at the workhouse infirmary. The experience of a patient going into hospital at this time was vastly different from that at the end. This was not just in terms of being attended by trained nurses or in the medical and surgical advances which had taken place. Different methods for treating diseases and the use of antiseptic and aseptic techniques to combat killer hospital infections led to a much higher standard of care than was previously available.

Book excerpt: Mutualism and health care, newly available in paperback, presents the first comprehensive account of a major innovation in hospital funding before the NHS. The voluntary hospitals, which provided the bulk of Britain’s acute hospital services, diversified their financial base by establishing hospital contributory schemes. Through these, working people subscribed small, regular amounts to their local hospitals, in return for which they were eligible for free hospital care. The book evaluates the extent to which the schemes were successful in achieving comprehensive coverage of the population, funding hospital services, and broadening opportunities for participation in the governance of health care and for the expression of consumer views. It then explores why the option of funding the post-war NHS through mass contribution was rejected, and traces the transformation of the surviving schemes into health cash plans. This is a substantial investigation into the attractions and limitations of mutualism in health care. It is highly relevant to debates about organisational innovations in the delivery of welfare services.

Book excerpt: Providing the first ever statistical study of a professional cohort in the era of the industrial revolution, this prosopographical study of some 450 surgeons who joined the army medical service during the Revolutionary and Napoleonic wars, charts the background, education, military and civilian career, marriage, sons' occupations, wealth at death, and broader social and cultural interests of the members of the cohort. It reveals the role that could be played by the nascent professions in this period in promoting rapid social mobility. The group of medical practitioners selected for this analysis did not come from affluent or professional families but profited from their years in the army to build up a solid and sometimes spectacular fortune, marry into the professions, and place their sons in professional careers. The study contributes to our understanding of Britishness in the period, since the majority of the cohort came from small-town and rural Scotland and Ireland but seldom found their wives in the native country and frequently settled in London and other English cities, where they often became pillars of the community.
