Quality improvement methodology when implemented can narrow quality gaps. Objective: The objective of the project was to create and implement a plan of action to address identified gaps in key clinical activities of asthma care among pediatric population in a private pediatric setting in Northern California Methods: The project was centered on the use of Education in Quality Improvement for Pediatric Practice (EQIPP), a program of the American Academy of the Pediatrics. Both the pediatrician and the DNP student took this course and employed its methods to improve asthma management. EQIPP supports providers in improving their practice with didactic materials that help participants develop quality improvement project and tools to evaluate the outcomes of that project. Results: Based on the asthmatic patient data analysis the quality improvement team identified that the clinic lacks compliance in the following areas of national guidelines. a) Diagnosis of asthma, b) Asthma action plan and c) Asthma control and follow up. The team then developed and implemented an improvement plan based on EQIPP. Conclusion: The quality improvement project enriched the pediatric practice management of asthma patients and similar projects could be implemented in other settings too. There is a clear clinical need for better asthma management programs in the pediatric primary care setting (Akinbami et al., 2011; Sleath et al., 2011). The current system for asthma management in primary care offices is not improving outcomes for pediatric asthma patients in the United States or the state of Montana. Barriers to providing comprehensive evidence-based pediatric asthma care exist in the primary care setting nationwide (US DHHS, 2010; & Meyer, 2011). The creation of an evidence-based pediatric asthma toolkit for the primary care provider based on the EPR-3 Asthma Diagnosis and Management Guidelines (NIH, NHLBI, 2007) is an attempt to alleviate prevalent barriers primary care providers face in pediatric asthma care. A toolkit was assembled based on literature reviewed, interviews with experts in the field, and suggestions from primary care providers after reviewing the tool. The toolkit was found to be evidence-based, appropriate for use in the primary care setting, efficient for use in a busy primary care setting, and desired for use by primary care providers in the state based on informal interviews and reviews. It is suggested that the Pediatric Asthma Toolkit for the Primary Care Provider be tested using a formal research study for its' effectiveness in improving pediatric asthma care in the primary care setting as well as improving asthma outcomes in the state of Montana. More research needs to be done in the U.S. to evaluate the effectiveness and use of asthma toolkits by primary care providers. The toolkit will need to be updated when new research is published on asthma care as well as when new national asthma guidelines are issued. The toolkit will also need to be updated with current available pharmaceuticals and devices for asthma. This professional project is in alignment with the national Asthma Control Goals for Healthy People 2020 as well as the National Asthma Education and Prevention Program recommendations for implementation of the EPR-3 Guidelines. It is also in alignment with the Montana Asthma Control Program's goals to create partnerships throughout the state and improve asthma outcomes for Montana.

Book excerpt: Children presenting to Emergency Departments with difficulty breathing related to asthma should be assessed and treated quickly to prevent deterioration of their condition. The purpose of this Direct Practice Improvement project is to determine if the initiation of a standardized asthma scoring tool to identify the need for a nebulized short acting B2 agonist (SABA) treatment, will decrease the time a patient with an asthma exacerbation arrives in the ED, to treatment time. Because no standardized system exists in the Pediatric ED to assess and categorize symptoms of asthma such as wheezing, retractions, respiratory rate, oxygen saturation and level of dyspnea, care is fragmented and inconsistent. Approaching care using Betty Neumann's health care systems model to reduce stress of the parent and child can be helpful in situations of those suffering breathing difficulties. Nursing staff were educated on a specific asthma severity scoring tool for assessing children with breathing difficulties to allow consistency and judicious treatments. This quantitative project measured time to treatment pre and post process improvement with children ages 2-17 presenting with difficulty breathing due to asthma. Data analysis using a bar chart showed an increase in the goal of 20 minutes to nebulizer post process implementation by 33%. A Ï72 test using age categories was executed that showed children ages 2-10 experienced longer time to nebulizer as compared to ages 10-17. Finally, a two-tailed t test performed showed a decrease to nebulizer mean time by 24.92 minutes post process execution as well as a p-value of 0.0043. The data analysis shows a significance in improvement of time to nebulized albuterol treatment overall with the process change therefore this asthma severity scoring tool is now a part of the respiratory assessment in the new EHR. Going forward, monthly chart audits are performed accessing time to nebulizer and ongoing education as needed.

Book excerpt: Asthma is the most commonly diagnosed chronic disorder in childhood and is linked with several problematic outcomes including frequent school absences, increased hospitalizations and decreased quality of life. Further, urban populations struggling with low socioeconomic status are disproportionately represented in prevalence statistics and suffer from increased functional morbidity relative to other children with asthma. These findings exist in the midst of largely effective pharmacological interventions. Asthma self-management programs (SMPs) target several behaviors linked to improved outcomes and are often used as an adjunct to medication management. SMPs have been employed using a variety of techniques and treatment targets in a wide range of settings. Data suggests that SMPs have a mild to moderate impact on functional morbidity outcomes and merit further research. In order to facilitate efficacious interventions to those at highest risk of problematic outcomes, common barriers impinging on program attendance must be alleviated. The school setting proves to be an ideal location to deliver SMPs due to their accessibility and available resources. The current project evaluates a school-based asthma SMP delivered in an urban setting. Results reveal statistically significant increases in quality of life and child reported knowledge as well as decreased utilization of urgent care outpatient treatment. Trends towards decreased agreement between children and caregivers on allocation of asthma management tasks reflect challenges with implementation of programs in the absence of frequent parental contact. School-based asthma SMPs afford several benefits to high-risk populations but also must reflect the need for family involvement in successful asthma management.
