Book excerpt: Patients coming into a facility for invasive procedures often times have a high level of anxiety about the procedure and the outcome, (Ghetti, 2012). Based on research studies, the theory of music therapy intervention can reduce the patient's anxiety before and during an invasive procedure, (Bradt, Dileo and Shim, 2013). Music is a welcome distraction proven to help the patient relax, lower blood pressure, heart rate, respiratory rate and cover up environmental noise, (Comeaux and Steele-Moses, 2013). Data reveals some patients need less sedation and pain medication to lessen base-line vital signs and anxiety levels if music therapy is used, (Matsota, Christodoulopoulou, Smyrnioti, Pandazi, Kanellopoulos, Koursoumi, E., and others, 2013). By implementing music therapy in phases through out the facility, education can be audited through the PDCA model, Plan Do Check Act for process improvement. Starting in the preoperative preparation area for patients having heart catheterizations will allow ample control over the study with less staff to train in the start of the project. This plan allows for the implementer to oversee the study with the assistance of the research committee. Patients will be educated on the trial when checking in for the procedure and given an option to participate. A VAS-A, Visual Analog Scale for Anxiety will be used before and after the procedure, (Abend, Dan, Maoz, Raz, and Bar-Haim, 2014). Patient satisfaction scores will be analyzed before and after the trial. The ultimate goal is to improve patient satisfaction scores by reducing level of anxiety, with the added benefit of possibly lowering cost per case. As evidence of less sedation involved allowing the patient to be dismissed sooner, (Bernatzky, Presch, Anderson and Panksepp, 2011).

The physical implications of stress and anxiety have been recognized as having a negative influence on intra procedure vital signs and even recovery time (Bringman and others, 2009). These facts play a major role in the holistic view of a patient, and result in stressors described in the Neuman Nursing Theory as an imbalance in the patient09́s internal line of defense (Gonzalo, 2011). Several research studies have indicated a clear correlation between music intervention and a reduction in pre-procedural anxiety (Nilsson, 2008), which is used as the basis for this research study . The experimental group is subjected to instrumental, calming music for half hour prior to their procedure, and natal signs and anxiety levels are measured via the Beck Anxiety Inventory (BAI) questionnaire before and after music. The control group is likewise filling out the BAI questionnaire and having their vital signs measured, but without music therapy. The testing and the results are thoroughly discussed with implicated stakeholders, as this inexpensive yet effective measure to reduce pre-procedural anxiety can be easily implemented in nursing practice.
