This study details a proposed eight-week treatment plan involving cognitive behavioral play therapy (CBPT) as an initial short-term intervention for Hispanic youths with substance abuse disorder. The study is based on the participation of these youths in individual cognitive behavioral play therapy sessions with time-limited psycho-educational counseling. While there exists no single best approach to treat these youths, a treatment modality that combines the reliable methods of cognitive behavior therapy and play therapy may offer clients a short-term way to seek relief of their symptoms without the need for hospitalization or the use of in-patient residential treatment programs.

Hispanic adolescents in substance abuse treatment programs have been found to differ from non-Hispanic Caucasian adolescents in the relationship of their delinquent behavior to psychoactive substance involvement. This study examined differences in posttreatment substance use and delinquent behaviors in Mexican American and non-Hispanic Caucasian adolescents during the initial six months following treatment for substance abuse. In addition, the impact of acculturation and cultural specific stress on the outcomes of Mexican American substance abusing adolescents was explored. The study included 40 Mexican American and 230 non-Hispanic Caucasian teens entering inpatient adolescent alcohol and drug treatment programs. Participants were interviewed during the treatment program and at six months following discharge. A resource person was interviewed as a corroborative informant. The Customary Drinking and Drug Use Record (CDDR), the Relapse Review and the Conduct Disorder Questionnaire (CDQ) were administered to assess psychoactive substance involvement, aftercare meeting attendance and delinquent behavior over the six month period of the study. Mexican American subjects also completed the Acculturation Rating Scale for Mexican Americans (ARSMA-II) and a subscale of the Hispanic Stress Inventory (HSI) measuring cultural and family conflict. Hierarchical multiple regression was used to test the interaction between ethnicity and delinquency in predicting posttreatment substance use. The relationship between acculturation and cultural stress on posttreatment outcome was also explored using regression and correlation. While Mexican American and Caucasian adolescents demonstrated similar rates of relapse and abstinence during the six months following substance abuse treatment, Mexican American adolescents had more intense substance use patterns and more negative consequences for alcohol and drug relapse. Additionally, compared to Caucasian youth, Mexican American adolescents displayed a stronger relationship between delinquent behavior and substance use intensity during the six months after treatment. Acculturation had an influence on outcomes for Mexican American adolescents, with Anglo orientation negatively correlated with posttreatment substance use, and overall acculturation related to likelihood to attend AA/NA meetings.

The prevalence of these two mental health problems among Hispanic adolescents have been identified by the National Monitoring the Future School Survey and the Youth Risk Behavior Surveillance statistics which indicated that Hispanic adolescents reported higher levels of alcohol and illicit drug use and disruptive behaviors than non-Hispanic White adolescents. In addition, Hispanic adolescents were founded to be the second highest ethnic group after African American adolescents in reporting disruptive behaviors. Moreover, Hispanic adolescents face more socio-cultural risk factors than non-Hispanic White adolescents. In the literature, Problem Behavior Theory has been validated among minorities groups which indicate that substance abuse and conduct disorders are explained by common risk and protective factors. Furthermore, Ecodevelopmental Theory has explained the dynamic of risk and protective factors among Hispanic adolescents in relation to individual, family, social, and cultural domains. Available interventions have targeted substance use and delinquent behaviors separately. However, no single intervention has proven itself effective in addressing the co-occurrence of these disorders while increasing resiliency skills in the Hispanic adolescents and their parents. The following doctoral project proposes a program design with the purpose of conducting a risk assessment and multidimensional family intervention in Hispanic adolescents at risk for substance abuse and conduct disorders. The program intends to improve academic achievement, school or vocational involvement and proactive parenting skills in Hispanic adolescents and their families.
