A complete chronological history of its visitations was regarded by the Council of the Sydenham Society as calculated to be of value, in various ways, to medical practitioners; enabling them to compare the disorder, on any occasion of its recurrence, with those previous epidemics which it may most resemble, and suggesting particulars requiring further investigation, whenever such an opportunity is renewed. The information extant on the subject was not readily accessible, being sometimes, as it were, concealed in volumes devoted chiefly to other topics, or otherwise scattered through the pages of periodicals long since discontinued. My father, in endeavouring to fulfil the difficult but honourable task with which he had been entrusted by the Council, spared no pains in the search, or care in the selection, of materials; and he shared with the Council a desire to present the facts which are recorded, as far as possible, in the words of the original observers, although, especially as respects the visitation of 1803, the miscellaneous character of the communications has rendered such an attempt by no means easy. He had much satisfaction in being the first to put into an English form the classical production of Sir George Baker. Book excerpt: Excerpt from Observations Upon the Natural History of Epidemic Diarrhoea The causative agencies from which springs the plentiful harvest of child mortality in large cities may be conceived as a felted mass of rootlets almost inextricably intertwined. Those of poverty, bad housing, bad feeding, and neglect, may be severally recognized, but the extent of their interrelations with actual respiratory and alimentary disease - those to which the greater part of the mortality is referred - can be traced only with difficulty. It is the object Of this work to aid ln the labour of cutting away the matrix and entangling fibres and to lay bare the hidden ramifications of at least one important causative agency - epidemic diarrhoea. This affection - if we accept provisionally the more novel and generally favoured conception. As to its nature - is revealed as something very like an ordinary infectious disease, and one which permeates all classes, while the excessive mortality it gathers round itself in urban centres must be regarded as something superadded, owing to the vicious circle it forms with those baneful conditions of slum life mentioned above. On the other hand, its peculiarly intimate association with the circum stances of domestic life, from the continual faecal pollution of the interior of the household by infants and others, tends to make it more so than other affections of the kind peculiarly a class disease, and an especial scourge of dirty neighbourhoods. Dirty towns may however be saved from excessive mortality by a high percentage of breast feeding. A notable point in the interesting comparison that can be drawn between diarrhoea and typhoid fever is that, in accordance with their peculiarly opposite age-incidence curves, the marked and habitual depositing of infectious excreta within the household in the former disease may make the question of water-closet versus conservancy pan a matter of far less importance than in the latter. The long delay in the appearance of this edition, since the sale of the first, has resulted from my desire to embody in it as far as possible the results of observations on the recent epidemics of Fever, made by other physicians as well as by myself. Although the original plan has been retained, the present edition is far from being a mere reprint of the first. Many parts of the work have been entirely rewritten. The statistical Tables in the first edition were based on 6,703 cases of Continued Fever admitted into the London Fever Hospital during ten years (1848-57), while those in the present edition are based on 28,863 cases admitted during twenty-three years (1848-70), comprising, in fact, the entire medical history of the Fever Hospital from the time that the different Continued Fevers were first distinguished in 1848, until, from the transfer of the pauper patients to the New Fever Asylums in 1871, the experience of the Fever Hospital ceased to be any test of the prevalence of Fevers in the metropolis. After the publication of the first edition, London was visited by great epidemics of Typhus and Relapsing Fever; the demands for admission into the Fever Hospital in consequence greatly increased, and to meet these demands the accommodation was more than doubled.
